WHEREAS the parties hereby agree to amend the term of the Original Agreement in accordance with the terms of the Original Agreement and any previously adopted amendment, which is incorporated by reference herein, as well as the terms provided herein. NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as follows: • The Original Agreement will end on June 30, 2023. • This Amendment, together with the Original Agreement, constitutes the entire understanding between the parties. The following additional changes are hereby made to the current Agreement as of July 1, 2023: 1. General Description of Contractor including target population, area served, and program being proposed. Contractor has offices in Greeley, Frederick, and in Fort Lupton supporting all of Weld County, Colorado. The North Range Behavioral Health (NRBH) Family Connects teams provide a continuum of prevention to intervention services for providers and families with young children (prenatally to eight (8) years of age). The Infant Early Childhood Mental Health (IECMH) Consultant uses the research informed practices that the Colorado Department of Early Childhood (CDEC) has adopted to address challenging behaviors in young children and influence a reduction in child care suspensions and expulsion rates. NRBH — Family Connects has been providing Infant Early Childhood Mental Health Consultation (IECMHC) focused services in child care settings since 2005. The IECMH Consultation team, at the direction of Employment Services of Weld County (hereinafter ESWC) staff, will provide Early Childhood Mental Health (ECMH) Consultation primarily to prospective, new and existing Weld County Child Care Home Providers. Secondarily, the IECMH Consultation team may provide early ECMH at Child Care Centers. These supports will focus on growing provider knowledge and skills to support children birth to six (6) years of age, with additional support through before and after school care for children six (6) to eight (8) years of age. These supports will be provided in the home and in center provider settings and align with State rules and regulations with an emphasis on reducing challenging behaviors. The IECMH Consultant can also act as a resource for parents whose children, ages birth to six (6) years of age, exhibit behavioral concerns. The IECHM Consultant may recommend solutions for the child care provider and/or parent and the Weld County Department of Human Services (WCDHS) to mitigate behavioral concerns in the home and make appropriate referrals to community resources and partners, to strengthen the health of a child and to improve the quality of child care. In addition, the IECMH Consultant, at the direction of ESWC may also support kinship, foster care, protective services, and Temporary Assistance for Needy Families (TANF) homes. 2. Type of service, brief description, and rate for each proposed service. Description of service should include any assessment tools or specific practices used in the provision of that service. IECMHC is a multi -level preventive intervention that partners mental health therapists with individuals who work with young children and their families to improve their social, emotional, and behavioral health and development. IECMHC builds the capacity of providers and families to understand the powerful influence of their relationships and interactions on young children's development. IECMHC includes skilled observations, individualized strategies, and early identification of children with and at risk for mental health challenges. One important goal of the CDEC is to reduce the rates of expulsion in the early care and education settings (child care settings) with a goal of eliminating early childhood expulsion. The U.S. Department of Health and Human Services and the U.S. Department of Education published a policy statement that emphasizes the need for prevention through additional resources, increased workforce competencies, the use of evidence -based interventions, and early childhood mental health consultation. Infant Early Childhood Mental Health Consultation: Child Age Range: Birth to eight (8) years of age (child specific impact ages). Content Age: IECMHC will provide knowledge and content primarily to child care home providers for children birth to eight (8) years of age. Adult Age: Any age for adult providers and family members. Gender Criteria: All Goals: Utilize consultation, reflective supervision, and training to increase Providers' skills to provide environments that help children be successful and gain social and emotional skills. Equip child care providers and families with the knowledge, skills and resources needed to reduce disruption in care for young children with challenging behaviors. Engage providers and families in the development of Individual Support Plans (ISP) when there are identified challenging behaviors and needs for individual children. Provide the consultation, modeling, and support for caregivers to implement the ISP and strategies. Complete, as appropriate, the Ages and Stages Questionnaire Social and Emotional (ASQ: SE) on children six (6) months to five (5) years of age as a screening tool to assess and monitor social and emotional development. Complete as appropriate the Devereux