RE: Notice of Intent to Award for Solicitation, RFP UHAA# 2023000170, Case
Management Agency (CMA) Activities and State General Fund Program Services.
Dear Offeror:
In accordance with the above -referenced solicitation, the evaluation committee
performed an evaluation of the quotations submitted in response to the solicitation. The
evaluation committee made a recommendation for award of a contract conforming to
the solicitation. I approve the evaluation committee's recommendation.
The State of Colorado Department of Health Care Policy and Financing (the
Department) intends to award a contract to:
Area 1: Cheyenne, Kit Carson, Lincoln, Logan, Morgan, Phillips, Sedgwick,
Washington, and Yuma Counties — Northeastern Colorado Association of Local
Governments (NECALG)
Area 2: Baca., Kiowa, and Prowers Counties — Prowers County Public Health and
Environment
Area 3: Bent, Crowley, and Otero Counties — Otero County
Area 4: Huerfano and Las Animas Counties — Las Animas County
Area 5: Arapahoe, Douglas, and Elbert Counties — Developmental Pathways (DP)
Area 6: Adams and Denver Counties — Rocky Mountain Human Services (RMHS)
Area 7: Clear Creek and Jefferson Counties — Jefferson County
Area 8: Boulder, Broomfield, and Gilpin Counties — Adult Care Management, Inc.
(ACMI)
Area 9: Weld County — Weld County
Area 10: Larimer County — Foothills Gateway, Inc.
Area 11: El Paso, Park, and Teller Counties — The Resource Exchange (TRE)

Area 12: Pueblo County — Unsuccessful solicitation process, the Department has
determined that it will not be possible to make an award and will make an award
pursuant to Competitive Negotiations pursuant to R-24-103-208-02.
Area 13: Chaffee, Custer, Fremont, and Lake Counties; Area 14: Alamosa, Conejos,
Costilla, Mineral, Rio Grande, and Saguache Counties; Area 16: Grand, Jackson,
Moffat, Rio Blanco, and Routt Counties; Area 17: Mesa County — Rocky Mountain
Health Maintenance Organization, Inc (RMHP).
Area 15: Eagle, Garfield, Pitkin, and Summit Counties — Garfield County
Area 18: Delta, Gunnison, and Hinsdale Counties — Unsuccessful solicitation process,
the Department has determined that it will not be possible to make an award and will
make an award pursuant to Competitive Negotiations pursuant to R-24-103-208-02.
Area 19: Montrose, Ouray, and San Miguel Counties — Montrose County
Area 20: Archuleta, Dolores, La Plata, Montezuma, and San Juan Counties —
Community Connections, Inc. (CCI).
Please be advised, the decision may be protested pursuant to Section 24-109-102, et
seq., C.R.S. Protests are due by Tuesday, June 27, 2023, at 3:00 pm (MT) and can be
submitted via email to: Nathan Weatherford, Esq., Procurement Director at
nathan.weatherford@state.co.us.
Offerors may obtain copies of quotation responses and evaluator worksheets through a
Colorado Open Records Act (CORA) request. Please email kathy.snow@state.co.us
with a request.
