APPLICATION (ROUND THREE [3]) FOR WELD COUNTY OPERATIONAL
TRANSPORTATION SERVICES - DEPARTMENT OF PUBLIC WORKS AND SENIOR
RESOURCE SERVICES, DBA 60+ RIDE
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a 2023 Community Development Block
Grant (CDBG) Program Application (Round Three [3]) for Weld County Operational
Transportation Services from the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Public Works, and Senior
Resource Services, dba 60+ Ride, to the Weld County CDBG Program, with further terms and
conditions being as stated in said application, and
WHEREAS, after review, the Board deems it advisable to approve said application, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the 2023 Community Development Block Grant (CDBG) Program
Application (Round Three [3]) for Weld County Operational Transportation Services from the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Public Works, and Senior Resource Services, dba 60+
Ride, to the Weld County CDBG Program, be, and hereby is, approved.

SUBJECT: 2023 CDBG App Round 3 — 60+ Ride
In accordance with the work session held on October 18, 2022, Weld County Public Works
is requesting the Commissioners consideration to submit a Community Development
Block Grant (CDBG) application on behalf of 60+ Ride.
The purpose of the CDBG application is to assist 60+ Ride (a 501(c)(3) nonprofit
organization) with the operational expenses to support staffing one full-time equivalent
(FTE) employee to operate their new wheelchair (ADA) accessible van. These operational
expenses are intended to augment their existing volunteer transportation program by
ensuring a consistent and flexible transportation service for low to moderate income
persons in Weld County.
The 60+ Ride program provides free transportation services for Weld County's residents
over 60 years of age. By hiring a dedicated driver to staff the ADA van, it will help address
an existing community need to provide ADA accessible rides for elderly and disabled
residents. Transportation is the number one reason older adults lose their independence.
The needs for transportation vary widely, including to medical appointments, grocery
stores, financial and government institutions, social events, houses of worship, and
personal grooming appointments. Not only do these services directly increase older adults'
ability to maintain their health and independence through the consistent availability of
medical care, but these same services reduce re -hospitalization, offer a way to increase
nutrition quality through fresh food, and decrease social isolation and depression. This
translates into both quality of life for Weld County's 60+ population and the financial benefit
of reducing the need for government social services and therefore the tax burden. Another
financial benefit comes in the form of reducing the impact of caring for aging family
members on workforce production because our services lessen the need for adult children
to take time off work to ensure the basic needs of their parents are being met.
This application requests operational transit funding in the amount of $112,000 from May
of 2023 to April of 2024. The grant application does not require the County to provide any
local match funds.
Since this application meets the CDBG eligibility criteria, staff recommends the Board
authorize staff's signature on this application to submit to the CDBG Advisory Committee
for consideration. I am available to answer any questions you may have.

Agency description: Please include information about your agency, its goals, mission, incorporation date,
management structure and experience, clientele served, and services offered. Please include an organizational

Senior Resource Services (SRS), a 501(c)(3) nonprofit organization dba 60+ Ride, was established in 2006 with the
mission to offer Weld County residents 60 years and older the opportunity to maintain their physical, emotional,
and mental health, while living independently. The organization recognized that a critical need for older adults
and their families (particularly low-income households) is access to affordable and accessible transportation to
maintain both their health and independence. In response to this need, 60+ Ride offers a volunteer driver
transportation program providing free rides to persons aged 60 and over, regardless of income, in the Weld
County area.
Since 2007, the organization has been providing transportation to Weld County older adults, supporting access
to key services related to maintaining a good quality of life while aging in place. Volunteer drivers and 60+ Ride's
part-time drivers provide the rides which are scheduled by staff utilizing a web -based scheduling system in
response to client phone calls. To further clarify its role in providing transportation services, the organization
began doing business as "60+ Ride" in 2018 to communicate its services more accurately to the public.
The organization is currently staffed by full-time Executive Director Janet Bedingfield (who has been with the
organization since 2015), two full-time employees including an Administrative Assistant and a Volunteer &
Outreach Coordinator, a part-time Office Assistant/Ride Scheduler, and three part-time drivers for the agency -
owned vehicle for the equivalency of one full time employee (FTE).
60+ Ride's program provides volunteer drivers and a staffed SUV to transport Weld County's aging population
(door through door) to medical appointments, grocery stores, financial and government institutions, social
events, houses of worship, and personal grooming appointments free of charge, regardless of income. Not only
do these services directly increase older adults' ability to maintain their health and independence through
consistent availability of medical care, but these same services reduce re -hospitalization, offer a way to increase
nutrition quality through fresh food, and decrease social isolation and the resulting depression. This translates
into both quality of life for Weld's residents aged 60 and older and the financial benefit of fewer people depending
heavily on government social services.
In 2019 the NFRMPO prepared an analysis of aging adults in Weld County and future trends and transportation
needs. This analysis determined that between 2015 and 2045, Weld County residents over the age of 60 are
expected to more than double, growing by 134% - from a current estimate of approx. 60,000 residents to more
than 120,000 residents. Residents aged 80 and above will grow at the highest rate, increasing by 308.4%.
60+ Ride provides services to all older adults aged 60 and older regardless of race, ethnicity, or income. The target
population is the most vulnerable individuals who do not drive and do not use public transportation because of
cost, accessibility, or lack of stamina. The majority of registered riders are women (80%) and 16% are Hispanic
(although 40% of rides are made by Hispanic riders who have a higher dialysis treatment need). Approximately
90% of riders are living at or below 50% ($31,000) of the Area Median Income for a single person ($62,000).

