On June 5'h,2023,Governor Jared Polls signed SB 23-082 into law,known as C.R.S.§19-7-314.5,creating the Colorado Fostering Success Housing Voucher Program. This Program will provide stable housing for youth who are currently 18 but have not yet turned 26,were in foster care on or after their 14th birthday and are actively participating in a Chafes or Foster Youth Successful Transition to Adulthood grant program's case management services. If awarded,this Grant will support vulnerable young people in their transition to adulthood. Grant applications are due to the Colorado Office of Children,Youth&Families by December 29"',2023. What options exist for the Board? • Approval of the Fostering Success Voucher Program Grant Application. • Deny approval of the Fostering Success Voucher Program Grant Application. Consequences:There will not be an application for Grant Funding. Impacts:ESWC will not participate in the Fostering Success Voucher Program and youth in Weld County will not have access to this service. The PYD principles are provided below . • Strengths-based - Taking a holistic approach that focuses on the inherent strengths of an individual , family or community, then building upon them . • Inclusive - Addressing the needs of all youth by ensuring that our approach is culturally responsive . • Engaging youth as partners - Ensuring the intentional , meaningful , and sustained involvement of youth as equitable partners in the programs, practices, and policies that seek to impact them . • Collaborative - Creating meaningful partnerships within and across sectors to effectively align our work . • Sustainable - Addressing long- term planning through funding, training, capacity building , professional development, and evaluation in order to ensure ongoing support and engagement of youth . When completing each section please provide the requested information for the three separate identified populations : • Population #3 : 18 to 26 -year-olds ( Housing Voucher recipients ) . Identification , Outreach , and Waitlist : I . Describe in bulleted detail how the program will identify and engage youth eligible for the Colorado Fostering Success Voucher program services in each of the eligible populations . Population #3 : • Young adults are referred to Chafee by the Weld County Department of Human Services ( WCDHS ) Child Welfare ( CW) Division through Family Engagement Meetings ( FEM ) , Team Decision - Making Meetings (TDM ) , Crystal Reports, Permanency Round Tables and Adolescent Care Exceptions Reports . • Additionally, young adults are referred :o Chafee by the WCDHS CW Division , State Educational and Training Voucher ( EN ) list, Permanency Round Tables, or Kinship Caseworker. • Outreach process : When the referral is received by the Youth Life Skills Navigator/Chafee Worker, contact is made with youth and the assessment is completed with discussion regarding Chafee being a voluntary program . The Transition Plan is created with the youth , providing ongoing support and services focusing on the outcomes in the Transition Plan . • Youth who are already working with a Chafee Worker will be identified if they are at imminent risk of homelessness . This will also include youth who are no longer in care, to assess need for a housing voucher. • Youth who are also engaged with our Weld County RHY provider, who also meet the qualification of the Colorado Foster Success Voucher Program , will also be connected to a Chafee Worker to begin the process of applying for the program . • Youth who are transitioning out of care will be identified and if homelessness is a factor, the youth will be complete the DOLA tool and VI -SPDAT. • Youth who have chosen not to enroll in the stipend Youth in Transitions Program , that are at a high risk of homelessness, will also be identified for the voucher program . How will you coordinate with your local COC to ensure youth are served through the Colorado Fostering Success Voucher Program and linked to other services as they are reaching the term of the vouchers ( through their 26th birthday) ? • Weld County Chafee has a well-established relationship with our COC and will create a transition plan well ahead of a youth ' s 26th birthday . Weld County Chafee plans on a holistic approach that involves e mployment supports, career development to ensure long term housing security is obtained . These e lements in addition to connecting youth to our local COC, will help to ensure a successful transition from this program . Program Narrative : Describe in bulleted detail and short summary how the program will design and deliver individualized services to achieve the purposes below. ( . Safe and stable housing A . Describe how the program will strategically partner with relevant agencies, including the local county child welfare agency, to access the full continuum of financial housing resources for youth including Supervised Independent Living Placements ( SILP ) , Chafee , the Foster Youth in Transition Program , Foster Youth to Independence ( FYI ) , Family Unification Program ( FUP ) vouchers , and Colorado Fostering Success Vouchers . • Weld County Chafee strategically partners with the above agencies and resources to our fullest extent to unsure access for the youth we work with to housing . We will continue to work with these agencies in addition to our own DHS Housing Stability Team . • Chafee also collaborates with North Range Behavioral Health ( NRBH ) with FUP vouchers . Chafee continues to work with youth in workshops and in one-on -one settings to discuss important life skills they will need to be self-sufficient such as having enough savings for startup costs, utilizing their permanent connections, and utilizing community resources . • Weld County Chafee also works closely with our Child Welfare team to coordinate and enroll youth in the Foster Youth in Transition Program into Chafee and help support independent skills . • Weld County Chafee also coordinates with case workers to support successful implementation of SILPs . B . Describe the status of the county and Chafee partners with local runaway and homeless youth ( RHY) providers , and / or continuums of care (CoC ) . Include efforts underway, through those partnerships , to prevent youth