Early Childhood Assessments (DECA) — Clinical assessment to monitor children birth to five (5) years of age that have identified challenging behaviors. Complete, as appropriate, the Parent Interactions with Children Observation Interaction (PICCOLO) to measure adult home provider change in care giving and relationships. Length of Service: Providers that are offered embedded IECMHC will be selected by ESWC in partnership with the IECMHC Consultant. IECMHC is an ongoing relationship that continues a minimum of one (1) year with increases/decreases in time commitment based on presenting challenges. Embedded collaborative agreements are established with child care providers and reviewed at minimum every six (6) months. Primary Intervention Type: IECMHC, training, screening/assessments, psychoeducation, prevention, and referrals to therapeutic intervention as needed Special Services: Access to Infant Mental Health Endorsed therapists that are trained in evidence -based practices with young children, families, and provider. IECMHC range of ability includes prevention and intervention services. Inclusionary Criteria: Early child care providers will be encouraged to participate in embedded consultation model focused on prevention and reducing suspensions and/or expulsions. Providers will ensure they have a written policy and practice as required by Colorado Department of Human Services (CDHS) and CDEC. This plan will be utilized with families and guide the reduction of expulsions from care. Rules that address the need for the provision of individualized social and emotional intervention supports and positive behavior support plans for any child in need. These new rules, 7.702.31 (H) and 7.702.31(Z) are effective February 1, 2016 and can be found at the Colorado Secretary of State website. Families of children with identified challenging behaviors are an integral part of the behavior plan. The IECMH Consultant will engage the family and obtain permission to do child specific work. The expectation for early care providers is that they will consistently engage in IECMHC. Consultation will be scheduled to ensure that providers can maintain adequate supervision of children. Maximum Program Capacity: Adults are the focus of this work and capacity is dependent on intensity of needs. It is estimated that a full time IECMH Consultant can be embedded in no more than nine (9) to twenty- four (24) home provider settings. The range will fluctuate based on the number of child specific cases or intensity of needs for providers. Family Connects will utilize a team approach to be able to meet the range of needs of early child care providers based on their language and location. Considerations will include if an IECMH consultant is already embedded at a site or is serving in that region. A typical week will consist of an estimated 24 hours in direct work with providers. The other 16 hours will include supervision, planning, professional development, meetings internally and at WCDHS and in the community. Medicaid Eligibility Medicaid and insurance do not pay for consultation, training and reflective supervision supports. Children with significant issues that are referred for mental health services (this requires a diagnosis) will have their therapeutic intervention billed to CHP+, insurance, Medicaid and/or family payor source. Referral Process/Timelines: The focus of this IECMHC contract will be to embed consultation in home settings that support Child Care Assistance Program (CCAP) eligible children/families. ESWC will work with home providers to schedule orientation meetings. ESWC will have IECMH consultant meet to explain supports and initiate consultation if interested. IECMHC FTE will communicate within two (2) business days to referring case manager and Child Care Manager initial plan and all developments throughout consultation. Supervisory Structure: Ongoing supervision for the IECMH Consultant will be provided by NRBH staff. ESWC staff will provide support to consultant during hours at WCDHS. Hiring for the consultant will be by NRBH but may include ESWC. IECMHC Outcomes in Weld County (2023-2024) The Family Connects IECMHC team will work to ensure the provision of services outlines in this contract using the entire team. The IECMH Program Director will be the point person working closely with WCDHS to ensure that the providers, families, and other needs can be addressed quickly. IECMH consultation team will work with twelve (12) to twenty-four (24) home child care providers. Number will vary based on location, number of challenging behaviors in home, and needs of providers. IECMH consultant will impact through periodic screening potentially thir -six (36) to one hundred sixty (160) children (school readiness and developmental screenings). Number will vary based on home provider capacity. Friends, Family and Neighbor (FFN) providers can only have four (4) children in care with licensed home settings typically capped at eight (8) children. Every provider engaged in embedded consultation will establish a strong process to address challenging behaviors o Written policy and procedures o Prevention focused (at first concern of behaviors) o Utilize an ISP Process • Parents will be actively engaged in the ISP