HomeMy WebLinkAbout20220910.tiffRESOLUTION RE: APPROVE ACCEPTANCE OF SUICIDE PREVENTION MINI GRANT AWARD FOR CHILD FATALITY PREVENTION SYSTEM (CFPS), AND SUMMARY OF TERMS AND CONDITIONS WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with the Suicide Prevention Mini Grant Award for Child Fatality Prevention System (CFPS), and Summary of Terms and Conditions between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Coroner's Office, and the Colorado Department of Public Health and Environment, Office of Suicide Prevention, commencing upon full acceptance, and ending June 30, 2022, with further terms and conditions being as stated in said grant award, and WHEREAS, after review, the Board deems it advisable to approve and accept said grant award, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Suicide Prevention Mini Grant Award for Child Fatality Prevention System (CFPS), and Summary of Terms and Conditions between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Coroner's Office, and the Colorado Department of Public Health and Environment, Office of Suicide Prevention, be, and hereby is, approved and accepted. Our role in accepting the grant will •include: 1. Utilize a standardized "Suicide Death Investigation Form" which will be provided. 2. Participate in local/regional suicide prevention coalition. 3. Provide materials to family impacted by suicide and 4. submit survey data for further study. What options exist for the Board? (include consequences, impacts, costs, etc. of options): 1. Approve accepting the funds and participate in the program. 2. Deny accepting the funds and not participate in the program. Recommendation: Considering our office already participates in such activities on behalf of CDPHE (Child fatality review board; violent death reporting; drug task force CDOT related statistics for motor vehicle fatalities) and have received similar small grants in the past I would appreciate approval of the funds. I think, if I could get a digital copy of the completed forms that were originally submitted, I can include them as backup. At this point, I would encourage the Coroner's Office to circulate a Pass Around memo with the Board, explaining the terms/provisions of accepting the grant award will be, and then I need to circulate the full packet through Legal and Finance prior to listing it on the Agenda for final acceptance by the Board via Resolution. I am not really sure if it is a grant since it more like an incentive payment to have them do several things, most of which they already do. Based on feedback received over the past year, we've updated the Suicide Investigation Form to improve flow and formatting to make it a more usable tool to gather information to help inform prevention efforts. The goals of this new mini grant funding include 1) increasing utilization of the Suicide Death Investigation Form, 2) bringing staff from coroner agencies and medical examiner offices into local or regional suicide prevention coalitions and working groups, 3) supporting suicide loss survivors, and 4) improving the Suicide Death Investigation Form. Urban coroner agencies and medical examiners offices can apply for up to $5,000.00 to be used towards costs incurred in the completion of the activities outlined in the application below.
