The Clinical and Community Health Division has been awarded a one-year Community Investment Grant in the amount of $25,000 by Northeast Health Partners. This grant funding was approved to support innovative programs designed to decrease health disparities and enhance health equity in Weld County. We will use this one-time grant funding to purchase equipment and supplies to support health education and clinical outreach in our Immunization, Sexually Transmitted Infections, Cancer Prevention and Early Detection/Women's Wellness Connection, Family Planning, WISEWOMAN and HPV grant programs to the underserved and vulnerable Medicaid and Medicaid -eligible populations in Weld County. The funds will be disbursed to WCDPHE in one lump -sum distribution upon acceptance of the award by the Chair and must be expended by June 30, 2023. Weld County established the second local health department in Colorado in 1938 and is the longest continuously operating local health department in the state. The Weld County Health Department achieved accreditation from the Public Health Accreditation Board in November 2016, making it one of eight agencies to earn this recognition to date in Colorado. The value of accreditation strengthens the Health Department to better serve Weld County residents through continuous quality improvement, partnerships, and accountability. The Weld County Health Department is organized into four service divisions: Administration and Finance; Environmental Health Services; Health Education, Communication, and Planning; and Clinical and Community Health. This grant would support outreach efforts for our clinical services grant programs funded by the Colorado Department of Public Health and Environment(CDPHE) and Weld County. Our Immunizations, Family Planning, STI, Cancer Prevention and Early Detection/Women's Wellness Connection (CPED/WWC), and WISEWOMAN grant deliverables all require that we reach out into the community to provide education and the delivery of services for vulnerable populations. The vast majority of patients that use our clinic are Medicaid, non-insured, and under -insured. We also serve many undocumented, immigrants, and refugees in our community. While we are not a primary care home, we are a healthcare safety net for many in our community. Our CDPHE grant deliverables require us to provide education and services to these vulnerable populations outside of our clinic. Recent evaluation of these programs, specifically looking at community outreach, has determined that we need to do more in the area of community outreach in terms of health education and the provision of clinical services such as screening, testing, immunizations, and treatment. As a result, we are now using our grant and county funding to create a Public Health Educator II position that will help us connect with community partners and coordinate the provision of our clinical services out in the community, specifically targeting areas where our vulnerable populations live, work, and recreate. While our clinical grant programs have been in operation in Weld County for decades, the new innovative initiative with this clinical outreach program is going to help us reach our most vulnerable populations in a meaning way that has not been previously attempted. It is our belief that when considering making meaningful impacts on our vulnerable populations, we need to meet them where they are rather than expecting them to come to our clinic. For example, we were contacted by a local middle school that had 130 students that were facing suspension because they needed required vaccinations. We held a immunization clinic at the school and completed vaccinations for 126 students with parental consent. It was a great success for students and parents, the school, and the public health department. We vaccinated more kids in 4 hours than we did in the previous month in our immunization clinic. Approximately 80 percent of the kids that were vaccinated that day were Medicaid, non-insured, or under -insured. Many of the community "pop-up" clinics are outside in places like dairy farms, feedlots, and parking lots. While we have trailer, it does not have any identifying information on it. We often have employees use their own tables and chairs during these clinics. We do not have any tents or other shelters. We also lack generators, lights, heaters, trash cans, storage bins for medical supplies, or portable hand -washing stations. It is our hope that with an award of funds through this community investment grant will enable us to procure these items so that we can hold "pop-up" clinics in parts of our community that need our help the most. The goal of this clinical outreach program is to meet the community outreach deliverables contained with our CDPHE grant programs' scope of work. The objectives of these requirements are to target specific populations in our community that are under served, and to partner with organizations and businesses to help us reach our vulnerable populations. We often and have historically partnered with traditional partners like schools, long-term care facilities, health fairs, and special community events and festivals. During our COVID mass vaccination efforts, we also develop new relationships with non-traditional partners with dairy farms, feel lots, libraries, community centers, and businesses. These non-traditional partners have significantly helped us make connections in areas where there seems to be significant barriers for our most vulnerable populations.. Q20. In order to reach program deliverables, we have first started by using available community -level health data such as poverty and minority census tracts and other health equity data to identify where our vulnerable populations live and work. After identifying these locations, we then work with community partners and businesses in and around those areas to establish connections and locations for our "pop-up" clinics. Our next step is to work with partners to advertise and promote education sessions in those communities around the clinical services programs we offer. After we have helped the community understand our intentions and the services we can provide, we then promote and schedule our "pop-up" clinics to provide needed screenings, tests, immunizations, treatments, and referrals. Our clinical grant programs are reviewed and renewed by CDPHE and the County on an annual basis. We must provide quarterly reports on patients and services provided to CDPHE. We would use this community investment grant to immediate start hosting "pop-up" clinics. Our goal for 2022 is to host at least 20 "pop-up" clinics in our health equity locations in Weld County. For every year moving forward, our goal would be to host at least 40 "pop-up" clinics annually. Q22.
