RE: APPROVE ACCEPTANCE OF PURCHASE ORDER FUNDS FOR HOUSING
NAVIGATOR PROGRAM AND AUTHORIZE DEPARTMENT OF HUMAN SERVICES
TO DISBURSE FUNDS
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Purchase Order for the Housing
Navigator Program between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, and the
Colorado Department of Human Services, commencing July 1, 2023, and ending June 30, 2024,
with further terms and conditions being as stated in said purchase order, and, and
WHEREAS, after review, the Board deems it advisable to approve said purchase order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Purchase Order for the Housing Navigator Program between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Human Services, and the Colorado Department of
Human Services, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Purchase Orders be, and hereby are,
accepted, and the Department of Human Services, be, and hereby, is authorized to disburse said
funds.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 5th day of July, A.D., 2023, nunc pro tunc July 1, 2023.

Housing Navigator Grant Program Purchase Order
Please review and indicate if you would like a work session prior to placing this item on the
Board's agenda.
Request Board Approval of the Department's Colorado Department of Human Services
(CDHS) Housing Navigator Grant Program Purchase Order. The Weld County Department
of Human Services (WCDHS) has received $120,000.00 in funds in the form of Purchase Order
IHBA 202400001232, from the Colorado Department of Human Services (CDHS). This funding
was a result of a Housing Navigator grant application and subsequent award, identified as Tyler
ID#2023-0825 and #2023-1134.
As outlined in the awarded grant application, this funding will establish an Intensive Housing
Navigator position within the WCDHS. In alignment with statewide priorities and to support
local community needs, this position will provide intensive case management to individuals and
families experiencing, or at risk of, homelessness who are receiving assistance from a WCDI-IS
program such as TANF and Old Age Pension, and to serve priority populations of WCDHS to
include seniors, individuals with disabilities and foster youth. This funding will support the
Housing Navigator and related services through June 30, 2024.
I do not recommend a Work Session. I recommend approval of this Purchase Order and
authorize the WCDHS to dispense the purchase order funds as appropriate.

0. PROJECT OVERVIEW
1. The Colorado Department of Human Services (the Department) is contracting with Weld
County Department of Human Services (the Contractor) to hire, train, and supervise
housing navigators, preferably with lived experience of housing insecurity who will
deliver housing navigation/housing stability services to those at risk of and currently
experiencing homelessness within Weld County. The title of this position in Weld County
will be Housing Stability Case Manager referred to as HSCM for the remainder of this
Statement of Work.
0. TERM OF PROJECT
1. Weld County Department of Human Services shall begin work upon acceptance of this
purchase order. All work pursuant to this purchase order shall be completed no later than
June 30, 2024.
0. GENERAL REQUIREMENTS
1. The Contractor shall perform all work in accordance with all applicable Federal and State
statutes, regulations, and rules, as now and hereafter amended, and the requirements
described within this Statement of Work and the purchase order.
2. The Contractor shall work closely and collaboratively with the Department, discuss
suggestions or issues as they occur and incorporate suggestions or guidance from the
Department while performing the work described within this Statement of Work and the
purchase order.
3. The Contractor shall coordinate and prioritize all work to ensure that all deliverables and
deadlines are met.
4. The Contractor shall employ an internal quality control process to ensure that all
deliverables are complete, accurate, easy to understand, and of high quality. The
Contractor shall provide deliverables that, at a minimum, are responsive to the specific
requirements, organized into a logical order, contain no spelling or grammatical errors,
formatted uniformly, and contain accurate information and correct calculations. The
Contractor shall retain all work papers generated for reference through the duration of the
project and project acceptance. The Contractor shall participate in the review and
revision process, until the Department provides written acceptance of the deliverable.
5. The Contractor shall provide copies of any supporting documentation to the Department
upon request of the Department and without charge.
6. The Contractor shall respond to all telephone calls, voicemails, and e-mail inquiries from
the Department within I business day.
7. The Contractor shall enable all Contractor staff to exchange documents and electronic
files with the Department staff in formats compatible with the Department's systems. The
Department currently uses Google Docs.
0. PROJECT REQUIREMENTS
1. The Weld County Department of Human Services (WCDHS) will hire and train a HSCM,
preferably with lived experience of housing insecurity. This Housing Navigator/Housing
Stability Case Manager will provide intensive case management to WCDHS Customers
experiencing homelessness.
1. WCDHS's training will include motivational interviewing, housing stability planning,
financial health classes, assistance with housing applications, and help finding
applicable resources.
2. WCDHS shall collaborate with the Department by providing a copy of the training
plan in writing and provide a hiring timeline.
3. DELIVERABLE: HSCM hire and training.
4. DUE: As outlined in Table 7.4.
2. Weld County Department of Human Services shall conduct outreach to 75% or more of
the county's unhoused cash assistance clients.
I. The outreach will include at least three (3) attempts at contacting an individual using
various methods to provide services.
2. WCDHS's outreach efforts will be documented for each household.

4. DUE: As outlined in Table 7.4.
3. Weld County Department of Human Services shall maintain an ongoing caseload of
20-30 individuals with whom the HSCM will develop housing plans and support an
individual from planning to application, to move in, and continue work with the family to
build natural supports and resources. The case managers will provide support and case
management during the housing search and after the family is rehoused.
1. The HSCM will engage with families/individuals to assess their unique situation and
offer referrals to relevant services. In addition, collaboratively create a Housing
Stability Plan, help with implementation of the client's plan, monitor,
document/review the household's progress, update a housing plan, and track the
outcomes of the families as they work towards housing stability.
2. Housing Stability Plans will be individualized, and specific to each household. The
plan will break down housing goals into attainable steps. The plans will be simple,
easy to read, and helpful. The plan is created by the household and case manager
collaboratively. The household will lead the process by establishing their housing
goals, and the HSCM will help follow through on the household goals.
3. Each Housing Plan will be documented, and progress will be tracked. The plan will
promote independence for the family, but aid with connections to resources, help
locating housing, troubleshooting with the household, and overall support. HSCM
will help create plans that build upon strengths and promote resiliency.
4. The HSCM will facilitate check -ins with households frequently (minimum once per
month) to review progress, readjust Housing Stability Plans, and keep informed of the
family's housing situation. HSCM will have consistent contact when first meeting a
household experiencing homelessness as the household's needs stabilize in housing
and resources contact will become less frequent based on client need.

4. The WCDHS aims to assist 10 to 15 households receiving Division of Economic, and
Workforce Support (DEWS) cash assistance clients are rehoused by the conclusion of the
fiscal year.
1. Once an individual is rehoused, their status in the CBMS system will be changed to
show that they are no longer unhoused. We will work collaboratively with our
Eligibility Department to reflect the information in the CBMS System.
2. The HSCM will check in with the household after they have entered housing.
3. At least $30,000 of this grant funding will be set aside to provide funding on behalf of
the customer to re-enter housing.

provide a quarterly Progress Report to the Department.

The total amount of funding for this fixed price purchase order for all work to be
performed pursuant to this Statement of Work is $120,000. This total amount of funding
will be the sole compensation to the Weld County Department of Human Services for the
services and/or deliverables provided.

0. HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT (HIPAA).
1. Federal law and regulations governing the privacy of certain health information requires a
business associate contract between the Department and the contractor. 45 C.F.R. §
164.504(E). Attached hereto and incorporated herein by reference and agreed to by the
parties is a HIPAA Business Associate Addendum for HIPAA compliance. Terms of the
Addendum shall be considered binding upon the execution of this contract and shall
remain in effect during the term of the contract including any extensions.
