This grant opportunity is a program of Easterseals Colorado, which helps provide family caregivers with a break from caregiving. The intent of these funds is to increase access to respite care options for family caregivers and to help build the capacity to provide additional services. The Department is now requesting to apply for the 2025-2026 Colorado Respite Coalition (CRC) State Respite Grant. If approved, the Department will be requesting $30,000.00 in grant funds to continue to offer respite options for Weld County caregivers. The grant period will be from July 1, 2025 to June 1, 2026. What options exist for the Board? Approval to submit an application for the 2025-2026 CRC State Respite Grant. Deny approval to submit an application for the 2025-2026 CRC State Respite Grant. Consequences: The Department of Human Services' Area Agency on Aging will not be able to apply for the grant funding. Impacts: The Department of Human Services' Area Agency on Aging will not have additional funding to serve families in need of respite services. Respite care provides family caregivers with a break from caregiving. Respite care can be provided in a home, at a center, or in the community. The key is that the caregiver must have the opportunity to leave, take time for themselves, and know their loved one is being cared for This grant program supports services across the lifespan and health and disability spectrums and serves the entire state of Colorado. This grant program is funded through Colorado State General Funds and is managed by CRC and the Colorado Department of Human Services, State Unit of Aging. The intent of these funds is to increase access to respite care options for family caregivers and to help agencies build the capacity to provide additional services. All funds are specified to be used to support families as a Payer of Last Resort (i.e., families who do not actively receive respite allocations through Medicaid waivers or other funds). Grant applicants must define how your agency will determine the populations most in need within its application. Special Note: CRC also offers a separate Family Respite Voucher Program, where families can apply directly for financial assistance for respite services. Please contact us for further information. Agencies can participate in both the voucher and grant programs but cannot serve the same families simultaneously using both funding sources. To qualify, the organization must: ❑ Be based in and provide services in the state of Colorado, to Colorado families. ❑ Be currently providing respite services including but not limited to adult day service, out -of -home, or in -home respite services to families of children or adults with special needs, foster families, and/or kinship caregivers. New programs are not eligible for funding. ❑ Comply with C.R.S. § 18-6.5-107 which requires fingerprint -based background checks to be completed on all providers (staff and volunteers). All background checks must be completed and reviewed by the grantee before services can be provided. ❑ Comply with C.R.S. § 26-3.1-111, which requires certain employers to obtain a CAPS check to determine if a person who will provide direct care to an at -risk adult is substantiated in a case of mistreatment of an at -risk adult. ❑ Carry General Liability, Automobile Liability, and Protected Health Information Insurance policies or add policy during the grant period. Award Amount & Term The organization/agency will be awarded funds to support families in need of respite care. The organization/agency is not required to provide a match or in -kind donations, but it is encouraged to leverage these funds to secure additional funding, when possible. • The anticipated award range is $10,000 - $30,000. • Grant Term is July 1, 2025— June 1, 2026. Services must be provided by June 1, 2026. 2 A program of easterseals `�_colrxado COLORADO RESPITE COALITION Scoring: Applications will be scored based on the point values listed within the application narrative, including how well agencies answer questions in the narrative, completion of the application, and adherence to grant program parameters. The grant review committee is selected by a statewide coalition of entities that focus on the needs of caregivers of at -risk persons. Grant Reporting: Grantees will execute a contract with Easterseals Colorado and must meet the outcomes determined in the grantee proposal. Grantees must evaluate determined outcomes, through surveys or other methods, and report results to Easterseals Colorado. Grantees will complete a data collection form outlining hours of service provided to each family and administer the Easterseals Colorado caregiver survey to each family served. These items will be sent with the grant contract upon award notification. All reporting will be due on June 9th, 2026. Payment: IMPORTANT: This grant is given on a reimbursement basis only. Monthly billing is required as of July 1, 2025. We will also accept quarterly billing in certain circumstances. Weld County AAA sits within the Home and Community Supports Division of the Weld County Department of Human Services. Our primary responsibility is to serve Weld County residents aged 60 and older through a wide range of programs that promote safety, stability, and quality of life. As part of this