AMENDMENT TO THE MEMORANDUM OF UNDERSTANDING (MOU)
BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES
AND ENVISION, CREATIVE SUPPORT FOR PEOPLE WITH
DEVELOPMENTAL DISABILITIES
This Amendment to the Memorandum of Understanding (MOU), made and entered into �tb day of
U'JCt b- , 2022 by and between the Board of Weld County Commissioners, on behalf of the Weld
County Department of Human Services, hereinafter referred to as the "Department", and Envision, Creative Support
for People with Developmental Disabilities, hereinafter referred to as the "Contractor".
WHEREAS the parties entered into an MOU to ensure efficient communication and collaboration
between the two oreanizations specific to the Children's Habilitation Residential Program (CHRP) and other
Home and Community Based Services (HCBS) waiver services, (the "Original MOU") identified by the Weld
County Clerk to the Board of County Commissioners as document No. 2021-0522, approved on February 24,
2021.
WHEREAS the parties hereby agree to amend the term of the Original Agreement in accordance with
the terms of the Original Agreement and any previously adopted amendment, which is incorporated by reference
herein, as well as the terms provided herein.
NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as follows:
• The Original Agreement was set to end on December 31, 2021.
• The Original Agreement was amended on:
• December 15, 2021 to extend the term date through December 31, 2022.
• The Amendments are identified by the Weld County Clerk to the Board of County
Commissioners as document number 2021-0522.
• These Amendments, together with the Original Agreement, constitutes the entire understanding
between the parties. The following additional changes are hereby made to the current Agreement:
1. Term
The Term of this Agreement shall be from January 1, 2023, through December 31, 2023. This
Agreement may be extended annually upon written agreement of both parties.
• All other terms and conditions of the Original Agreement remain unchanged.

Please review and indicate if you would like a work session prior to placing this item on the Board's
agenda.
Request Board Approval of the Department's Memorandum or Understanding (MOW Amendment
with Envision, Creative Support for People with Developmental Disabilities. The Department is
requesting an Amendment to the existing Memorandum of Understanding (MOU) with Envision,
Creative Support for People with Developmental Disabilities (Envision) to continue the term of the MOU
for the next year. This will ensure the continuation of efficient communication and collaboration between
the two organizations specific to the Children's Habilitation Residential Program (CHRP) and other
Home and Community Based Services (HCBS) waiver services.
This Amendment will extend the term of this MOU to January 1, 2022 through December 31, 2022 with the option
to extend annually upon written agreement of both parties. The Department has no financial obligations under this
MOU.
I do not recommend a Work Session. I recommend approval of this Amendment and authorize the Chair to sign.

RE: APPROVE MEMORANDUM OF UNDERSTANDING FOR CHILDREN'S HABILITATION
RESIDENTIAL PROGRAM (CHRP) AND HOME AND COMMUNITY BASED SERVICES
(HCBS) WAIVER SERVICES AND AUTHORIZE CHAIR TO SIGN - ENVISION
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Memorandum of Understanding for the
Children's Habilitation Residential Program (CHRP) and other Home and Community Based
Services (HCBS) Waiver Services between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, and Envision, commencing January 1, 2021, and ending December 31, 2021, with
further terms and conditions being as stated in said memorandum of understanding, and
WHEREAS, after review, the Board deems it advisable to approve said memorandum of
understanding, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Memorandum of Understanding for the Children's Habilitation
Residential Program (CHRP) and other Home and Community Based Services (HCBS) Waiver
Services between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, and Envision,
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said memorandum of understanding.

Request Board Approval of the Department's Memorandum of Understanding (MOU)
with Envision, Creative Support for People with Developmental Disabilities. The
Department has entered into a Memorandum of Understanding (MOU) with Envision Creative
Support for People with Developmental Disabilities (Envision) to ensure efficient
communication and collaboration between the two organizations specific to the Children's
Habilitation Residential Program (CHRP) and other Home and Community Based Services
(HCBS) waiver services.
Envision assists children who are determined to have intellectual or developmental disability
(IDD) or developmental delay in receiving services they are entitled to. When there are concerns
of abuse or neglect, the Department will work to alleviate risk and partner with Envision to get
the correct services in the home. An Envision Intake Case Manager (ICM) will be co -located
within the Division of Child Welfare in order to ensure early identification of children and
families who will benefit from Envision's assistance. Additionally, the ICM will help educate
families and Department staff on services that are available within the community. The
Department will provide the 1CM with information regarding child protection or safety issues
within the home of IDD children.
This agreement is effective January 1, 2021 through December 31, 2021 with the option to
renew, in writing, on an annual basis. The County has no financial obligations under this MOU.