Complete project description: Clearly explain what will be accomplished with the CDBG funding that you
are requesting, including who will benefit from the project. Be specific as to what the CDBG funds will be
used for. Attach additional information as needed.
The purpose of this CDBG funding request is for operational transit funding to assist 60+ Ride with
implementing and sustaining a new Staffed Accessibility Van Program. The operating costs would be for
one full-time employee to drive a new wheelchair (ADA) accessible van increasing the capacity to serve
rural residents, including free transportation services for those needing wheelchair accessibility. The funds
from this request will be used to cover operational expenses associated with the Staffed Accessibility Van
Program, including the addition of a full-time driver dedicated to operating the van for the program. The
agency believes the program necessitates a consistent driver who is up to date on the special, required
training needed to safely operate this unique vehicle.
60+ Ride currently provides volunteer and staff driver transportation within Weld County but is limited to
ambulatory clients due to the lack of an ADA-accessible vehicle and related operations funding. 60+ Ride
relies on its volunteers' use of personal vehicles in rural Weld. While we are continually recruiting, there is
also a consistent shortage of volunteer drivers in rural areas because of the smaller populations where
clients reside.
This vehicle will serve low to moderate income older adults who do not currently have access (or adequate
access) to affordable transportation and/or wheelchair accessible transportation to medical and wellness
services. 60+ Ride reports that these are the top unmet needs for services requested from the organization,
and this new service is being driven by the need to fill that critical gap. The van would be driven by
paid/trained staff and would be used to provide both ambulatory and wheelchair -accessible transportation
to rural Weld County residents free of charge.
The project involves CDBG providing 60+ Ride with $112,000 in fiscal year 2023 for operating support of
the van program. Operating costs include a full-time, trained driver, a part-time scheduler, insurance,
maintenance, fuel, community communications and program overhead, which are all direct costs to this
program. 60+ Ride will use this operational funding from Weld CDBG's Round 3 (FY 2023) to sustain the
new programming until fully operational and 60+ Ride is able to secure FTA transit operations funding.
Please include a timeline to show how you will complete the project and expend all funding within 12
months of receiving your grant.
April 2023 — We will notify Board of Directors of the approval of the grant and award amount and complete
agreements/contracts required by the grant program.

May 2023 — April 2024 — We will provide ADA rides to Weld County LMI seniors in rural locations though
the newly established Staffed Accessible Van Program. All appropriate operational costs related to the
grant will be tracked, including the use of QuickBooks Online by our contracted bookkeeper.
The agency will continue implementing a communications plan which inform community partners about
the ongoing transportation services. This will include utilizing presentations to gatherings of older adults
and civic groups, delivering printed materials to locations frequented by older adults needing services
including medical providers, NFRMPO's Mobility Coordinator's networking, United Way of Weld County's
Aging Well Volunteer Recruiter, municipality communications departments, and digital media.
May 2024 — We will complete and submit the required reporting for this grant opportunity.
2022 Weld County CDBG Application- Community Components
Briefly explain why this project is necessary in the community, and why CDBG funds should be used to
address the need.
This new program will serve low to moderate income older adults (a CDBG National Objective) who do not
currently have access (or adequate access) to affordable transportation or wheelchair accessible
transportation to medical, wellness and social services. 60+ Ride reports that rural and wheelchair !
accessible transportation are the primary unmet service requests received by the organization.
According to the American Community Survey 5 -year estimates (2013-2017), approximately 40% of
Greeley/Evans residents over the age of 65 reported having a disability. In the rural communities of Weld
County between 25% and 58% of residents 65 years old and older reported having a disability. The same
survey also reported that 3.8%, or 3,790 households in Weld County had no vehicle.
While the majority of the 8,521 rides provided in 2019 (the last year of full operations before the pandemic)
were in the Greeley/Evans area (about 85%), more recently, 60+ Ride has actively expanded to other parts
of rural Weld County. In 2019, approximately 40 volunteers provided 1,267 rides to 146 riders in 17 rural
communities. One of the organization's 2022 goals is to continue to expand its capacity to provide access
to our free transportation services to the isolated, rural areas of Weld County by increasing our outreach
efforts to recruit an additional 25 rural volunteer drivers.
There are significant numbers of older adults in rural Weld County without adequate access to
transportation, including those with disabilities, either because they cannot afford specialized
transportation programs or because they live in isolated, rural communities where low-cost services are
not available. Each month, 60+ Ride turns away numerous rural clients including more than five callers !.
requesting wheelchair accessible rides. The older adult community which needs this service (and providers !,
of services who might refer to this service) are aware we lack sufficient numbers of rural volunteers and
also the capacity to transport wheelchairs, so many never contact us.
In reference to wheelchair accessible transportation, the public Greeley/ Evans Transit (GET) system's
paratransit program is invaluable, but the services are not adequate to meet the needs of the frail

