HomeMy WebLinkAbout20250606.tiffResolution Approve Memorandum of Understanding for Pilot Program Concerning Placement Families for Children and Youth with High Acuity Needs, and Authorize Chair to Sign — Raise the Future Whereas, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and Whereas, the Board has been presented with a Memorandum of Understanding for a Pilot Program Concerning Placement Families for Children and Youth with High Acuity Needs between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and Raise the Future, commencing March 1, 2025, and ending June 30, 2025, with further terms and conditions being as stated in said memorandum of understanding, and Whereas, after review, the Board deems it advisable to approve said memorandum of understanding, a copy of which is attached hereto and incorporated herein by reference. Now, therefore, be it resolved by the Board of County Commissioners of Weld County, Colorado, that the Memorandum of Understanding for a Pilot Program Concerning Placement Families for Children and Youth with High Acuity Needs between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and Raise the Future, be, and hereby is, approved. Be it further resolved by the Board that the Chair be, and hereby is, authorized to sign said memorandum of understanding. Raise the Future wil offer current implemented pilot processes that include referral processes, service delivery, and population descriptions as well as administer at least one Trust -Based Relational Intervention (TBRI) series for caregivers. The term of this MOU is March 1, 2025 through June 30, 2025, and may be extended if the project continues beyond the initial agreement. What options exist for the Board? • Approval of the MOU with Raise the Future. • Deny approval of the MOU with Raise the Future. Consequences: The Department will not be able to participate in the Raise the Future pilot program. Impacts: Weld County DHS clients will not receive the benefits of the services from Raise the Future. Ross 37'1 ro Pass -Around Memorandum; February 25.2025 — CMS ID 9134 4 RAISE the FUTURE 1325 S Colorado Blvd I Suite B700 Denver, CO 80222 MEMORANDUM OF UNDERSTANDING The following is a Memorandum of Understanding (MOU) between Raise the Future (Raise) and Weld County Department of Human Services (County). The County has agreed to participate in a project to pilot a program to target placement families for children and youth with high acuity needs. Please see below for more details about the pilot program. The project is funded through Colorado Department of Human Services/ Division of Child Welfare (DCW). The MOU is in effect from March 1, 2025 - June 30, 2025. The MOU can be extended if the project continues beyond the initial agreement. Administration 1. The project leadership team will include staff from Raise (VP of Programs, Family Support Director and others as deemed necessary); staff from DCW (Placement Resource Administrator, and others as deemed necessary); and senior staff from County. 2. Raise and DCW will meet at least monthly with the pilot County leadership to design, implement and review project interventions and to make recommendations for modifications throughout the pilot program. 3. County will provide consultation to other counties selected to participate in project expansion. 4. County will work with Raise to provide a quarterly report to DCW to include overall project progress and aggregate client data to include: a. Child and caregiver demographics, b. Types of service provided, and number of clients served, c. Frequency of services, number of hours including direct and indirect, (location; and in person, virtual etc.) d. Participants goals and progress. 5. DCW will track macro level outcomes and will keep Raise and County informed. Connect Youth. Support Families. The population will be focused on High Acuity Youth who meet the criteria identified in the High Acuity Referral checklist (see Appendix A). Raise will offer the current implemented plot processes, including the referral process, service delivery, and population descriptions. County will communicate any concerns with the referral process, service delivery and population descriptions. 2. Raise will Deliver at least one Trust -Based Relational Intervention (TBRI®) series for caregivers during the pilot phase. • Administer the pre -post survey to TBRI Series Participants i. Demographics collected by Individuals. ii. Identify relationship of the caregiver to the child/youth. iii. Scale knowledge level. 3. Raise will provide in -home coaching/support to four to six (4-6) caregiver families referred by the pilot County during the pilot period. • Utilize standardized assessments to set family -centered goals for in - home coaching. i. Demographics are collected by individuals. ii. Relationships of the caregiver to the child/youth. iii. Gather family -based information at the beginning of the intervention. iv. Utilize tools and observation to set goals. v. Reassess success and goal achievement after intervention. 4. Raise will meet with identified caregiver(s) in preparation for youth being placed in the home to assess caregivers needs. 5. Raise, County case team, caregiver and youth (if possible and appropriate) will meet twice at minimum to develop a step-down plan and a family regulation plan, prior to delivering in- home services. Meetings take place in - person or virtually depending on the circumstances. Plan to include: • Crisis resources • Identified support network • Identified needs for family member Connect Youth. When possible, Raise and designated County case worker, will meet with youth and current caregiver/placement at least twice to engage in step- down planning and develop a regulation plan for the transition process. 7. Raise and designated County caseworker will enroll caregiver in TBRI® training prior to youth placed in the home (or returned home). 8. Raise and designated County caseworker will proactively create a support plan (Front -load support) and coaching upon youth placement (or return home). The support plan will be retained in the County casefile and with the Raise case file. 9. Raise will regularly assess additional needs and provide referrals and resource recommendations to the family and to the casework team. 10. Raise will work with County to assess referral and communication processes and improve as necessary, Continuous Quality Improvement (CQI). 11. County and Raise will collect metrics and outcome data. 12. Raise will provide appropriate staffing to deliver the intervention. Each TBRI® series will be delivered by a qualified practitioner. In -home coach high acuity caseload will not exceed six (6) families at a time. TBRI® practitioners will receive regular coaching and case consultation by a qualified supervisor. The County will not receive funding for participating in the pilot or through the implementation. Nor will the County be charged for the services that are delivered in the context of this MOU. Have significant behavioral or emotional challenges that prevent a community -based or residential placement from keeping them safe; and/or 2. Have multiple failed/disrupted placements; and/or 3. Have a history of and/or current dysregulation episodes requiring hospitalization; and/or 4. Have a history where placements have been difficult to locate/secure; and/or 5. The caseworker/team has reason to believe the youth will be difficult to place due to their history, current situation, and/or the youth's caregiver requiring a high level of skills. 6. At an age that may exclude from professional support within the next 5 years ❑ They have a discharge plan that includes return to familial setting o They have an identified caregiver of support o The caregiver is willing to utilize new tools with the youth o There is historical difficulty with engagement in services ❑ Other services have been exhausted or will not meet the needs If all of these factors exist, this is an appropriate referral.