MEMORANDUM OF UNDERSTANDING BETWEEN THE WELD COUNTY
DEPARTMENT OF HUMAN SERVICES AND ENVISION, CREATIVE SUPPORT FOR PEOPLE WITH
DEVELOPMENTAL DISABILITIES
THIS MEMORANDUM OF UNDERSTANDING is entered into this 91 day of F¢,brQc r', 2021,
by and between the Board of County Commissioners, on behalf of the Weld County Department of Human Services,
hereinafter referred to as "Human Services," and Envision, Creative Support for People with Developmental
Disabilities, hereinafter referred to as "Envision".
Purpose
1. To establish the protocol for the coordination of the Human Services, the Child Welfare and Family
Resource Divisions and Envision Case Management, specific to the Children's Habilitation Residential
Program (CHRP) and other Home and Community Based Services (HCBS) waiver services.
2. Human Services and Envision share the value that children with special needs, specifically children
determined to have an intellectual or developmental disability (IDD) or developmental delay should be able
to receive services they are entitled to with the least amount of trauma and distress to child(ren) and
families. Envision is better able to meet the needs of these children and families when there is no indication
of abuse or neglect. When there are concerns of abuse or neglect, Human Services will work to alleviate
risk and partner with Envision to get the correct services in the home.
3. Human Services and Envision each have individual roles concerning the welfare of current and potential
populations served. It is in the best interest of shared clients that good communication and cooperation is
the standard between these programs.
4. In order to ensure efficient communication and collaboration, Envision will provide an Intake Case
Manager (ICM) who will act as a liaison between Envision and Human Services staff. The ICM will be co -
located in the Division of Child Welfare in order to ensure that:
a. Children who are eligible for CHRP and/or Children's Extensive Support (CES) waivers are
identified early on in the assessment process, including children and families working with
prevention,
b. Families and caseworkers are educated about the services that are available within the community
through the CHRP and/or CES waivers and
c. IDD determination and CHRP eligibility are completed in a timely manner.
Term of this Memorandum of Understanding
This Memorandum of Understanding ("MOU" or "Agreement") shall be effective beginning January 1, 2021 and
shall end December 31, 2021. This MOU may be renewed by both parties, in writing, on an annual basis.
Responsibilities of Envision Intake Case Manager (ICM) Liaison
• Complete application processes with the caseworker and/or family.
• Attend Review, Evaluate and Direct (RED) Team, Family Team meetings, Team Decision Making (TDM)
Meetings, and Group Supervision as needed to assess the needs of IDD children and families.
• Provide quarterly training to new casework staff around the CHRP and CES waivers.
• Carry a caseload of Human Services children and families, including Child Welfare and Prevention cases,
and prioritize these cases above other work -related requirements within the time allotted to Human
Services.
• Set up regular times to be available for Human Services caseworkers and outreach staff to drop -in to
discuss children and families on their caseloads who may be IDD and CHRP eligible.
• Present data, in conjunction with the Child Welfare Liaison, to Human Services Administration at least
quarterly or as requested.
• Participate in state meetings and trainings as requested by Human Services.
Responsibilities of Human Services Child Welfare Liaison and/or Quality Assurance Staff or Caseworker
• Meet regularly with the ICM to discuss workload.
• Conduct monthly staffings with the ICM and Envision Quality Assurance Director to review children in
various stages of referral process.
• Assist the ICM to gather all information needed to determine the child(ren)'s eligibility for services.
• Provide ICM information related to the child protection/safety issues of the home.
• Inform the ICM when a case has closed.
• Determine if the child needs to be placed out of the home and contact the ICM regarding placement.
• Review the Placement Information Form to identify children who may be eligible for waiver services and
provide that information to the ICM.
Expected Outcomes
• Increased collaboration between Envision Case Management and Human Services, particularly the Child
Welfare and Family Resource Divisions.
• Early identification of children who may be IDD and who may qualify for CHRP or CES waivers.
• Reduction in the number of families who enter Child Welfare due to inability to receive necessary services
and thus avoid Child Welfare involvement and keep child(ren) at home.
• Provision of services and placement through the CHRP waiver rather than through Child Welfare services
and funds.
• Reduction in length of time cases are open by providing families with the appropriate services they need to
successfully support their child(ren) to return home.
• Streamline process for children who are aging out of foster care and entering adult IDD services.
Modifications. This Agreement may be modified in writing, in whole or in part, upon approval of the signers.
Termination. This Agreement may be terminated by any of the signers upon thirty (30) days written notice.
Entire Agreement/No Third -Party Beneficiary. This Memorandum of Understanding contains the entire
agreement and understanding between the parties to this Memorandum of Understanding and supersedes any other
agreements concerning the subject matter of this transaction, whether oral or written. It is the express intention of
the parties that any entity other than the undersigned parties receiving services or benefits under this Memorandum
of Understanding shall be deemed an incidental beneficiary only.
No Waiver of Immunity. No portion of this Memorandum of Understanding shall be deemed to constitute a waiver
of any immunities the parties or their officers or employees may possess, nor shall any portion of this Memorandum
of Understanding be deemed to have created a duty of care that did not previously exist with respect to any person
not a party to this Memorandum of Understanding.
Independent Contractor. The Envision liaison shall perform his/her duties hereunder as an independent contractor
and not as an employee of County. Envision liaison shall be solely responsible for his/her acts performed pursuant to
this Agreement. Under no circumstances shall Envision liaison be deemed to be an agent or employee of Weld
County. Envision liaison is not entitled to unemployment insurance or workers' compensation benefits
through Weld County and Weld County shall not pay for or otherwise provide such coverage. Unemployment
insurance benefits will not be available to Envision liaison for services provided under this Agreement.
Envision shall pay when due any/all applicable employment taxes and income taxes and local head taxes (if
applicable) incurred as a result of the services provided under this Agreement.
Severability. If any provision of this Memorandum of Understanding should be held to be invalid, illegal, or
unenforceable for any reason, the validity, legality, and enforceability of the remaining provisions shall not in any
way be affected or impaired thereby.
IN WITNESS WHEREOF, the parties hereto have duly executed the Agreement as of the day, month, and year first
above written.
