,) This well shall be used in such a way as to cause no material injury to existing water rights. The issuance of this permit
does not ensure that no injury will occur to another vested water right or preclude another owner of a vested water right from
seeking relief in a civil court action.
2) The construction of this well shall be in compliance with the Water Well Construction Rules 2 CCR 402-2, unless approval
of a variance has been granted by the State Board of Examiners of Water Well Construction and Pump Installation
Contractors in accordance with Rule 18.
3) Approved pursuant to CRS 37-92-602(3)(b)(I I)(A) as the only well on a residential site of 1 acre(s) described as lot 6, block
75, filing 3, Aristocrat Ranchettes Subdivision, Weld County.
4) The use of ground water from this well is limited to ordinary household purposes inside one single family dwelling. The
ground water shall not be used for irrigation or other purposes. 285 feet - ADF 5/17/ 16
5) The total depth of the well shall not exceed .266 -feet, which corresponds to the base of the Lower Arapahoe aquifer. At a
minimum, plain casing shall be installed and grouted through all unconsolidated materials and shall extend a minimum of
ten feet into the bedrock formation to prevent production from other zones.
6) The pumping rate of this well shall not exceed 15 GPM.
7) The return flow from the use of this well must be through an individual waste water disposal system of the
non -evaporative type where the water is returned to the same stream system in which the well is located.
This well shall be constructed not more than 200 feet from the location specified on this permit.
NOTE: To ensure a maximum productive life of this well, perforated casing should be set through the entire producing
interval of the approved zone or aquifer indicated above.
NOTE: This permit will expire on the expiration date unless the well is constructed by that date. A Well Construction and
Test Report (GWS-31) must be submitted to the Division of Water Resources to verify the well has been constructed. An
extension of the expiration date may be available. Contact the DWR for additional information or refer to the extension
request form (GWS-64) available at: http:flwww.water.state.co.us
NOTICE: This permit has been approved for a distance from the south section line that is different than the distance
specified on the well permit application. The distance was modified in order to place the well on the subject lot. You are
hereby notified that you have the right to appeal the issuance of this permit, by filing a written request with this office within
sixty (60) days of the date of issuance, pursuant to the State Administrative Procedures Act. (See Section 24-4-104 through
106, C.R.S.)

SITE CONDITIONS
It is our understanding that the site will be used to construct a new residence that will have 4
bedrooms. The site is vegetated by native grasses and weeds. The property is bordered to the north,
west and east by vacant lots. Coleman Avenue is located to the south. We are unaware of any
existing septic permits for the site, or of the presence of natural or cultural features at the site.
A total of six percolation test borings and one soil profile boring were drilled on July 17, 2015. The
percolation borings were drilled at depths ranging from of 3 to 5 feet and the soil profile boring was
drilled to a depth of 8 feet. The borings were drilled at the locations show on the Site Plan, Figure 1.
Soils in the area of the proposed absorption bed consisted of silty sand and sandy lean clay to a
depth of 5 feet. Siltstone bedrock was encountered at a depth of 5 feet in the profile boring. The
absorption bed must remain 4 feet above the bedrock layer, Summary boring logs are attached with
this report.
The borings were located in the field by pacing from property lines and/or existing site features.
Relative surface elevations at the boring locations were obtained by measurements with an engineer's
level from a temporary bench mark (TBM) shown on the Site Plan. The accuracy of the boring
locations and elevations should only be assumed to the level implied by the methods used to
determine each.
Groundwater conditions were elevated in each boring at the time of subsurface explorations, and one
day after the drilling. Groundwater, or the physical features present from past high groundwater were
not observed to a depth of 8 feet.

INSTALLATION INSTRUCTIONS
1. Excavation should be performed in such a way as to limit/eliminate traffic over the area of the
absorption bed.
2. All wastewater lines shall be constructed of compatible pipe, primer, bonding agent and fittings.
3. Use Schedule 35 pipe except where piping is placed under driveways or roadways — in these
locations use Schedule 40 pipe.
4. All system piping shall be bedded with select, granular material. Material may be on -site or
imported material.
5. Compact soils around tank and distribution box to ensure piping will remain level and settlement
will not create bellies within the piping.
