I'd like to come in at 2p on the 19th. Thanks, Lori On Tue, Jan 17, 2023 at 9:21 AM Chloe White <cwhite@weldgov.com>wrote: Good morning, In order to submit a Special Events Permit (SEP) Application, an appointment does need to be made. Below, I have listed a few options for my availability this week and next. A complete application must be submitted to me no later than Friday, January 27, 2023, to meet the 30 -day deadline. If you are not available during these times, please let me know and I will try my best to accommodate your schedule. • Wednesday, January 18, 2023: anytime between 3-5:00 p.m. (please keep in mind we are expecting inclement weather this evening and there's always the possibility the County will announce a snow closure Wednesday morning) • Thursday, January 19, 2023: anytime between 12-5:00 p.m. • Monday, January 23, 2023: anytime between 12-4:00 p.m. • Thursday, January 26, 2023: anytime between 12-4:00 p.m. If approved, a Special Events Permit, which is dual licensed by both Weld County and the Colorado Department of Revenue, would allow a qualifying applicant to serve alcohol at the event. Please note, only the applicant would be authorized to profit from the sale of alcohol, so only the Rotary Club of Mead Foundation could interact with the sale of alcohol and profit from said sales (not the Platteville Rotary Club). You already have the first page of the State's application, but I have attached the second page, as well as the County's application form/questionnaire, to this email. Attachments: 1. Colorado Special Events Permit (DR 8439) — page 2 specifically 2. Weld County Special Events Permit Form Based on the documents you attached to your original email, please see the below notes thus far: On the State's SEP form: o Missing the date of birth, phone number, and email address of the event manager o The date of the event appears to still say February 18, 2023, not February 28, 2023, which you indicated in your email On the diagram of the premises: o The diagram needs to reflect bars, walls, partitions, ingress, egress, location of liquor storage, and dimensions of each. If the event is to be held outside, please submit evidence of intended control of the boundaries (i.e. fencing, ropes, barriers, etc.). Because the event is to be held at an establishment permitted by Weld County through Use by Special Review Permit, USR18- 0124, a diagram of the parking plan is not necessary at this time. Missing documents: o Weld County Special Events Permit Questionnaire 2 • Please be sure to provide exact information about the company you'll have serving alcohol at the event o Deed, lease, or written permission of the property owner(s) for use of the property for this event • I understand this is forthcoming — it will need to be submitted at the same time as the application o Certificate of good standing o A copy of the food license or temporary food permit • Fees: o $25.00 payable to the Colorado Department of Revenue o $100.00 payable to Weld County (Please note: I can only accept a check for the State's fee, but I can accept either exact cash or a check for the County's fee) If you have any questions or concerns, please do not hesitate to contact me. Do not click links or open attachments unless you recognize the sender and know the content is safe. Hello: I called last week while I was in Greeley to inquire about where to deliver a Special Event Liquor License application. I was told to email you first to schedule an appointment. I am now emailing you to schedule an appointment. I am the President of the Mead Rotary Club. We, in conjunction with the Platteville Rotary Club are holding a Mardi Gras fundraiser on 02/28/23. The event will be at the St. Vrain River Ranch Event Center just outside the town limits of Platteville. I've attached the application, TIPS training certificates and a drawing of the layout. We plan to have a company from Loveland come out with their horse trailer bar and actually handle the serving, but several of our members are trained and willing to be backup servers. I am waiting for a written statement from the event center owner, who is a member of the Platteville Rotary, to provide me with a statement demonstrating written consent to have the event at her space. Please let me know what else I need to provide, and some potential dates/times we can meet.
