Approve Local Public Health Agency (LPHA) Mini -Grant Opportunity Application
for Injury and Violence Prevention and Authorize Department of Public Health and
Environment to Submit Electronically
Whereas, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
Whereas, the Board has been presented with a Local Public Health Agency (LPHA)
Mini -Grant Opportunity Application for Injury and Violence Prevention from the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Public Health and Environment, to the Colorado
Department of Public Health and Environment, Office of Gun Violence Prevention and
Core State Injury Prevention, with further terms and conditions being as stated in said
application, and
Whereas, after review, the Board deems it advisable to approve said application, a copy
of which is attached hereto and incorporated herein by reference.
Now, therefore, be it resolved by the Board of County Commissioners of Weld County,
Colorado, that the Local Public Health Agency (LPHA) Mini -Grant Opportunity Application
for Injury and Violence Prevention from the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department
of Public Health and Environment, to the Colorado Department of Public Health and
Environment, Office of Gun Violence Prevention and Core State Injury Prevention, be,
and hereby is, approved.
Be it further resolved by the Board that Cathy Hoover, Department of Public Health and
Environment, be, and hereby is, authorized to electronically submit said application.

Brief description of the problem/issue:
The Health Education, Communication, and Planning Division seeks approval to apply for $9,500 from CDPHE's
State Injury Prevention Program. This division provides oversight to the Child Fatality Review team which
routinely reviews and provides prevention suggestions on deaths related to unrestrained children in vehicles,
and unsafe sleeping environments. If successful, HECP would utilize funds to purchase car seats/boosters to be
made available to community partners, as well as pay for printing of already established car seat and safe sleep
campaign materials.
What options exist for the Board?
1. Authorize WCDPHE to submit the grant application
2. Do not authorize the grant application
Consequences: Current child fatality review funding only covers the cost to convene the review
meetings and .10 FTE for data gathering and entry. Each meeting injury prevention suggestions are made as
part of the review process, but a lack of funding restricts implementation of preventative measures. This grant
funding would allow the team to act on a few of the prevention measures deemed necessary for Weld County.
Impacts: With this award, HECP will utilize current relationships such as United Way, DHS, Nurse Family
Partnerships, Salud Clinics etc., to disseminate not only car seats, but also educational materials tied to safe
sleep and motor vehicle safety.
Costs (Current Fiscal Year / Ongoing or Subsequent Fiscal Years):
No county funding is required as this grant will cover all costs of materials and printing. This is one time funding,
and all funds must be expensed by July 31, 2025.
Recommendation: I recommend the approval to place this grant application on the future BOCC agenda for
formal consideration.

What is your project description, including focus population?*
Weld County Department of Public Health and Environment (WCDPHE) would like to request $9,500
to purchase and distribute car seats and boosters, as well as print educational matenals not only on
car safety for youth, but also safe sleep These items will be made available, with extra emphasis on
partners that serve young/single moms, low- SES, and immigrant families
How was the need for this project determined?*
WCDPHE manages the county Child Fatality Review teams The deaths we review typically involve a
child unrestrained or poorly restrained inside of a vehicle, or who has passed due to unsafe sleeping
conditions
Further, from 2020 through 2023, the age -adjusted traffic -related motor vehicle death rate in
Weld County was 3 2 per 100,000 residents, more than double the statewide rate of 1 4 per
100,000 This is specific to vehicle occupants, and excludes pedestrian, motorcyclist, and
bicyclist deaths' According to the Colorado Department of Transportation's Fatal Crash Data, 5
out of the 18 Weld County residents under age 18 who died in traffic accidents in which they
were vehicle occupants were not restrained at the time 2

How will you ensure that funding will be spent by deadline?*
We have selected strategies that are not new to Weld County, and are relatively easy to solicit,
disseminate, and wrap up within the short time frame. We will utilize our existing networks to partner
on the car seats and boosters. This was done recently through United Way of Weld County whom we
have a strong relationship with. We have identified partners such as our mobile clinic, health clinics,
in -home nurses, and department of human services to disseminate educational materials.

2025 Injury and Violence Prevention Mini -grants Initiative
CDPHE the Office of Gun Violence Prevention and the general injury teams are partnering to invite
applications for the 2025 Injury and Violence Prevention Mini -grant Initiative for LPHAs This funding
opportunity aims to support communities to implement time -limited activities that address either of the
following 1) Unintentional injury with a priority on motor vehicle safety, traumatic brain injury (TBI), or
Adverse Childhood Experiences (ACEs), and 2) Gun violence prevention
Goals The overarching goals of this mini -grant opportunity are to reduce morbidity and mortality due to
unintentional inlunes and gun violence, as well as to promote unintentional injury and gun violence
prevention
Funding Criteria
• Total funding available $180k Grants available between $5-10k
• Reimbursement of costs incurred
• Awarded LPHAs must incur the costs by June 30th, 2025 (for projects focused on gun violence
prevention) and July 31st, 2025 (for projects focused on unintentional injuries) and submit
invoices for reimbursement within 45 days of project end
No agency can receive more than $10,000 Multiple awards are possible, but each must be small
enough to total no more than $10,000
Project Requirements Must be specific to unintentional injury or firearm violence primary or secondary
prevention, interruption, or harm reduction activities Priority on evidence -based projects/outputs
Project Examples
• Event sponsorship or implementation
• Training/education
• Community partnership strategic planning process — must focus on planning for unintentional
injury or firearm violence work with collaborators
Implementation of planned activities focused on reducing firearm violence and/or unintentional
injuries in communities Ideally, implementation would be a collaborative process that involves
partners Examples include outreach and education, purchase of safety material to distribute,
environmental changes, etc
Development or expansion of partnerships/coalitions focused on gun violence and/or
unintentional injuries — must be willing to sustain without these funds (as this is short-term
funding)
Evaluation of new or ongoing work focused on preventing unintentional injuries
Reporting to CDPHE on data -to -action success using injury data where you report on the injury
data results that you used and the resulting activity you implemented or plan to implement
Staff and partner capacity building (including professional development and continuing
education) specific to unintentional injury prevention
Youth development programming, can only fund a portion, must be able to sustain remainder
Awardees will be required to submit final deliverables upon the completion of the project and no later
than June 30, 2025 for Office of Gun Violence initiatives and July 31, 2025 for Core Injury Prevention
projects.

The application will consist of a short Google Form where you will be asked for the following
information:

a. What is your project description, including focus population?
b. How was the need for this project determined?
c. How will you ensure that funding will be spent by deadline?
d. Will your project be complete by the deadline or is it an ongoing project? If it is ongoing,
please describe how you will sustain the work.
e. Please list any partners involved.
f. Please share the outcomes that your project would contribute to
g. Please share any relevant deliverables that you will share at the conclusion of the
project to demonstrate that you used the funding as proposed.
2. Agree to the following terms and conditions:
a. Acknowledgment of spending deadline and that if funds are not used by the deadline,
they will be forfeited
b. Agree to the cost reimbursement model which means submitting invoices for expenses
incurred
c. Confirm that the funds will not be duplicative or supplanting other funds
3. Attach a budget
The Application form will open for application submission on April 7, 2025, and applications will be
reviewed as they come in until funding runs out. No applications will be accepted after May 15, 2025. If
all funding is awarded prior to this date, the application will be closed.