process when children are identified with challenging behaviors. • Providers will participate in regular meetings with IECMH consultant to ensure a preventive approach. • The IECMH consultant and providers will -establish a baseline for suspensions and expulsions to serve as the starting point to determine reductions in suspensions/expulsions. • IECMH team will work with ESCW to establish a system to identify children at risk for suspensions and expulsions from all Weld County Child Care Programs. Consultants will utilize the Preschool Expulsion Risk Measure (PERM) to gather and report data. • IECMH Program leadership will work with ESCW to determine a schedule to provide IECMH training to WCDHS staff two (2) to three (3) times, or as requested to include initial introduction to IECMH deliverables. • Screening and assessments will be completed to measure school readiness, developmental progress for children and adult child engagement. Any amendments or modifications to this agreement shall be in writing signed by both parties. No additional services or work performed by Contractor shall be the basis for additional compensation unless and until Contractor has obtained written authorization and acknowledgement by County for such additional services. 6. Compensation/Contract Amount. County agrees to pay an amount no greater than $93,322.00 for the term of this Agreement, as set forth in Paragraph 3. County agrees to pay Contractor through an invoice process during the course of this Agreement in accordance with the Budget as described in Exhibit A. Contractor agrees to submit invoices which detail the work completed by Contractor. The County will review each invoice and if it agrees Contractor has completed the invoiced items to the County's satisfaction, it will remit payment to Contractor. Contractor agrees to work within the confines of the Scope of Services outlined in Exhibit A. 2o - oZo -7 County will not withhold any taxes from monies paid to the Contractor hereunder and Contractor agrees to be solely responsible for the accurate reporting and payment of any taxes related to payments made pursuant to the terms of this Agreement. 7. Independent Contractor. Contractor agrees that it is an independent Contractor and that Contractor's officers, agents or employees will not become employees of County, nor entitled to any employee benefits from County as a result of the execution of this Agreement. Contractor shall perform its duties hereunder as an independent Contractor. Contractor shall be solely responsible for its acts and those of its agents and employees for all acts performed pursuant to this Agreement. Contractor, its employees and agents are not entitled to unemployment insurance or workers' compensation benefits through County and County shall not pay for or otherwise provide such coverage for Contractor or any of its agents or employees. Unemployment insurance benefits will be available to Contractor and its employees and agents only if such coverage is made available by Contractor or a third party. Contractor shall pay when due all applicable employment taxes and income taxes and local head taxes (if applicable) incurred pursuant to this Agreement. 8. Subcontractors. Contractor acknowledges that County has entered into this Agreement in reliance upon the particular reputation and expertise of Contractor. Contractor shall not enter into any subcontractor agreements for the completion of this Project without County's prior written consent, which may be withheld in County's sole discretion. 9. Ownership. All work and information obtained by Contractor under this Agreement or individual work order shall become or remain (as applicable), the property of County. 10. Confidentiality. Contractor agrees to keep confidential all of County's confidential information. Contractor agrees not to sell, assign, distribute, or disclose any such confidential information to any other person or entity without seeking written permission from the County. Contractor agrees to advise its employees, agents, and consultants, of the confidential and proprietary nature of this confidential information and of the restrictions imposed by this agreement. 11. Warranty. Contractor warrants that the services performed under this Agreement will be performed in a manner consistent with the standards governing such services and the provisions of this Agreement. Contractor further represents and warrants that all services shall be performed by qualified personnel in a professional and workmanlike manner, consistent with industry standards, and that all services will conform to applicable specifications. 12. Acceptance of Services Not a Waiver. In no event shall any action by County hereunder constitute or be construed to be a waiver by County of any breach of this Agreement or default which may then exist on the part of Contractor. Acceptance by the County of, or payment for, the services completed under this Agreement shall not be construed as a waiver of any of the County's rights under this Agreement or under the law generally. 13. Insurance and Indemnification. Contractor shall procure at least the minimum amount of automobile liability insurance required by the State of Colorado for the use of any personal vehicle. Proof of said automobile liability insurance shall be provided to County prior to the performance of any services under this Agreement. 