Gentlemen:
The Town of Pierce, Colorado, requests this permit to construct and maintain
a ten-inch and eight-inch diameter sanitary sewer line along and across
county roads in accordance with the following description. These locations
were discussed previously with Mr. Glenn Billings.
The sewer line will cross County Road No. 35 one-fourth mile south of the
intersection of County Road No. 88 and No. 35, then proceed north along
County Road Nov 35, approximately six feet east of the west right-of-way
fence to this intersection where it will cross County Road No. 88. The
sewer line will continue west along the north side of County Road No. 88,
approximately 17 feet north of the centerline for a distance of one and one-
fourth miles to the intersection of County Road No. 88 and No. 33. The
sewer line will cross County Road No. 33 at this intersection and will
continue north, approximately 17 feet west of the centerline of County
Road No. 33 a distance of 1,200 feet, more or less, to the point where it
enters the U. S. Highway No. 85 Right-of-Way which is also the first street
in the Town of Pierce. All county road crossings described herein will be
constructed using an open cut procedure.
The Town will also construct an eight-inch sewer line along the centerline
of Third Street extended south of the Town of Pierce a distance of 750 feet,
more or less. All sewer lines to be constructed in accordance with this
permit are shown on the attached Location Map.

ground, or they will be marked with a post and sign as requested by

The Contractor will be J-Mark Construction Company, Littleton, Colorado. This
contractor is bonded and, under the contract with the Town of Pierce, must
perform all maintenance for faulty material and workmanship relative to the
proposed construction for a period of one year after completion of the
installation. It is also requested that the encasement of the carrier pipe
at County Road Crossings will not apply for this permit.

be used in the boring and no tunneling shall be permitted.

In the event any changes are made to this roadway in the future that
would necessitate removal for relocation of this installation, Permittee will
do so promptly at its own expense upon written request from Weld County, Colorado,

In accepting this Permit the undersigned, representing the Perm,ittee, verifies
that he has read and understands all of the foregoing provisions; that he has
authority to sign for and bind the Permittee; and that by virtue of his signature
the Permittee is bound by all the conditions set forth herein.

REQUIREMENTS FOR INSTALLATION OF UTILITIES WITHIN THE RICHT OF WAY
DEPARTMENT OF HIGHWAYS, WELD COUNTY, COLORADO
.. No cleated or track equipment to work on or move over asphalt surfaces without
mats.
2. All disturbed portions of right of way are to be returned to organic condition
at a time designated by Weld County, Colorado.
3. Any materials from excavation as the result of utility installation will be
removed from highway surface EACH DAY.
4. Under no condition is an asphalt surface to be cut unless otherwise specified
conditions of this permit.
`i. Mud-jacking will be required where bores are larger that lines or encasements.
6. The ends of bored sections ARE NOT to be covered before being inspected.
7. No plastic line is to be installed without encasement under pavement .egarc..eaa
of pressure and size.

8. Encasement of line will be required from toe of slope to tow of slope under
all highways and paved county road intersections and approaches, if lines are r2
ovor 2" diamotor or more than 100 psi procsuro.

9. Manholes and other points of access to other underground utilities may be

10. A'll requests for installation will be reviewed in the field by representatives
of Weld County, Colorado prior to any issuance of any permit.
11. Where special cases warrant the Department will permit the installation of '
underground utilities paralleling highways along the slope of the shoulder.
• In the event such permits are issued, it will be required by the contractor
that material removed must be replaced in like kind and with same or better
compaction. No segregration of material will be permitted.
12. The construction of underground installations will generally require the
services of an inspector furnished by Weld County, Colorado. The decision
in regard to which project requires inspection is reserved by Weld County, Colorado,
All costs relating to the inspection of such utility installation will be
borne by the permittee.
13. All drainage facilities or siphons will be opened immediately upon completion
•
of work at sites of drainage or siphon installation. Damaged portion of •
drainage or siphon facility will be replaced. All damaged sections to be
inspected by Weld County, Colorado, inspector before bein concealed in any •
manner. Drainage ditches or borrow pits to be restored to originalrcondition
, immediately after backfilling is completed.
14. No meters are to be placed within the highway right of way, excepting within
corporate limits where municipal regulation allow installation. •

NOTE TO APPLICANT: This permit shall be made available at the site where and
when work is being done.

SPE...,,L PROVISIONS: •
General :
C en Cuts across the driving surface of the roadway may be permitted •
at the discretion of Weld County, Colorado and only after the following
provisions are met.
1. No open trench& or hazardous ruts will be left over night
on or weekends.. Excavations will be securely tamped to
prevent hazards developing before excavation is resumed.
2. Final back fill and tamping operations will be executed in
no more than o" mechanically tamped lifts and the final 1"
will be a dry select road base material compacted to 90%
of original.
3. Site inspections by Weld County, Colorado, will determine
whether excavated material is suitable for backfill. If it
is determined unsuitable the material will be removed from
the site at the expense of the permittee.
• 4. Permittee assumes responsibility for the maintenance of all
open cuts for a period of one (1) year after construction
whether in a gravelled surface or a paved roadway. If said
cut is in need of repairs permittee is allowed seven (7)
days 'after notification for the execution of said repairs. •
5. • If the permittee has not repaired said open cut or cuts •
with the allowed seven (7) days, Weld County, Colorado, will
repair said cut and will bill permittee for all expenses . •

6. No open cuts within paved roadway will be allowed without scoring
pavement parallel to the center line of the utility and perpendicular
• to the center line of the road with a minimum width of 12".

7. All water lines and sewer lines are to be suitably encased to
allow for repair and or maintenance without further distributing
roadway. -
•
8. No puddling will be allowed.
• 9. That in all cases where existing black top or hard surfaced roads
must be cut, then in such case a non-cancellable cash or surety
bond payable to the County shall be required in the name of the
contractor or the person causing the hard surfaced roadway to be
• disturbed prior to the commencement of the work, said bond shall
assure recovery by the County of any expense incurred, within a period
of one year following the comole-c: .;:d acceptance of the restored

• portion or portions of the hard surfaced roadway. Said recovery to
be in the amount necessary to repair or replace said diistrubed
• portion of the hard surfaced roadway. Said bond shall not be cancelled
• - without thirty days notice to the County. The amount of said bond
shall be set by the Board of County Commissioners and shall be
reasonable; however, in no event shall the bond be less than $100.00.
