RE: APPROVE ADDENDUM TO CONTRACT FOR YOUTH AT-RISK OF HOMELESSNESS
(YARH) SUMMATIVE EVALUATION AND PARTICIPATION COMPARISON OF
PATHWAYS TO SUCCESS GRANT PROJECT,AND AUTHORIZE CHAIR TO SIGN-
CENTER FOR POLICY RESEARCH
WHEREAS,the Board of County Commissioners of Weld County,Colorado,pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County,Colorado,and
WHEREAS,the Board has been presented with an Addendum to the Contract for Youth
At-Risk of Homelessness (YARH) Summative Evaluation and Participation Comparison of
Pathways to Success Grant Project between the County of Weld,State of Colorado,by and
through the Board of County Commissioners of Weld County,on behalf of the Department of
Human Services,and Center for Policy Research,commencing January 1,2024,and ending
December 31,2024,with further terms and conditions being as stated in said addendum,and
WHEREAS,after review,the Board deems it advisable to approve said addendum,a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County,Colorado,that the Addendum to the Contract for Youth At-Risk of Homelessness
(YARH)Summative Evaluation and Participation Comparison of Pathways to Success Grant
Project between the County of Weld,State of Colorado,by and through the Board of County
Commissioners of Weld County,on behalf of the Department of Human Services,and Center for
Policy Research,be,and hereby is,approved.
BE IT FURTHER RESOLVED by the Board that the Chair be,and hereby is,authorized
to sign said addendum.
The above and foregoing Resolution was,on motion duly made and seconded,adopted
by the following vote on the 27th day of December,A.D.,2023.

Brief description of the problem/issue:The Department's is requesting to amend its contract with the Center
for Policy Research(CPR),a Colorado nonprofit corporation,as a comparison site for the summative
evaluation of Colorado's Pathways to Success project.This contract is known to the Board as Tyler ID#2022-
0907.
The purpose is to prevent and address homelessness among Colorado's eligible Youth At-Risk of
Homelessness(YARH),(ages 14-23),by implementing with fidelity the Pathways Model Intervention Engaging
Youth in a Coach Like Way.The objective of the project is to assist in building evidence for the comprehensive
service models developed and initially implemented by supporting Sites and conducting a federally led
summative evaluation.Employment Services of Weld County(ESCW)will be participating as a comparison
site,which will involve specific activities as set forth in the contract,including training and technical assistance
activities,implementation activities,evaluation,and project management.
This Addendum will renew the contract for an additional twelve months,effective as of January 1,2024 and
ending at midnight on December 31,2024.
What options exist for the Board?
• Approval of the Center for Policy Research Addendum.
• Deny approval of Center for Policy Research Addendum.
Consequences:Employment Services of Weld County will not participate in the Federal Pathways to
Success project.
Impacts: Children between the ages of fourteen(14)to twenty-three(23)years old who are homeless
or at-risk of becoming homeless will not be provided this service.
Costs(Current Fiscal Year/Ongoing or Subsequent Fiscal Years):
• Total cost=CPR will compensate ESWC in an amount that will not exceed a$10,000 site payment.
ESWC will invoice CPR in accordance with Attachment A.
• Funded through the Administration for Children,Youth and Families/Children's Bureau from a series of
Federally funded grants.

CONTRACT ADDENDUM BETWEEN
WELD COUNTY DEPARTMENT OF HUMAN SERVICES,EMPLOYMENT SERVICES OF ESWC
AND THE CENTER FOR POLICY RESEARCH
Addendum to renew the contract that became effective as of March 1,2021,for an additional
twelve months between Center for Policy Research(CPR),a Colorado nonprofit corporation,and
Department of Human Services—Employment Services of Weld County(ESWC)as a comparison site
for the summative evaluation of Colorado's Pathways to Success project.CPR and ESWC agree to
renew the parties'contract effective as of January 1,2024 and ending at midnight on December 31,
2024.CPR will compensate ESWC for an amount that will not exceed a$10,000 site payment.ESWC
will invoice CPR on a semi-annual basis in accordance with Attachment A.
This contract addendum is entered into effective as of the date set forth above.