14. Professional Liability (Errors and Omissions Liability). The policy shall cover professional misconduct or lack of ordinary skill for those positions defined in the Scope of Services of this contract. Contractor shall maintain limits for all claims covering wrongful acts, errors and/or omissions, including design errors, if applicable, for damage sustained by reason of or in the course of operations under this Contract resulting from professional services. In the event that the professional liability insurance required by this Contract is written on 2 a claims -made basis, Contractor warrants that any retroactive date under the policy shall precede the effective date of this Contract; and that either continuous coverage will be maintained or an extended discovery period will be exercised for a period of two (2) years beginning at the time work under this Contract is completed. Minimum Limits: Per Loss Aggregate $ 1,000,000 $ 2,000,000 15. Indemnity. The Contractor shall defend, indemnify and hold harmless County, its officers, agents, and employees, from and against injury, loss damage, liability, suits, actions, or claims of any type or character arising out of the work done in fulfillment of the terms of this Contract or on account of any act, claim or amount arising or recovered under workers' compensation law or arising out of the failure of the Contractor to conform to any statutes, ordinances, regulation, law or court decree. 16. Non -Assignment. Contractor may not assign or transfer this Agreement or any interest therein or claim thereunder, without the prior written approval of County. 17. Interruptions. Neither party to this Agreement shall be liable to the other for delays in delivery or failure to deliver or otherwise to perform any obligation under this Agreement, where such failure is due to any cause beyond its reasonable control, including but not limited to Acts of God, fires, strikes, war, flood, earthquakes or Governmental actions. 18. Compliance with Law. Contractor shall strictly comply with all applicable federal and State laws, rules and regulations in effect or hereafter established, including without limitation, laws applicable to discrimination and unfair employment practices. 19. Non -Exclusive Agreement. This Agreement is nonexclusive, and County may engage or use other Contractors or persons to perform services of the same or similar nature. 20. Entire Agreement/Modifications. This Agreement including the Exhibits attached hereto and incorporated herein, contains the entire agreement between the parties with respect to the subject matter contained in this Agreement. This instrument supersedes all prior negotiations, representations, and understandings or agreements with respect to the subject matter contained in this Agreement. This Agreement may be changed or supplemented only by a written instrument signed by both parties. 21. Fund Availability. Financial obligations of the County payable after the current fiscal year are contingent upon funds for that purpose being appropriated, budgeted and otherwise made available. Execution of this Agreement by County does not create an obligation on the part of County to expend funds not otherwise appropriated in each succeeding year. 22. Employee Financial Interest/Conflict of Interest — C.R.S. §§24-18-201 et seq. and §24-50-507. The signatories to this Agreement state that to their knowledge, no employee of Weld County has any personal or beneficial interest whatsoever in the service or property which is the subject matter of this Agreement. 23. Severability. If any term or condition of this Agreement shall be held to be invalid, illegal, or unenforceable by a court of competent jurisdiction, this Agreement shall be construed and enforced without such provision, to the extent that this Agreement is then capable of execution within the original intent of the parties. 24. Governmental Immunity. No term or condition of this contract shall be construed or interpreted as a waiver, 3 express or implied, of any of the immunities, rights, benefits, protections or other provisions, of the Colorado Governmental Immunity Act §§24-10-101 et seq., as applicable now or hereafter amended. 25. Force Majeure. Neither the Contractor nor the County shall be liable for any delay in, or failure of performance of, any covenant or promise contained in this Agreement, nor shall any delay or failure constitute default or give rise to any liability for damages if, and only to extent that, such delay or failure is caused by "force majeure." As used in this Agreement, "force majeure" means acts of God, acts of the public enemy, unusually severe weather, fires, floods, epidemics, quarantines, strikes, labor disputes and freight embargoes, to the extent such events were not the result of, or were not aggravated by, the acts or omissions of the non- performing or delayed party. 