from discharging from foster care to a homeless shelter facility or Chafee eligible who are being served through the homeless system . • Current efforts underway include a relationship with our RHY Provider, Marc Fortney . He has referred youth who could potentially be Chafee eligible . • Marc has created a space for our Weld County Chafee workers to provide information on Chafee program services and supports to staff. Describe resources available through Emergency Service Grants ( ESG ) , rapid re- housing , other HUD resources ( resources available through non - profits ) , and Colorado Fostering Success Vouchers . • Weld County Chafee intends to support youth with fees, deposits, assisting with finding housing prospects, incentives to landlords such as double deposits to help secure housing. Other supports Weld County Chafee intends to utilize are paid Work Experiences to support employment opportunities as well as utilize workforce programs for employment and training opportunities when appropriate . Weld County Chafee will continue to work with our internal Weld County DHS resource, Housing Stability Team , to continue to maximize all expertise in housing and resources through various partners . We will also continue to utilize the CSBG Block Grant when no other internal Employment Services of Weld County ( ESWC) resources are able to be accessed . D . Briefly describe barriers to securing safe and stable housing in the communities you serve and efforts your county is engaged in to address those barriers . • Barriers include affordable housing, lack of credit or a co-signer, and landlords or property management companies unwilling to work with the voucher . The Youth Life Skills Navigator/Chafee Worker address the barrier of affordable housing by continuing to outreach to the Housing Authority , collaborating closely with the FUP vouchers through United Way of Weld County ( UWWC) and NRBH , and communicating with community landlords to educate about the needs of Weld County youth . The Chafee staff work on educating youth on the importance of credit as well as safely building credit when they turn 18 . The Chafee staff will assist youth with communication with landlords to mediate issues as well as educate youth on using their permanent connections as references . By taking these steps the landlords are more willing to work with our program and our youth in the future . The Youth Life Skills Navigator/Chafee Worker collaborates with other county staff to investigate alternative housing options , including diligent searches . • Chafee provides several specific workshops on community resources, permanent connections , and rental/ housing workshops yearly to teach our youth the importance of community connections . Youth Life Skills Navigator/Chafee Worker ask the community members that are experts in these fields to teach these topics . For example , we have partnered with a local property management company who Page 6 of 10 COLORADO Office Office of Children, CDHS Youth & Families Division of Child Welfare accepts FUP vouchers to teach a workshop about things landlords are looking for on a rental applications and proper landlord/tenant communications . This helps build trust among our local landlords on our program as well as utilizing the FUP voucher as they see the youth have supports . An additional workshop provided includes safety . This involves an officer from the Sheriff' s Department teaching the various ways of remaining safe when the youth is living on their own and in general personal safety . E . Describe ways in which the program will collaborate with family , kin , mentors , supportive adults , housing authorities , or community partners to help young people secure safe and stable housing . • The Youth Life Skills Navigator/Chafee Worker will work with each youth and the youths supportive connections on an individual basis . Chafee will be involved with monthly staffing' s , TDM ' s FEM ' s , Permanency Round Tables and Youth Voice Permanency Round Tables and one -on -one support as needed . The Youth Life Skills Navigator/Chafee Worker will collaborate with case involved members of the team in coming up with creative and supportive plans for youth in their endeavor to secure safe and supportive housing F . Colorado Fostering Success Voucher Program ( only needs to be completed by programs applying for the housing vouchers and corresponding case management funding ) : ' 1. Is your program prepared to implement the requirements of this program ? ( Yes ) 2 . This will allow for goal setting and specified support for the youth by the Chafee Worker toward compliance with voucher requirements . All tools at our disposal will be utilized to motivate the youth toward compliance within the program ' s power. Budgeting and ongoing efforts to ensure rules of the housing voucher are being met will be an ongoing conversation with Chafee youth . 5 . This voucher program ends on their 26th birthday . What will the program do to identify that they have access to safe and stable housing and ongoing support after 26? Page 8 of 10 COLORADO Office of Children, CDHS Youth & Families Division of Child Welfare • By working with our Weld County DHS Housing Support Team to ensure all community resources are utilized if needed to support stable housing and ongoing support after 26 will be an important element when such an event for our youth takes place . Ensuring that any programs associated with employment and training are utilized to help with stabilization prior to their 26th birthday . This will also include working with the Northern Colorado Continuum of Care team as well . X . Training and Program Support A . Continued training on assisting youth who are transitioning to independence and supporting youth with housing and rent are important services Weld County Chafee strives at providing. The Weld County Life Skills Navigator/Chafee Worker continues to gain training in various Workforce programs to assist Chafee youth with a direction towards independence and the specifics of housing and the rules and guidelines specific to this program and vouchers . An example is an increase in knowledge of Registered Apprenticeships and On -the -Job Training opportunities . B .
