Hi Mariah,
I am so sorry that I did not provide that information the first time!
This is in reference to Tyler ID v2021-3067 which is the 2021-2022 Individual Provider Contract, as well as the new list of
Individual Providers that were approved for 2022-2023.
I am checking on their respite services to see if that will be closing as well, so I will get back to you on that one.

I am working on processing the below information as an item of Communications and am wondering which
contract you are pointing to because this provider has an Individual Provider Contract, a Respite Contract, and
is on the new list for upcoming Individual Provider Contracts for the 2022-2023 SFY. Is the current contract and
the future contract both cancelled?

We have been notified that foster care providers Cassy Crawford and Ryan Rutledge will no longer be providing Foster
Care Services as of today, July 6, 2022 and their current contract is no longer in effect.
