AGREEMENT AMENDMENT BETWEEN
THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES
AND UNITED WAY OF WELD COUNTY
This Agreement Amendment, made and entered into Z t� day of 2022 by and between the Board
of Weld County Commissioners, on behalf of the Weld County Department of Human Services, hereinafter referred to as the
"Department", and United Way of Weld County, hereinafter referred to as the "Contractor".
WHEREAS the parties entered into an Agreement for the Volunteer Recruitment Initiative, (the "Original Agreement")
identified by the Weld County Clerk to the Board of County Commissioners as document No. 2021-0226, approved on January 20,
2021.
WHEREAS the parties hereby agree to amend the term of the Original Agreement in accordance with the terms of the
Original Agreement and any previously adopted amendment, which is incorporated by reference herein, as well as the terms
provided herein.
NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as follows:
• The Original Agreement will end on August 15, 2022.
• These Amendments, together with the Original Agreement, constitutes the entire understanding between the parties.
The following additional changes are hereby made to the current Agreement:
1. Term
This agreement is being renewed for an additional two-year term, for the period of August 16, 2022 through August 15,
2024.
• All other terms and conditions of the Original Agreement remain unchanged.

RESOLUTION
RE: APPROVE AGREEMENT FOR AGING WELL VOLUNTEER RECRUITMENT
INITIATIVE ORGANIZATION PARTICIPATION AND AUTHORIZE CHAIR TO SIGN -
UNITED WAY OF WELD COUNTY, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Agreement for Aging Well Volunteer
Recruitment Initiative Organization Participation between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, on behalf of the Department
of Human Services, and United Way of Weld County, Inc., commencing upon full execution of
signatures, and ending August 15, 2022, with further terms and conditions being as stated in said
agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Agreement for Aging Well Volunteer Recruitment Initiative
Organization Participation between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human Services,
and United Way of Weld County, Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 20th day of January, A.D., 2021.

RE: Agreement with United Way of Weld County for

Please review and indicate if you would like a work session prior to placing this item on the
Board's agenda.
Request Board Approval of the Department's Agreement with United Way of Weld County
(UWWC) for Aging Well Volunteer Recruitment Initiative. United Way of Weld County
(UWWC) has received two (2) years of funding to run a volunteer recruitment initiative that
would recruit volunteers for Weld County older adult serving organizations. Weld County Area
Agency on Aging (AAA) will be able to utilize volunteers who are recruited and referred by this
initiative. As such, AAA agrees to on -board and train any referred volunteers and provide
UWWC data related to the retention of volunteer participants. Additionally, AAA will help
promote the volunteer recruitment initiative and participate in related UWWC meetings.
This agreement is effective as of the date of execution through August 15, 2022 with the option
to extend the term. The County has no financial obligations under this Agreement.
Legal (G. Kalousek) has reviewed and approved the Agreement.
I do not recommend a Work Session. I recommend approval of this Agreement and authorize the Director
to sign.

