RE: APPROVE GRANT APPLICATION FOR HOMELESSNESS INITIATIVES
INNOVATIONS PROGRAM AND AUTHORIZE DEPARTMENT OF HUMAN SERVICES
TO SUBMIT ELECTRONICALLY
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Grant Application for the Homelessness
Initiatives Innoivations Program from the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human Services,
to the Colorado Department of Human Services, with further terms and conditions being as stated
in said grant application, and
WHEREAS, after review, the Board deems it advisable to approve said grant application,
a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Grant Application for the Homelessness Initiatives Innoivations
Program from the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, to the Colorado
Department of Human Services, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that Julie Witkowski, Family Resource
Division Manager, be, and hereby is, authorized to electronically said grant application.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 22nd day of March, A.D., 2023.

Housing Navigator Grant and Homelessness
Initiatives Innovation Grant Request for Application
Please review and indicate if you would like a work session prior to placing this item on the
Board's agenda.
Request Board Approval of the Department's Colorado Department of Human Services
(CDHS) Housing Navigator Grant and Homelessness Initiatives Innovation Grant Request
for Application. The Department of Human Services is requesting approval to submit
applications for funding for the Colorado Department of Human Services (CDHS) Housing
Navigator Grant Program and Homelessness Initiatives Innovation Grant Program.
The Housing Navigator Grant will establish an Intensive Housing Navigator position within the
Weld County Department of Human Services (WCDHS). In alignment with statewide priorities
and to support local community needs this position, if awarded, would provide intensive case
management to individuals and families experiencing, or at risk of, homelessness who are
receiving assistance from a WCDHS program such as TANF and Old Age Pension, and to serve
priority populations of WCDHS to include seniors, individuals with disabilities and foster
youth. WCDHS's response to the grant, via survey, will include $120,000 ($500,000 available
statewide) to support the Housing Navigator and related services through June 30, 2024.
In addition, Weld County will request funds made available through a CDHS Homelessness
Initiatives Innovation Grant in the amount of $50,000 ($250,000 available statewide). The
Homelessness Initiatives Innovation Grant will establish internal access to funds designed to
prevent homelessness for WCDHS families, seniors, people with disabilities, and foster youth
who are not eligible to receive housing assistance from other funding sources.
These grant applications must be received by 5:00 p.m. MST Wednesday, March 22, 2023. If
grant money is awarded the anticipated contract start date is upon execution through June 30,
2024.

I do not recommend a Work Session. I recommend approval for the Department to submit an application
for funding for both the Housing Navigator Grant and the Homelessness Initiatives Innovations Grant
and delegate authority to the Family Resource Division Head to submit both applications electronically.

Innovation in Homelessness Initiatives
• Amount Requested: $50,000.00
• What particular innovation are you proposing and what outcomes do you hope to generate
as a result of this project?
WCDHS is seeking to establish a supportive services fund for WCDHS customers
experiencing or at risk of homelessness. Funds (with appropriate documentation) will be
payable directly to landlords as eviction prevention (up to three months' rent) or to assist
households with securing new housing (deposit and up to two months' rent). An estimated
20 households will be served. Funds will be approved only if the household cannot
independently meet the need, if the need cannot be met through other federal and state
entitlement programs and if the customer is not eligible for or has exhausted ERA, TANF or
CSBG funding.
• Can you describe the need for this initiative in your county?
According to the National Low Income Housing Coalition, the fair market rent rate for a two -
bedroom apartment in Weld County in 2022 was $1,272.00. The average annual income for
Weld County renters was $37,585.60 and the income needed to sustain a $1,272.00 rent
payment was $50,880.00. In 2022, over 50% of the households that received assistance
through the ERA program were at or below 30% of the area median income (AV l). Monthly
AVI for a household of four in 2022 was $2,475.00, totaling $29,700.00 annually,
significantly lower than the $50,880.00 needed to sustain rent on a two -bedroom
apartment.
• Describe your county's capacity to implement the initiative in a timely manner.
The Family Resource Division (FRD) within WCDHS is uniquely positioned to implement
supportive service funding. FRD oversees our Human Services Funds Distribution (HSFD)
program, a collaboration of representatives from all Divisions within the Department. HSFD
reviews all WCDHS requests for supportive services on a weekly basis to ensure eligibility,
avoid duplication and to identify appropriate source of funding for eligible requests. In
addition, FRD has successfully managed the Community Services Block Grant for over 10
years. And finally, FRD has successfully implemented the ERA program since 2021 and is in
the process of expanding ERA to include housing case management.
• Describe your county's alignment with CDHS' priorities for service populations.
Supportive Services funding will be targeted to support households who are financially
eligible for food, cash assistance and medical benefits. Funds will be prioritized for WCDHS
families, seniors, people with disabilities and youth who are exiting or who have been in
foster care.

Last month, OHS' Family Resource Division applied for two grants through the State. The first application was for the
Housing Navigator Program (Tyler ID 2023-0825). This program was awarded in full, for a total of $120,000.00. The
second application was for the Homelessness Initiative Innovations Program (Tyler ID 2023-0826). This program was not
awarded at this time. The attached award notice from the State is the official notification for both grants.

Happy Friday. I'm writing with what I hope is very good news. Our committee reviewed your
applications and is pleased to offer you your full $120,000 request for the Housing Navigator Grant.
At this time, we are not able to offer additional funds for your Innovation Grant Application, but the
committee very much appreciated that application as well and if others do not select to move forward
your application may be reconsidered at that time.
As a next step, could we find some time in the next week or two to meet and discuss the Scope of Work
required for contracting?
