This communication item is in regards to the Mead Community Center Nutrition
Program MOU, Tyler ID #2017-0500. We have received the attached termination notice

Re: Notice of Termination — Memorandum of Understanding between Mead Community Center (Town
of Mead) and Area Agency on Aging's Senior Nutrition Program dated January 16, 2016 ("Agreement")
Dear Ms. Skoglund:
This letter serves as notice of termination ("Notice") of the above -referenced Agreement between the
Mead Community Center/Town of Mead ("Town") and Weld County acting by and through the Board of
County Commissioners on behalf of the Department of Human Services' Area Agency on Aging's Senior
Nutrition Program (the "Program"). Pursuant to paragraph 5.a. of the Agreement, the date of
termination shall be thirty (30) days from the date of this Notice.
The basis for this Notice includes the fact that the Friendly Fork program provided by the Program does
not align with the current Older Active Adult programming desires of the Town and the Town has
experienced significant staff growth and facility demands resulting in the Town's inability to continue to
commit to the use of the Mead Community Center for Program -related use.
Therefore, the Town is terminating the Agreement pursuant to paragraph 5.a. of the Agreement with a
termination date of thirty (30) days from the date of this Notice.
