I. PROJECT BACKGROUND AND OVERVIEW
The Office of Public Health Practice,Planning,and Local Partnerships(OPHP)invites you to
apply for the OPHP FFY 2022-23 Preventive Block Grant(PBG)via this Request for Application
(RFA)Goggle form.
The purpose of this RFA Google form is not to be competitive,but to better understand and
match the needs to the funding.
This funding supports assessment,planning,capacity assessment,and public health
improvement(CHAPS)efforts of individual LPHAs,district LPHAs,and regional or
cross-jurisdictional LPHA partnerships. OPHP's goal is to support a wide variety of LPHAs
throughout Colorado and to ensure that this funding supports LPHAs with CHAPS needs.
This funding may be used to fund CHAPS work for the following;
1)An LPHA-to-LPHA partnership
2)Districts(LPHAs with multiple counties)
3)Single County LPHAs
It is anticipated that the funding wilt scale appropriately for RFA Goggle form responses with
more counties and partnerships,depending on the proposed work/initiative/project.
The total block grant amount being distributed is$450,000.
Note on LPHA to LPHA Partnerships(P1 above)
Partnerships do not have to be format,contractual arrangements and they do not have to be
geographically proximate.We encourage partnerships among any LPHAs that have common
goals and priorities.
Thematic partnerships are also encouraged.These are collaborations anchored by a public
health priority area and close geographic proximity is not necessary.An example of a
thematic partnership would be LPHA 1(in the northwest),LPHA 2(in the east),and LPHA 3
(in the southwest)collaborating on suicide prevention.

2. Indirect(FEtA)Cost Rate:
Definition:Indirect costs are those that have been incurred for common or
joint objectives and cannot be readily identified with a particular final cost
objective or grant account.Indirect costs may be called Facilities and
Administration costs(FEtA)at some agencies.Indirect costs are different from
administrative costs in most cases.For example,a program employee can be
considered administrative and not be included in an agency's indirect costs.
Costs classified as indirect can differ depending upon your organizational
structure and accounting practices but some common examples include
depreciation on buildings and equipment,operating and maintenance costs of
facilities,and general administrative expenses such as the salaries and
expenses of executive officers and accounting or legal staff.

V. SELECTION,EVALUATION,AND AWARDS
The technical aspects of applications will be assessed based on the soundness of the
applicant's approach and the applicant's understanding of the requirement.Past

experience/qualifications will be assessed by considering the extent to which the
qualifications,experience,and past performance are likely to foster successful,
on-time performance.Technical and past experience assessments may include a
judgment concerning the potential risk of unsuccessful or untimely performance,and
the anticipated amount of State resources necessary to insure timely,successful
performance.The State may use all information available regarding past performance
as defined in C.R.S.§24-106-107 et.seq.
The program has carefully designed a scoring and selection process to ensure fair
selection of the best qualified applicants.The criteria for scoring includes budget,
project design,feasibility,partnerships and alignment with the CHAPS process.The
criteria for scoring are in direct correlation to the required application components.
Applications that fail to follow all of the requirements may not be considered.
Financial Risk Assessment Rating in Evaluation
The financial risk rating determined from the submitted Financial Risk Assessment
Questionnaire or FRMS rating(Local Public Health Agencies only)will be communicated
to reviewers during the application review process.Applications that fail to follow all

This Request for Applications(REA)is issued by the Colorado Department of
Public Health and Environment(CDPHE),also referred to as the"State".The
CDPHE contact listed in these instructions is the sole point of contact
concerning this RFA.
During the solicitation process for this RFA,all official communication with
applicants will be via notices on the CDPHE program website listed in the
schedule of activities.Notices may include any modifications to administrative
or performance requirements,answers to inquiries received,clarifications to
requirements,and the announcement of the apparent winning applicant(s).

II. Project Description:This project serves to support Local Public Health Agency(LPFiA)assessment,planning.
capacity assessment,and public health improvement(CHAPS)efforts for partnerships,districts,and single
LPI-tAs during the current Preventive Health and Health Services Block Grant cycle that ends on September 30,
2023..By using the updated Colorado Core Public Health Capabilities and Services framework that modernizes
government public health,LPHAs will he able to better meet a growing population and address complex public
health needs in their assessments,planning,capacity assessments,and public health improvement(CHAPS)
efforts.This health project will implement a work plan to progress assessment,planning,capacity assessment,and
public health improvement(CHAPS)efforts for Weld County Department of Public Health and Environment.
The Preventive Block Grant(PBG)is a federal grant that is distributed and managed by the Office of Public
Health Practice,Planning,and Local Partnerships(OPHP)at the Colorado Department of Public Health and
Enviromttent(CDPI-iE)and is therefore,subject to federal funding guidelines,rules,and regulations.The PBG
operates on a federal funding cycle that runs front October l to September 30.

Contractor and OPHP that will take place no later than thirty(30)days after contract

OPHP will schedule the post-award meeting with the Contractor with a minimum of 7
calendar days of notice prior to the meeting.

V. Additional Provisions:

The following terms and conditions are in addition to the standard purchase order terms and conditions
and are to he read and interpreted in conjunction with the provisions of the purchase order.Wherever used
i►►die following provisions,"Contractor"and"Vendor"shall have the same meaning.Contractor and/or Vendor—
any party to which a Purchase Order is issued.