Division, we work closely with partner entities including the Case Management Agency, Long Term Care Eligibility Technicians, Aging and Disability Resources for Colorado (ADRC), and Adult Protective Services to ensure a coordinated, person - centered approach to aging services. One of our key priorities is supporting family caregivers, who are essential to helping older adults remain safely at home. Through collaborative partnerships and community engagement, we deliver services that ease caregiver burden and reduce the risk of burnout. As one of 16 Area Agencies on Aging in Colorado —and a proud member of the national aging services network —we are committed to enhancing the lives of older adults and their caregivers. These respite services provide unpaid caregivers with critical time to rest and tend to their own well-being, ultimately helping to sustain caregiving relationships and delay or prevent institutional care for their loved ones. Question 2: The Weld County AAA Caregiver Respite Program has been serving the community since the 1990s. Our staff do not directly provide respite services; instead, we operate a consumer -directed voucher program that gives caregivers the flexibility to access care that fits their individual circumstances. Caregivers may receive a voucher every six months during the fiscal year, and each voucher can be used as needed — whether on a recurring basis (such as weekly in -home care), for adult day programs, or during times of crisis, travel, or caregiver fatigue. All voucher coordinators within the program hold at least a bachelor's degree and work closely with caregivers to ensure services are accessible and responsive. The autonomy and flexibility of this model make it highly effective in addressing the diverse needs of caregiving families. In Fiscal Year 2025, the total budget for the Caregiver Respite Program was $155,000. Of that amount, $10,000 was generously provided by Colorado Respite Coalition Grant, while the remaining funds came from a combination of federal and state dollars allocated through the Older Americans Act. Question 3: The funding allocated to the Weld County AAA will be used to supplement existing federal and state dollars currently supporting the Caregiver Respite Program. However, we anticipate a reduction of approximately 30% since we can no longer have the ability to supplement our budget with ARPA dollars, which will significantly impact our ability to maintain current service levels. This reduction will present challenges in serving our existing caseload of caregivers. To address this, our caregiver coordinator will prioritize individuals with the greatest need by utilizing the Caregiver Prioritization Worksheet. This tool helps identify caregivers who lack other sources of respite support and ensures that limited funds are directed to those in the most vulnerable situations. The program's consumer -directed model allows caregivers the flexibility to select the provider, setting, and schedule that best align with their needs —including during public health emergencies (PHE), when adaptable and personalized support is especially critical. All individuals receiving respite services —whether through Older Americans Act funding or supplemental grant dollars —will be closely monitored in accordance with State Unit on Aging (SUA) policies and procedures. When funding allows, caregivers on the waitlist will be considered for services based on prioritization criteria. The Caregiver Prioritization Worksheet, recognized as a best practice by the SUA in 2024, accounts for key factors such as rural location, minority status, physical limitations, and low- income status to guide equitable access to services. In the event of a public health emergency, outreach and communication efforts with caregivers will be enhanced to ensure their ongoing safety and support. The consumer -directed nature of the program ensures caregivers retain autonomy and flexibility in choosing how, when, and by whom respite services are delivered. Question 4: The primary objective of this grant is to sustain our current caregiver respite services in Weld County despite significant anticipated funding reductions. With a projected 30% decrease in funding —from $155,000 in Fiscal Year 2025 to $106,000 in Fiscal Year 2026 —the Caregiver Respite Program faces the potential need to reduce the amount of support provided to current participants. The requested grant funding is critical to maintaining existing voucher levels and continuing service to those already enrolled in the program. If the grant is fully funded, we anticipate being able to provide an additional 900 hours of respite care to caregivers in Weld County. This added capacity will help alleviate the burden on families and reduce the number of individuals on our unmet needs report and waitlist. Program effectiveness will be evaluated through several key methods. First, we will use our internal database to monitor the number of individuals served and the hours of respite delivered. We will also track changes in our waitlist and unmet needs data to assess the reach and responsiveness of the program. Additionally, all recipients will be encouraged to complete a Performance Measures Survey, distributed annually in February. This survey helps us gather critical feedback