RE: APPROVE MEMORANDUM OF UNDERSTANDING FOR DISTRIBUTION OF STATE
OF COLORADO BIRTH CERTIFICATES AND IDENTIFICATION (I.D.) VOUCHERS,
AND AUTHORIZE CHAIR TO SIGN - UNITED WAY OF WELD COUNTY
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Memorandum of Understanding for the
Distribution of State of Colorado Birth Certificates and Identification (I.D.) Vouchers between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services, and the United Way of Weld County,
commencing upon full execution of signatures, and ending June 30, 2026, with further terms and
conditions being as stated in said memorandum of understanding, and
WHEREAS, after review, the Board deems it advisable to approve said memorandum of
understanding, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Memorandum of Understanding for the Distribution of State of
Colorado Birth Certificates and Identification (I.D.) Vouchers between the County of Weld, State
of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of
the Department of Human Services, and the United Way of Weld County, be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said memorandum of understanding.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 11th day of October, A.D., 2023.

Brief description of the problem/issue : The Department is requesting to enter into an MOU with United Way
of Weld County for collaboration to expand access to critical State of Colorado issued identification documents
for the Distribution of State of Colorado Birth Ce�tificates, Identification Cards and Driver's Licenses . WCDHS
will be a sub-recipient of the Colorado Department of Public Health and Environment ( CDPHE) ID Project
Grant through Metro Caring and UINWC . UVWVC will provide vouchers and training to implement this
program .
What options exist for the Board ?
• Approval of the MOU between WCDHS and UWWC .
• Deny approval of the MOU between WCDHS and UWWC ,
Consequences : There would not be an MOU between WCDHS and UWWC for the distribution of State
of Colorado Birth Certificates , Identification Cards and Driver's Licenses .
Impacts : These services would not be available to Weld County clients .
Costs (Current Fiscal Year / Ongoing or Subsequent Fiscal Years) :
o Metro Caring of Oenve� and UWWC will provide Vouchers and training to WCDHS at no cost .
Recommendation :
• Approval of the MOU and authorize the Chair to sign .

Memorandum of Understanding
Distribution of State of Colorado Birth Certificate and ID Vouchers
This Memorandum of Understanding ( " MOU " ) is entered into upon the date of execution by signature
between Weld County Department of Human Services ( "Weld DHS" ), 315 North 11`h Avenue, Greeley,
Colorado, 80631, and United Way of Weld County ( UWWC), 814 9th Street, Greeley, Colorado, 80631 .
Background of Agreement . The following statements are provided to aid in the understanding and
interpretation of this Agreement .
A. UWWC annually receives a-number of vouchers from Metro Caring of Denver, Colorado, that bearers
may use to receive State of Colorado birth certificates, identification cards, and driver' s licenses at no
cost to the bearer.
B . Weld DHS works with clients that can benefit from access to these vouchers .
C. In order to more directly assist Weld DHS clients, UWWC will provide vouchers that Weld DHS can
issue to clients .
WHEREAS, Weld DHS has agreed to collaborate with UWWC to expand access to critical State of Colorado -
issued identification documents in Weld County .
NOW THEREFORE, UWWC and Weld DHS agree as follows :
1 . This MOU is in effect upon si�nature of the last signer through June 30, 2026, and is contingent upon
Metro Caring' s ongoing receipt of the Colorado Department of Public Health and Environment ( CDPHE )
Identification ( ID ) Project Grant award .
2 . Weld DHS agrees to be a sub- recipient of the CDPHE ID Project Grant at its location ( s ) and will submit
reports on the 5`h day of the month for vouchers distributed in the previous month .
a . If the S`h falls on a holiday/weekend , Weld DHS will submit the following business day.
b . Reports will include the voucher recipient' s full name, the type of voucher distributed , basic
demographic information, and the reason for needing the document requested .
c . UWWC agrees to provide all documents needed for reporting .
3 . UWWC will provide training to key Weld DHS staff on how to distribute ID vouchers to recipients as
well as how to report on voucher distributions .
4 . UWWC receives vouchers from Metro Caring and as such assumes responsibility for the costs of
creating and printing unique vouchers and required supplemental materials .
5 . Weld DHS will only distribute vouchers printed and provided by Metro Caring through UWWC for birth
certificates, identification cards and driver's licenses . As UWWC receives varying amounts of vouchers
from Metro Caring over time , UWWC will supply birth certificate vouchers and Department of Motor
Vehicles ( 0MV) vouchers on an as available basis . Quantities are based upon prior distribution
amounts and budget constraints . Allotments will increase or decrease by UWWC at its sole discretion
depending upon available amounts and redemption rates .
6 . Contingent upon performance and available funding, this MOU may be extended in writing by both
parties after June 30, 2026 .