26. No Third -Party Beneficiary. It is expressly understood and agreed that the enforcement of the terms and conditions of this Agreement, and all rights of action relating to such enforcement, shall be strictly reserved to the undersigned parties and nothing in this Agreement shall give or allow any claim or right of action whatsoever by any other person not included in this Agreement. It is the express intention of the undersigned parties that any entity other than the undersigned parties receiving services or benefits under this Agreement shall be an incidental beneficiary only. 27. Board of County Commissioners of Weld County Approval. This Agreement shall not be valid until it has been approved by the Board of County Commissioners of Weld County, Colorado or its designee. 28. Choice of Law/Jurisdiction. Colorado law, and rules and regulations established pursuant thereto, shall be applied in the interpretation, execution, and enforcement of this Agreement. Any provision included or incorporated herein by reference which conflicts with said laws, rules and/or regulations shall be null and void. In the event of a legal dispute between the parties, Contractor agrees that the Weld County District Court shall have exclusive jurisdiction to resolve said dispute. 29. Public Employment Retirement Program. Contractor is responsible for notifying Weld County of any previous participation in the Colorado Public Employee Retirement Program. Contractor must notify Weld county of the most recent employment for a PERA contributing employer. 30. Public Contracts for Services C.R.S. §8-17.5-101. Contractor certifies, warrants, and agrees that it does not knowingly employ or contract with an illegal alien who will perform work under this contract. Contractor will confirm the employment eligibility of all employees who are newly hired for employment in the United States to perform work under this Agreement, through participation in the E -Verify program or the State of Colorado program established pursuant to C.R.S. §8-17.5-102(5)(c). Contractor shall not knowingly employ or contract with an illegal alien to perform work under this Agreement or enter into a contract with a subcontractor that fails to certify with Contractor that the subcontractor shall not knowingly employ or contract with an illegal alien to perform work under this Agreement. Contractor shall not use E -Verify Program or State of Colorado program procedures to undertake pre -employment screening or job applicants while this Agreement is being performed. If Contractor obtains actual knowledge that a subcontractor performing work under the public contract for services knowingly employs or contracts with an illegal alien Contractor shall notify the subcontractor and County within three (3) days that Contractor has actual knowledge that a subcontractor is employing or contracting with an illegal alien and shall terminate the subcontract if a subcontractor does not stop employing or contracting with the illegal alien within three (3) days of receiving notice. Contractor shall not terminate the contract if within three days the subcontractor provides information to establish that the subcontractor has not knowingly employed or contracted with an illegal alien. Contractor shall comply with reasonable requests made in the course of an investigation, undertaken pursuant to C.R.S. §8-17.5-102(5), by the Colorado Department of Labor and Employment. If Contractor participates in the State of Colorado program, Contractor shall, within twenty days after hiring a new employee to perform work under the contract, affirm that Contractor has examined the legal 4 work status of such employee, retained file copies of the documents, and not altered or falsified the identification documents for such employees. Contractor shall deliver to County, a written notarized affirmation that it has examined the legal work status of such employee and shall comply with all of the other requirements of the State of Colorado program. If Contractor fails to comply with any requirement of this provision or of C.R.S. §8-17.5- 101 et seq., County, may terminate this Agreement for breach, and if so terminated, Contractor shall be liable for actual and consequential damages. Except where exempted by federal law and except as provided in C.R.S. § 24-76.5-103(3), if Contractor receives federal or state funds under the contract, Contractor must confirm that any individual natural person eighteen (18) years of age or older is lawfully present in the United States pursuant to C.R.S. § 24-76.5-103(4), if such individual applies for public benefits provided under the contract. If Contractor operates as a sole proprietor, it hereby swears or affirms under penalty of perjury that it: (a) is a citizen of the United States or is otherwise lawfully present in the United States pursuant to federal law, (b) shall produce one of the forms of identification required by C.R.S. § 24-76.5-101, et seq., and (c) shall produce one of the forms of identification required by C.R.S. § 24- 76.5-103 prior to the effective date of the contract. 