population who do not have the stamina to utilize its services, live in areas not covered by GET, or who
cannot afford the minimal $6 round trip fee.
What are the quantifiable goals of this project? How will they be measured?
(Example: Install 2,000 feet of 24 -inch storm sewer line in census tract; or provide healthcare for 100 low-
' -income residents)
There are may quantifiable goals for providing wheelchair accessible transit to Weld County. They
include, but are not limited to the following:
• An estimated 120 older adults in Weld County requiring free transportation, either ambulatory or
wheelchair accessible, will be served annually with an estimated 1,020 trips per year.
95% of these older adult riders utilizing the Accessible Van Program will report they have
maintained or improved their physical, emotional, and/or mental health and wellness by having
scheduled, reliable transportation to destinations including the hospital, medical & mental health
appointments, grocery shopping, nutrition programs/ meals.
95% of older adult riders who request accessible services to enroll in and maintain vital mainstream
benefits which support their health, wellness, and housing independence (e.g., SNAPS,
Medicare/Medicaid, housing assistance, AAA assistance, etc.) will receive transportation.
95% of older adult riders who request accessible services to personal and social events (Senior
Centers, faith programs, community events, etc.) to reduce their social isolation and connect them
with other people and community support systems will receive transportation.
100% of older adult riders receiving accessible transportation services will receive guidance and
referrals to community resources for older adults for services and information that they did not
previously know or understand how to access (e.g., AAA, Catholic Charities, etc.)
The organization uses three evaluation strategies to measure effectiveness: 1) Formative Evaluation is
conducted through interviews and open-ended questionnaires. 2) Administrative Evaluation includes
intake forms and driver reports on each rider providing information concerning geographical location and
demographics served. This data is analyzed to determine whether various regions served are receiving
sufficient information that the number of existing volunteers meets the needs of clients, and that the
number of clients being served meets the program's estimates, etc. 3) Program Evaluation determines
success if the data indicates that clients' needs are satisfactorily met, our services have increased their
ability to remain independent, and if necessary, adjustments to services have been made to maximize that
satisfaction level. Reports based on program data are run monthly or as needed to provide pertinent data
to funders, future funders, the Board of Directors, and staff to inform future program decisions.

