Please find the attached Takeout and Delivery Permit issued by the Liquor Enforcement Division.
Please note that the Takeout and Delivery Permit shall be displayed at all times in a prominent place on the licensed
premises and that the Takeout & Delivery permittee's employee making delivery shall be required to carry, or have
immediate access to a copy of the Takeout and Delivery permit in the delivery vehicle. The copy of the permit may be
electronic.