31. Acknowledgment. County and Contractor acknowledge that each has read this Agreement, understands it and agrees to be bound by its terms. Both parties further agree that this Agreement, with the attached Exhibit A, is the complete and exclusive statement of agreement between the parties and supersedes all proposals or prior agreements, oral or written, and any other communications between the parties relating to the subject matter of this Agreement. General Description of Contractor including target population, area served, and program being proposed. Contractor has offices in Greeley, Frederick, and in Fort Lupton supporting all of Weld County, Colorado. The North Range Behavioral Health (NRBH) Family Connects teams provide a continuum of prevention to intervention services for providers and families with young children (prenatally to eight (8) years of age). The Infant Early Childhood Mental Health (IECMH) Consultant uses the research informed practices that the Colorado Office of Early Childhood (OEC) has adopted to address challenging behaviors in young children and influence a reduction in child care suspensions and expulsion rates. NRBH — Family Connects has been providing Infant Early Childhood Mental Health Consultation (IECMHC) focused services in child care settings since 2005. The IECMH Consultation team, at the direction of Employment Services of Weld County (hereinafter ESWC) staff, will provide Early Childhood Mental Health (ECMH) Consultation primarily to prospective, new and existing Weld County Child Care Home Providers. Secondarily, the IECMH Consultation team may provide early ECMH at Child Care Centers. These supports will focus on growing provider knowledge and skills to support children birth to six (6) years of age, with additional support through before and after school care for children six (6) to eight (8) years of age. These supports will be provided in the home and in center provider settings and align with State rules and regulations with an emphasis on reducing challenging behaviors. The IECMH Consultant can also act as a resource for parents whose children, ages birth to six (6) years of age, exhibit behavioral concerns. The IECHM Consultant may recommend solutions for the child care provider and/or parent and the Weld County Department of Human Services (WCDHS) to mitigate behavioral concerns in the home and make appropriate referrals to community resources and partners, to strengthen the health of a child and to improve the quality of child care. In addition, the IECMH Consultant, at the direction of ESWC may also support kinship, foster care, protective services and Temporary Assistance for Needy Families (TANF) homes. 2. Type of service, brief description and rate for each proposed service. Description of service should include any assessment tools or specific practices used in the provision of that service. IECMHC is a multi -level preventive intervention that partners mental health therapists with individuals who work with young children and their families to improve their social, emotional, and behavioral health and development. IECMHC builds the capacity of providers and families to understand the powerful influence of their relationships and interactions on young children's development. IECMHC includes skilled observations, individualized strategies, and early identification of children with and at risk for mental health challenges. One important goal of the OEC is to reduce the rates of expulsion in the early care and education settings (child care settings) with a goal of eliminating early childhood expulsion. The U.S. Department of Health and Human Services and the U.S. Department of Education published a policy statement that emphasizes the need for prevention through additional resources, increased workforce competencies, the use of evidence -based interventions, and early childhood mental health consultation. Infant Early Childhood Mental Health Consultation: Child Age Range: Birth to eight (8) years of age (child specific impact ages). Content Age: IECMHC will provide knowledge and content primarily to child care home providers for children birth to eight (8) years of age. Adult Age: Any age for adult providers and family members. Gender Criteria: All Goals: Utilize consultation, reflective supervision, and training to increase Providers' skills to provide environments that help children be successful and gain social and emotional skills. Equip child care providers and families with the knowledge, skills and resources needed to reduce disruption in care for young children with challenging behaviors. Engage providers and families in the development of Individual Support Plans (ISP) when there are identified challenging behaviors and needs for individual children. Provide the consultation, modeling and support for caregivers to implement the ISP and strategies. Complete, as appropriate, the Ages and Stages Questionnaire Social and Emotional (ASQ: SE) on children six (6) months to five (5) years of age as a screening tool to assess and monitor social and emotional development. Complete as appropriate the Devereux Early Childhood Assessments (DECA) — Clinical assessment to monitor children birth to five (5) years of age that have identified challenging behaviors. Complete, as appropriate, the Parent Interactions with Children Observation Interaction (PICCOLO) to measure adult home provider change in care giving and relationships. Length of Service: Providers that are offered embedded IECMHC will be selected by ESWC in partnership with the IECMHC Consultant. IECMHC is an ongoing relationship that continues a minimum of one (1) year with