1. Community Human Services Network: 60+ Ride collaborates with multiple agencies serving
Greeley/Weld County for transportation referrals and to ensure that our riders are knowledgeable
about critical services. This referral network includes the following stakeholders serving older
adults: hospitals, doctors, dentists, community health centers (indigent medical/mental health
care), churches, Area Agency on Aging, Catholic Charities, Alzheimer's Assoc., Immigrant and
Refugee Center of Northern Colorado, United Way, Weld Food Bank, Meals on Wheels, and Senior
Centers (located throughout Weld County). Referrals for paratransit services are provided through
the Greeley/Evans Transit system.
2. In 2018, Executive Director Janet Bedingfield initiated the formation of a new Senior Services Task
Force including AAA, United Way, the Weld Community Foundation, and Catholic Charities to focus
on collaboration of services, identifying service gaps, reducing duplication, and increasing
visibility/public awareness of the needs of the aging population. The task force has evolved into the
UWWC's Aging Well Collective Impact collaborative which continues the same goals but is a larger
group more broadly representative of agencies serving Older Adults. With over 40 nonprofits,
businesses, and medical interests represented, the Aging Well Collective Impact Group has achieved
success in a short time. A grant from Next50 was secured to cover two years of funding for a
Volunteer Recruiter focused specifically on recruiting volunteers for Weld County entities serving
older adults. The position was filled early in the fall of 2020, is housed at United Way of Weld
County, and the program has been a success even during the pandemic. More recently the Aging
Well Group has applied for and received Age Friendly Community status for Greeley, Evans, and
Garden City with plans to add all of Weld County at some point in the future. The group is currently
completing an Action Plan covering the Age -Friendly projects. Janet continues as an active member
of the Aging Well Executive Committee and is co-chair of the Transportation work group.
3. Janet also serves as one of two executive committee members for the North Front Range
Metropolitan Organization's (NFRMPO, a CDOT planning region) Weld County Mobility Committee,
bringing a voice for inclusion of older adult transportation needs to this regional transportation
planning group. Janet has acted as a consultant for the NFRMPO and was part of the interview team
for the new positions of Mobility Manager and Mobility Coordinator toward the August 2021 launch
of their One Call One Click center known at Ride NoCo.

The only other available similar non-profit transit service is Via Mobility, who operates out of Boulder
County. Weld County has 32 municipalities with the remainder of the 4,000 square miles covering
unincorporated areas of the county. This large area equates to a great need for multiple providers to be
able to serve Weld County's most sensitive populations. Involvement in UWWC's Aging Well Collective
Impact Group and the NFRMPO's Weld County Mobility Committee will ensure we effectively coordinate
with other organizations to ensure all Weld residents are effectively receiving these transportation
services. The organization uses an online scheduling system (RideScheduler) which acts as a data collection
system, allowing us to track all unduplicated clients, all trips provided, location of the trips, purpose of the
trips provided and active volunteer numbers.
2022 Weld County CDBG Application- Capacity
If we are unable to grant your full request, what is the minimum amount that you would be willing to
accept that would still allow your project to be viable?
In order for the new Staffed Accessibility Van Program to be successful we must have the vital operational
expenses required to safely provide services to clients. If the Weld County CDBG is unable to help with the
costs of meeting safety requirements through effective operations, or partially funds this request, 60+ Ride
will continue conversations with other private and government funders to identify a viable source of
revenue to achieve this goal and establish the program to meet the needs of Weld County citizens.
If the project is not awarded this amount of CDBG funding, how will it be affected?
The Staffed Accessible Transportation Program will be postponed until full operational funding can be
secured. We will continue looking for additional funds to support our goal of increasing our services to
include a staffed vehicle serving the unmet needs of rural and wheelchair bound residents.
Leveraging funds is important to our program's mission. What other funds are currently available to
support this project or leverage CDBG funds? Please include sources, dollar amounts and status of such
funds.
The Weld County Commissioners have already designated $13,000 from the FY22 general fund to support
the 20% local match requirement for purchasing the ADA-compliant van that will be utilized for this
program.
80% of the accessibility van cost ($52,000) is being sourced from CDOT 5310 rural FTA funds vetted through
the North Range Front Metropolitan Planning Organization.

The goal of this program is to provide ADA transportation services to LMI Weld County residents aged 60 and
older during these six months of FY22.
How long would clients receive services from your organization under this project?
Clients would be eligible for services under this program throughout the length of this CDBG funding cycle if
they continue to meet the requirements of this grant.

Yes, our program has experienced staff to assist with training the new hire who will assist with providing the
ADA van services.

We will continue to pursue other funding opportunities from agencies who support transit operations to help
leverage our program's ongoing operational expenses such as the Colorado Department of Transportation
(CDOT) and North Front Range Metropolitan Planning Organization (NFRMPO).
What is the CDBG-cost per client for this project? (Total CDBG funds requested divided by number of clients
served)
$933.33
How do you determine current eligibility? What client demographic information does your organization
collect?
Clients must be 60 or older, live in Weld County, and for non -accessible transportation they must be able to
walk to and from the vehicle as well as get in and out unassisted other than an arm assist or light walker.
With the expansion to include a program for non -ambulatory people supported, this last requirement will not
apply. For the purpose of this grant, we will also track low -and -moderate income persons, similar to what we
do with our City of Greeley residents. Also, all people 60 and older are considered LMI by federal
government definition
Have you worked with other agencies to decrease service duplication and increase effectiveness? If so, how?
The only other non-profit organization offering this service is Via Mobility Services. There is enough need in
Weld County for both agencies to provide these services.