SCHEDULE 1 SCOPE OF WORK—YEAR 4
This Scope of Work describes the work to be conducted by Department of Human Services—
Employment Services of ESWC(ESWC)in partnership with the Center for Policy Research(CPR)and its
subcontractors,and the Colorado Department of Human Services,Division of Child Welfare,for the YARH-3
project led by Mathematica Policy Research,Inc.(Mathematica)for the U.S.Department of Health&
Human Services,Administration for Children&Families,Office of Planning,Research,&Evaluation(HHS,
ACF,OPRE),Contract Number HHSP2332015000351/75P00119F37041).
Background information
Starting in 2013,the Administration for Children,Youth and Families/Children's Bureau funded a series of
grants called Youth At-Risk of Homelessness(YARH):Planning Phase(YARH-1),Formative Evaluation Phase
(YARN-2),and the Summative Evaluation Phase(YARH-3).Colorado's YARH grant is known as'Pathways to
Success'.Its purpose is to prevent and address homelessness among Colorado's eligible youth(ages 14-23)
who are homeless or at-risk of becoming homeless by implementing with fidelity the Pathways Model
Intervention Engaging Youth in a Coach Like Way.
In July 2019,ACF launched YARH-3,which will continue to provide important information to the field by
supporting YARH Sites'evidence-building activities.YARH Sites may engage in a range of evidence-building
activities,with related deliverables,requirements,or milestones.YARH-3 will incorporate evaluation TA;
assessments of YARH Sites'readiness for summative evaluation;federally led evaluation of at least one
comprehensive service model,including an implementation study and an impact study;and ongoing
dissemination of knowledge gained through YARH-3 Site activities.
Mathematica is the prime YARH-3 contractor to OPRE,Building Capacity to Evaluate Interventions for
Youth/Young Adults with Child Welfare Involvement at Risk of Homelessness described in Contract Number
HHSP2332015000351/75P00119F37041,including any modifications or extensions thereof(YARN-3).CPR
will serve as a YARH Site for Colorado under the Building Capacity to Evaluate Interventions for
Youth/Young Adults at Risk of Homelessness.
Overview of the Project
The objective of the YARH-3 project is to assist OPRE in building evidence for the comprehensive service
models developed and initially implemented in YARH-2 by supporting YARH Sites and conducting a
federally led summative evaluation.(YARH is now known as SYSIL-Supporting Youth Succeed in Life.)The
YARH Site will complete work related to building evidence for the comprehensive service model they
developed and initially implemented in YARH-2,with modifications as suggested by YARH-2 work,and/or

documentation of readiness for a summative evaluation.CPR expects ESWC to work with CPR liaisons
throughout the course of the term of the Agreement.ESWC will continue its YARH-3 work in January 2024.
CPR expects activities to end no later than December 31,2024,pursuant to this agreement and annual
renewal provisions.ESWC will work with CPR to complete the activities outlined in the Scope of Work
below in a timely manner where the parties agree to the following.
As a comparison site for the YARH-3 summative evaluation,ESWC commits that staff assigned to YARH-3
will perform the following activities:
Training and Technical Assistance(TA)Activities
• Attend YARH 3 events.
• Attend evaluation training for comparison sites.
Implementation Activities
• Operate ESWC Chafee program as it is currently designed and implemented(i.e.,'business as
usual')ESWC agrees that it will not substantially change,alter or adjust its'Chafee program during
the course of ESWC participation in the YARH-3 summative evaluation.Notify and discuss with CPR
any contemplated changes and refrain from any changes that would disrupt the YARH-3
summative evaluation per CPR's recommendations.
• Strive to enroll a minimum of 27 youth in year four of YARH-3 based on the Pathways screening tool.
Evaluation
• Participate in monthly CPR YARH-3 evaluation calls.
• Gain informed consent/assent for each youth to be enrolled in YARH-3.
• Enter quality data on a timely basis in Trails,SYSIL,and the Pathways Management Information
System(PMTS)to collect information not available through Trails and administrative sources.
• Participate in other YARH-3 evaluation activities as requested.
Project Management
• Attend YARH-3 meetings and events as needed.All attempts will be made to streamline Pathways'
meetings and the number of them.
Fees,Reimbursement,and Invoicing
CPR will compensate ESWC in an amount that will not exceed a$10,000 site payment.ESWC will invoice
CPR in accordance with Attachment A.