increases/decreases in time commitment based on presenting challenges. Embedded collaborative agreements are established with child care providers and reviewed at minimum every six (6) months. Primary Intervention Type: IECMHC, training, screening/assessments, psychoeducation, prevention, and referrals to therapeutic intervention as needed Special Services: Access to Infant Mental Health Endorsed therapists that are trained in evidence -based practices with young children, families, and provider. IECMHC range of ability includes prevention and intervention services. Inclusionary Criteria: Early child care providers will be encouraged to participate in embedded consultation model focused on prevention and reducing suspensions and/or expulsions. Providers will ensure they have a written policy and practice as required by Colorado Department of Human Services (CDHS) and OEC. This plan will be utilized with families and guide the reduction of expulsions from care. Rules that address the need for the provision of individualized social and emotional intervention supports and positive behavior support plans for any child in need. These new rules, 7.702.31 (H) and 7.702.31(Z) are effective February 1, 2016 and can be found at the Colorado Secretary of State website. Families of children with identified challenging behaviors are an integral part of the behavior plan. The IECMH Consultant will engage the family and obtain permission to do child specific work. The expectation for early care providers is that they will consistently engage in IECMHC. Consultation will be scheduled to ensure that providers can maintain adequate supervision of children. Maximum Program Capacity: Adults are the focus of this work and capacity is dependent on intensity of needs. It is estimated that a full time IECMH Consultant can be embedded in no more than nine (9)-24 home provider settings. The range will fluctuate based on the number of child specific cases or intensity of needs for providers. Family Connects will utilize a team approach to be able to meet the range of needs of early child care providers based on their language and location. Considerations will include if an IECMH consultant is already embedded at a site or is serving in that region. A typical week will consist of an estimated 24 hours in direct work with providers. The other 16 hours will include supervision, planning, professional development, meetings internally and at WCDHS and in the community. Medicaid Eligibility Medicaid and insurance do not pay for consultation, training and reflective supervision supports. Children with significant issues that are referred for mental health services (this requires a diagnosis) will have their therapeutic intervention billed to CHP+, insurance, Medicaid and/or family payor source. Referral Process/Timelines: The focus of this IECMHC contract will be to embed consultation in home settings that support Child Care Assistance Program (CCAP) eligible children/families. ESWC will work with home providers to schedule orientation meetings. ESWC will have IECMH consultant meet to explain supports and initiate consultation if interested. IECMHC FTE will communicate within two (2) business days to referring case manager and Child Care Manager initial plan and all developments throughout consultation. Supervisory Structure: Ongoing supervision for the IECMH Consultant will be provided by NRBH staff. ESWC staff will provide support to consultant during hours at WCDHS. The IECMH Program Coordinator will be the point person working closely with WCDHS to ensure that the providers, families and other needs can be addressed quickly. • IECMH consultation team will work with nine (9)-24 home child care providers. Number will vary based on location, number of challenging behaviors in home, and needs of providers. IECMH consultant will impact through periodic screening potentially 36-160 children (school readiness and developmental screenings). Number will vary based on home provider capacity. Friends, Family and Neighbor (FFN) providers can only have four (4) children in care with licensed home settings typically capped at eight (8) children. Every provider engaged in embedded consultation will establish a strong process to address challenging behaviors o Written policy and procedures o Prevention focused (at first concern of behaviors) o Utilize an ISP Process • Parents will be actively engaged in the ISP process when children are identified with challenging behaviors. • Providers will participate in regular meetings with IECMH consultant to ensure a preventive approach. • The IECMH consultant and providers will -establish a baseline for suspensions and expulsions to serve as the starting point to determine reductions in suspensions/expulsions. • IECMH team will work with ESCW to establish a system to identify children at risk for suspensions and expulsions from all Weld County Child Care Programs. • IECMH Program leadership will work with ESCW to determine most effective way to collect data on suspensions and expulsions. Recognizing that trust is a critical element for reflective consultation. • Screening and assessments will be completed to measure school readiness, developmental progress for children and adult child engagement.