VI. Monitoring:
CDPI-IE's monitoring of the purchase order for compliance with performance requirements will be conducted
throughout the purchase order period by the Senior Public Health Systems&Liaison Professional of the Office of
Public Health Practice,Planning,and Local Partnerships.Methods used will include a review of documentation
determined by CDPHE to be reflective of performance to include progress reports and other fiscal and
programmatic documentation as applicable
VII. Resolution of Non-Compliance:
The Contractor will be notified in writing within ten(10)calendar days of discovery of a compliance issue.
Within(30)calendar days of discovery,the Contractor and the State will collaborate,when appropriate,to
determine the action(s)necessary to rectify the compliance issue and determine when the action(s)must be
completed.The action(s)and timeline for completion will be documented in writing and agreed to by both parties.
If extenuating circumstances arise that require an extension to the timeline,the Contractor must email a request to
the Senior Public Health Systems&Liaison Professional of the Office of Public Health Practice,Planning,and
Local Partnerships and receive approval for a new due date. The State will oversee the
completion/implementation of the action(s)to ensure timelines are met and the issue(s)is resolved.If the
Contractor demonstrates inaction or disregard for the agreed upon compliance resolution plan,the State may
exercise its rights under the Terms and Conditions of this Purchase Order.
VIII. Attestation:
The Vendor agrees to perform services in accordance with the terms and conditions of the Purchase Order to
include Statement of Work and Budget.

1.What CHAPS phase are you currently in?
We are in Phase 3 of current CHAPS
WCDPHE has been doing a local CHAPS process and learning from it since the inception of the 2008
Public Health Act.We were one of the State of Colorado's pilot PHIP health agencies.We conducted
assessment and planning activities beginning in 2007 culminating in our first local community health
improvement plan in 2012.In 2016,we achieved PHAB accreditation which helped us further improve
our local CHIP process.We are currently in our 3`d CHIP cycle.Our first CHIP cycle was a five-year cycle
which focused on three priorities—Healthy Eating,Active Living,and Mental Wellbeing.After that first
cycle,in conjunction with our partners,we decided to implement three-year CHIP cycles to better align
with our health care partners and also better align with our ongoing triennial community health
assessment survey.Our second CHIP cycle also focused on healthy eating,active living,and mental
wellbeing.Our current CHIP priorities are mental health and housing.You would think we would have
our CHAPS process pretty refined by now,but we are still learning and improving.Hence,we are
applying for this funding.PHAB accreditation requires continuous updating of our community health
assessment which is what we are doing currently and are seeking funding assistance from OPHP to help
us take a closer look at one of our priority areas.
2.What challenges are you facing in your assessment,planning,or implementation efforts and what is
needed to overcome them?
An ongoing challenge has been our need to include timely additional information from our residents on
their lived experience related to some of their largest health concerns.Community voice is essential in
better understanding the trends and data we're collecting quantitatively and further broaden
everyone's understanding of the data from our robust probability-based community health survey.
Preliminary analyses from our 2022 Community Health Survey shows that resident's top health concerns
include:
• healthcare access,quality,and availability and insurance costs
• the impact of COVID-19
• mental health issues(including access)
• air quality and pollution
• aging and older adult health
• housing and availability and affordability
• the impact on personal health of chronic diseases
Although we would like to conduct focus groups on each of these topics,we know that is not feasible
for now.To overcome this challenge,we want to conduct several focus groups with different hard-to-
reach populations across Weld County to expand on the data collected in our survey.We are finalizing
the plans over the next 30-45 days with the goal of conducting focus groups in October and November
2022.We know that one possible topic area to get more in-depth information from our focus groups
will be around mental health access and wellbeing.We are closely collaborating with our local mental
health agency—North Range Behavioral Health—to organize these focus groups and we are planning to
work with a qualified vendor to actually conduct and provide either transcripts,notes,and themes or a
more formal analysis of the data from the focus groups.
3.What activities and outcomes will your program achieve over the next year(s)?
Activities include planning,scheduling,conducting,analyzing,and disseminating the findings from the
focus groups.Most of the planning and scheduling activities should be complete by the time this grant is
awarded but a health department data analyst's time will be needed to ensure successful
implementation of this activity.Additionally,data collected from focus groups will need to be aligned
with other data from the community health survey as well as other potential sources to provide a better
picture of how residents experience health and health issues in Weld County.The health department
data analyst will take the findings from the focus groups and other data and prepare a report as well as
give presentations to our community partners including the coalition that oversees our CHIP activities—
Thriving Weld.Another beneficial outcome will be the better understanding of key health issues by
stakeholders in Weld County through the creation of reports specific to our health topics of interest
such as healthcare access,mental health,etc.that utilize data from focus groups and the survey.
Deliverables for the grant include timely final workplans,budgets,and grant reports.
4.How will this funding be used to achieve your objectives.
This funding will be used to support partial funding for a qualified focus group facilitation vendor and for
some salaries of health department staff to carry out the activities to plan,conduct,analyze,and
disseminate focus group data in order to create reports that are able to provide a vital means of sharing
community experiences and elevating the voice of hard-to-reach and vulnerable community members.
By funding these activities,a better understanding of key health issues of great importance to Weld
County communities can be realized and stories on the effects of mental health and mental healthcare
access can be shared with the broader community to further complement the data gathered from the
Community Health Survey.The funding of salaries is not due to any loss of state funds for these
staff.Other costs incurred for the focus groups such as mileage,food,childcare or incentives will be paid
by Weld County Government or possibly a community partner organization.Any unspent funds will be
returned to OPHP.