ORGANIZATION PARTICIPATION AGREEMENT

This Agreement ("Agreement") is made thisWday of 2021 by and between Weld County
Area Agency on Aging a Colorado Governmental Jurisdiction ("AAA") PO Box 1805 Greeley, Colorado
80632 and United Way of Weld County, Inc. a Colorado Non Profit Corporation ("UWWC"), PO Box 1944,
Greeley, Colorado, 80632.
Background of Agreement. The following background statements are provided to aid in the
understanding and interpretation of this Agreement.
A. At the behest of the Aging Well Collective Impact workgroup, United Way of Weld County ("UWWC")
applied to the Colorado statewide funder NextFifty Initiative and was granted two years of funding
to run a volunteer recruitment initiative that would recruit volunteers for Weld County older adult
serving organizations.
B. Included with the application were letters of support from a number of Weld County organizations,
all of which stated their intent to work with UWWC on the project that they might benefit from
volunteers that would be recruited to work with their programs.
C. Definition of terms
1. Aging Well Volunteer Recruitment Initiative (VRI): The collection of entities including the
organizations which have signed the agreement, UWWC, and the volunteer recruitment
coordinator, which collectively are working to identify and recruit volunteers for the participating
organizations.
2. Aging Well Volunteer Recruitment Coordinator (VRC): The staff person hired by UWWC who will
oversee the VRI and coordinate between the participating organizations, potential volunteer
participants, and UWWC.
3. Participating Organization: All organizations which enter into an agreement to join the VRI will be
known as participating organizations.
4. Potential Volunteer Participant: Any person who has had interactions with the VRI by means of
conversation, a sign up form etc., but who has not formally began volunteering with a
participating organization.
5. Volunteer Participant: Any person, who having been referred to a participating organization
through the VRI, has finished on -boarding with the participating organization and begun actively
volunteering.
6. Program Participant: Any person served by a service provided through a participating
organization.

NOW THEREFORE in consideration of the mutual promises of the parties and good and valuable
consideration it is agreed as follows:
1. Term. The term of this Agreement shall be from the date of this agreement to August 15, 2022. This
term may be extended through verbal or written consent.
2. Volunteers will be recruited for nonprofit and governmental programs that serve Weld County older
adults aged 60+. Generally those recruited will be Weld County residents, though those of other
counties may also be included.
3. Related to this agreement, Weld County Area Agency on Aging shall:
a. Provide UWWC with what is required to promote AAA volunteer opportunities, including a high
resolution logo, language for describing the opportunity, print materials, staff time, etc.
b. Participate in VRI meetings both with the group as a whole and individual meetings between AAA
and the UWWC VRC.
c. Once a potential volunteer participant has been referred to AAA, AAA will provide the on -
boarding, training, and support in order for the volunteer participant to enter into and retain an
active volunteer position.
d. Provide UWWC with data relating to the retention of recruited volunteer participants and overall
satisfaction with the initiative.
e. Agree to some form of general promotion of the VRI, be that digital, print, or verbal; as made
possible by the governing by-laws and capacity of AAA.
f. Maintain Commercial General Liability insurance with at least a $1,000,000 combined single limit
coverage of bodily injury, property damage or combination thereof and $2,000,000 aggregate.
4. Related to this agreement, United Way of Weld County shall:
a. Hire a staff person to fill the role of VRC who will:
1) Develop marketing materials both print and digital to be used to advertise the VRI.
2) Develop and maintain a single entry point for volunteers interested in participating in the
VRI
3) Help build and maintain profiles and volunteer positions listed on the UWWC volunteer
management website known as Connect or Galaxy Digital
4) Interface with AAA in order to customize the information included in the single entry
point and on Connect to match the needs of AAA
5) Share information on potential volunteer participants identified through single entry
point and/or Connect
6) Follow up on placements to make sure the volunteer is a good fit and if not to help
relocate volunteer participants as needed
7) Provide volunteer management support and training as needed and/or requested

8) Make regular reports of progress to the Aging Well Collective Impact workgroup
b. Run a social media campaign to assist the recruitment effort of the VRC
c. maintain Commercial General Liability insurance with at least a $1,000,000 combined single limit
coverage of bodily injury, property damage or combination thereof and $2,000,000 aggregate
5. Nothing contained in this agreement shall be construed to create a joint venture or partnership
between AAA and UWWC.
6. Neither party has the authority to act for the other party, or to bind the other party in any respect
whatsoever, or to incur any debts or liabilities in the name of or on behalf of the other party.
7. Either AAA or UWWC may terminate this Agreement at any time for any reason with a 60 day notice,
which may be made by paper or electronic means.
8. AAA and UWWC agree to intentional and consistent ongoing communications about project status.
9. AAA and UWWC agree to make mention in public communications about the project.
10. Any prior agreements are no longer effective once this Agreement is signed by the parties.
