The purpose of this contract amendment to the CDPHE Core Immunization grant is to maintain access to COVID-19 vaccines for uninsured and underinsured adults,and to maintain awareness and increase vaccination rates in Colorado.The CDC's Bridge Access Program will provide no cost COVID-19 vaccines for a limited time after these products move to the commercial market in Fall 2023.This temporary program aims to increase access to COVID-19 vaccinations specifically for uninsured and underinsured adults.CDPHE will provide vaccines,as available,and funding to support immunization activities. WCDPHE will offer COVID-19 vaccines alongside other routine vaccines at after-hours and weekend vaccination clinics to increase access to eligible individuals.The department's immunization outreach program currently hosts frequent after-hours/weekend events,we will simply add COVID-19 vaccines to the list of available vaccines at offsite clinics to meet contract requirements. The total Bridge Access funding allocation to Weld County will be$108,438.The FY24 Core Immunization Services contract runs from the beginning of October 2023 to the end of September 2024. If approved,a portion of the funding will be utilized to hire a term-limited,part-time Medical Assistant (Grade 17 Step 1)position to help support the delivery of the program deliverables.The position would be term-limited to the end of October 2024 and part-time working approximately 20 hours per week.No additional FTEs are requested. What options exist for the Board?(include consequences,impacts,costs,etc.of options): With the approval of the Board,this will allow WCDPHE to offer routine vaccines(including COVID-19 vaccines)at no cost to eligible uninsured and underinsured clients.The supplemental funding provided by CDPHE will cover the vaccine cost and the cost of the additional term-limited, part-time Medical Assistant for service delivery and program compliance. If approved,this item will be placed on a future BOCC agenda for formal Board action. If the Board declines this contract amendment,the WCDPHE will continue to offer COVID-19 and other routine vaccines to uninsured and underinsured eligible individuals,however,we will not be able to utilize the funding to offset costs of routine offsite clinics after-hours and on weekends. Entity Name:Weld County Department of Public Health and Environment IL Project Description:This project serves to maintain immunization awareness and increase vaccination rates in Colorado.Local public health agencies will assure core immunization services,according to established best practices and standards,to improve the health of individuals and communities. The Colorado Department of Public Health and Environment(CDPHE)is funded by the Centers for Disease Control and Prevention(CDC)to address potentially life-threatening diseases and reduce risk.The project supports Local Public Health Agencies(LPHAs)through the Vaccines for Children(VFC) program,and creates a coordinated infrastructure to align with national immunization standards.The project supplies Section 317 vaccines at no cost to remove the financial barrier for adults without insurance. The project counters anti-vaccination attitudes and beliefs by engaging the community,and delivering education in the service area.The project encourages healthcare providers and school-based settings to use the Colorado Immunization Information System(CIIS),and to contribute to the centralized system by entering data.The project is intended to convene the public and health professionals,reduce mortality and morbidity rates caused by vaccine-preventable diseases,and protect the health of Coloradans. III. Primary Activity#1 The Contractor shall implement core immunization services within its jurisdiction. Sub-Activities#1 1.The Contractor shall promote within the jurisdiction all ACIP-recommended vaccines available through the following distribution channels: a. VFC vaccines available through the VFC program for the eligible population served. b. Section 317 vaccines available through the Section 317 program for the eligible population served. c. Privately-purchased vaccines available for the eligible insured population served. 2.The Contractor shall assure immunizations are provided within the jurisdiction. 3.The Contractor shall screen patients for eligibility to receive publicly funded vaccine. 4.The Contractor shall send immunization data to CIIS for all immunizations administered by the agency within seven(7)days of vaccine administration. 5.The Contractor shall promote use of CIIS to providers and schools in the jurisdiction. 6.The Contractor shall review the county level Immunization Rates Report provided by CIB for the following information: a. Children 19-35 months of age no later than 30 days after distribution by CIB b. Adolescents 13-17 years of age no later than 30 days after distribution by CIB c. Compare to the CDC National Immunization Surveys;Child and Teen and CIIS-generated statewide rates provided by CIB in order to increase awareness of county versus state and national immunization rates within the 30 days that the rates report is distributed by the CIB. The Contractor shall participate in the Bridge Access Program to provide COVID-19 vaccines to the following adult populations: a.Uninsured b.Underinsured 2. The Contractor shall offer COVID-19 vaccines alongside other routine vaccines. 3. The Contractor shall schedule a minimum of two(2)monthly vaccination clinics to increase access according to the following criteria: a. One of the two clinics must be scheduled during evening hours, after regular business hours. The Contractor shall promote publicly funded COVID-19 vaccines available through the following programs: a. VFC b. Bridge Access Program Standards and 1. The content of electronic documents located on CDPHE and non-CDPHE Requirements websites and information contained on CDPHE and non-CDPHE websites may be updated periodically during the contract term.The contractor shall monitor documents and website content for updates and comply with all updates. 2. The Contractor shall use the final results of the CDC National Immunization Surveys;Child and Teen and CIIS-generated statewide rates provided by CIB via email communication in order to increase awareness of county versus state and national immunization rates. 3. The Contractor shall comply with the requirements for entering/submitting immunization data into CIIS as agreed to in the CIIS Letter of Agreement found within the online CIIS Resource Center located on the following website,https://www.cophr.com/emrlogin.asp.The content of this website is incorporated and made part of this contract by reference. 4.
