Providers on the approved provider list via PA dated 05/25/21 (Tyler #2021-1458 attached for reference) that were
NOT issued contracts:
Child Placement Agency Services
• Quality Life Services L.L.C. — Agency no longer operating (Related: Tyler #2020-2090).
Residential Child Care Facility Services
• Avanti House — Facility no longer operating.
• Cedar Springs Hospital - Services not needed at this time.
• Tennyson Center for Children — Facility no longer operating.
• Great Circle (Facility ID #1698828 only) — Contract not needed/Youth no longer placed.
Providers on the approved provider list via PA dated 05/25/21 (Tyler #2021-1458 attached for reference) that were
issued contracts but are being cancelled:
Residential Child Care Facility Services
• Savio House — Facility no longer operating (Tyler #2021-1794).
Please let me know if any further information is required such as additional Tyler ID numbers as they are related if
necessary.
