Approve Amendment #3 to Facility Usage License Agreement and Rental
Guidelines for Area Agency on Aging (AAA) "Friendly Fork" Senior Nutrition
Program, and Authorize Chair to Sign — Archdiocese of Denver and Guardian
Angels Catholic Church
Whereas, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
Whereas, the Board has been presented with Amendment #3 to the Facility Usage
License Agreement and Rental Guidelines for the Agency on Aging (AAA) "Friendly Fork"
Senior Nutrition Program between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, and Archdiocese of Denver and Guardian Angels Catholic Church,
commencing upon full execution of signatures, and ending August 30, 2026, with further
terms and conditions being as stated in said amendment, and
Whereas, after review, the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
Now, therefore, be it resolved by the Board of County Commissioners of Weld County,
Colorado, that Amendment #3 to the Facility Usage License Agreement and Rental
Guidelines for the Area Agency on Aging (AAA) "Friendly Fork" Senior Nutrition Program
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, and
Archdiocese of Denver and Guardian Angels Catholic Church, be, and hereby is,
approved.
Be it further resolved by the Board that the Chair be, and hereby is, authorized to sign
said amendment.
The Board of County Commissioners of Weld County, Colorado, approved the above
and foregoing Resolution, on motion duly made and seconded, by the following vote on
the 24th day of September, A.D., 2025:

PASS -AROUND TITLE: Facility Usage License Agreement Addendum #3 with the Archdiocese of
Denver/Guardian Angels Catholic Church.

Brief description of the problem/issue: The Department's Home and Community Support Area
Agency on Aging Division provides a senior lunch program called Friendly Fork. Through this program
seniors may receive free lunches at various locations throughout Weld County. The Department
entered into a Facility Usage License Agreement with the Archdiocese of Denver/Guardian Angels
Catholic Church known to the Board as Tyler ID# 2022-2505. Since the original agreement was
approved, the following has occurred:
• Addendum #1 was approved on September 11, 2023 and is known to the Board as Tyler ID#
2023-2614, which updated the cost per week and holiday schedule, and extended the terms to
August 31, 2023 through August 30, 2024.
• Addendum #2 was approved on August 28, 2024 and is known to the Board as Tyler ID# 2024-
2276, which updated the holiday schedule and extended the terms to August 31, 2024 through
August 30, 2025.
The Department is requesting to enter into Addendum #3, which updates the holiday schedule and
extends the terms to August 31, 2025 through August 30, 2026.
The Addendum has been reviewed and approved by Legal (B. Howell).
What options exist for the Board?
Approval of the Facility Usage License Agreement Addendum #3 with the Archdiocese of
Denver/Guardian Angels Catholic Church.
Deny approval of the Facility Usage License Agreement Addendum #3 with the Archdiocese of
Denver/Guardian Angels Catholic Church.
Consequences: There will be no Friendly Fork meal site agreement with the Archdiocese of
Denver/Guardian Angels Catholic Church.
Impacts: Seniors participating in the Friendly Fork Program in Mead, Colorado will not have a
meal site to receive nutritious meals on Thursdays.
Costs (Current Fiscal Year / Ongoing or Subsequent Fiscal Years):
Total cost = $50.00 per week for 47 weeks ($2,350.00 total)

ADDENDUM to Rental Agreement from Aug. 31, 2022, known to the Board as Tyler IDU 2022-2505 between
Guardian Angels Catholic Church and Weld County Department of Human Services, AAA Friendly Fork Program.
The undersigned parties agree to a weekly rental fee of $50 per meeting/week. All prior arrangements and contract
agreement remain in place.
These are currently the dates where the Senior luncheons don't take place at the Catechetical Center of Guardian
Angels for 2025/26:
Nov. 27, 2025 (Thanksgiving)
Dec. 25, 2025 (Holiday)
Jan. 1, 2026 (Holiday)
A week in June or July 2025 for Vacation Bible School
1 credit for canceled meeting on Sept. 4, 2O25 (making it 47 weekly meetings)
Term The term of this Agreement shall be from Aug. 31, 2025 through Aug. 30. 2026. This Agreement may be
extended annually upon written agreement of both parties.