on program impact and identify opportunities for improvement. Our goal is for at least 90% of caregivers to report feeling more supported and less stressed because of receiving respite services. We also aim for 90% of participants to rate the quality of the program as "good" or "excellent." These outcomes will demonstrate the essential role respite care plays in supporting caregiver well-being and sustaining older adults in their homes and communities. Question 5: Weld County AAA maintains strong and enduring partnerships with a wide range of formal and informal providers throughout Weld County, and we are deeply committed to sustaining and strengthening these relationships. We actively participate in numerous community outreach initiatives, including regular involvement in resource fairs and public events. These opportunities allow us to share information about AAA services and foster collaboration with local stakeholders. Our outreach spans a variety of venues such as senior and recreation centers, churches, VA facilities, libraries, health care organizations, and home care agencies. These connections play a vital role in extending the reach of our programs and ensuring that community members are aware of available resources. We also rely on the support of community volunteers who enhance the delivery of our programs. The caregiver/respite program is consistently promoted by our Information and Assistance team and through our established network of community partners. To remain responsive to community needs, we actively gather feedback through multiple channels — including email, phone, comment cards at service sites, and our annual satisfaction survey. In addition, we are guided by an active Advisory Board that provides valuable insights, raises concerns, and offers recommendations for continuous program improvement. Question 6: If awarded, none of the grant funds will be used for salaries, benefits, or indirect costs. In the event of a public health emergency, communication with individuals receiving respite care will be intensified to ensure their needs are being met. Services will continue as the program remains consumer -directed, ensuring flexibility and autonomy for caregivers in selecting and utilizing respite services. Question 4: Our primary objective for the grant is to enhance and broaden respite options for caregivers in Weld County amidst impending budget cuts. Anticipating a 40% reduction in funding from Fiscal Year 24 to Fiscal Year 25, with the caregiver/respite program facing a budget decrease from $145,000 to $87,000, the grant award's fundamental aim is to mitigate the adverse impact on recipients and prevent potential cuts to voucher amounts. The organization will assess outcomes by monitoring the delivery of services to individuals requiring respite care throughout the county using our internal database. Simultaneously, we will track the reduction in our waitlist and the number of individuals on our unmet needs report. If the grant is fully funded, we anticipate being able to allocate an additional 900 hours of respite care to families in Weld County. Additionally, all recipients will be encouraged to participate in a Performance Measure survey during FY 25 to evaluate their overall satisfaction with the program and identify areas for improvement. Our target is to achieve a satisfaction rate of 90% or higher among program participants. Question 5: Weld County AAA maintains long-standing formal and informal partnerships with numerous providers across Weld County, and we are committed to sustaining these collaborations. We actively engage in multiple community outreach efforts, including participation in various resource fairs held throughout the year. These events serve as platforms to inform the public about the services offered by AAA and facilitate collaboration with local stakeholders. Examples of venues where we participate include local senior/recreation centers, churches, VA facilities, libraries, health care facilities, and home care agencies. Our agency relies on the support of multiple community volunteers to bolster our diverse programs. We will continue to promote the caregiver/respite program through our Information and Assistance team and leverage our connections with community partners. Additionally, we gather feedback from the community through various channels, including email, phone, comment cards at programming sites, and our annual satisfaction survey. Moreover, we maintain an active advisory board that provides valuable input, addresses concerns, and offers suggestions to enhance our programs. Question 6: Should the additional funds be granted, note that none of the grant funds will be allocated towards salaries, benefits or indirect costs. All funds will be strictly dedicated to supporting caregiver recipients' The allocation for 'other expenses' serves as an approximate representation of this program's specific contribution to the overall expenses for the Older American Act programs. Question 7: Despite being designated as urban according to the Colorado Rural Health Center, Weld County ranks as the third largest county in Colorado, spanning 3,987.24 square miles. This expansive geographical area poses significant challenges in accessing the formalized services required by our senior population.
