Re: Acceptance of Weld County's Collaborative Management Program MOU for SFY 2021-22
Dear Director Ulrich,
The purpose of this letter is to communicate information about your county's participation in the
Collaborative Management Program (CMP) pursuant to 24-1.9-101, C.R.S. .
The Weld County CMP Memorandum of Understanding (MOU) for SFY 2021-22 has been accepted. As specified
in Section XI of your county's MOU, Weld County needs to submit the attached attestation statement with
signatures prior to receiving the earned incentive funds.
The Colorado Department of Human Services, Division of Child Welfare appreciates Weld County's continued
commitment to coordination and integration of services to children, youth, and families in their community.

Attestation
In accepting the incentive funding award for Weld County, I, the County Commissioner(s) and/or County
Director, hereby attest that the Collaborative Management Program will:
• Track clients served in all target populations listed in the CMP site MOU;
• Agree to not duplicating clients in the approved databases (Trails and/or Efforts to Outcome); and
• Comply with Operation Memorandum #OM-CW-2019-0003 regarding data collection.
