Weld County Area Agency on Aging (WCAAA), a division within our department, has a longstanding commitment to supporting caregivers and older adults, and I am proud of the work we have done to meet our community's evolving needs. For over thirty years, WCAAA has operated a successful Caregiver Respite Program funded exclusively through the Older Americans Act (OAA). This program offers consumer -directed vouchers, giving caregivers the autonomy to choose the type of respite care that works best for their unique circumstances. However, like many programs nationwide, we are facing significant funding reductions, specifically, our caregiver program. This grant would allow us to maintain and expand critical caregiver supports at a time when demand is higher, and waitlists are at capacity. This project is directly aligned with our department's mission: Engaging and partnering with the community to improve the safety, health, and well-being of individuals and families through the delivery of responsive and collaborative services. It also reflects our vision of ensuring that Weld County residents are connected to the resources they need to thrive and feet safe and empowered in their communities. With grant support, we will launch several innovative initiatives. We are excited for the potential to partner with a new Adult Day Care Center in Greeley, which will bring structured daytime respite back to our community for the first time since 2015. The Center will also offer transportation, removing a major barrier for many families. In addition, WCAAA will expand access to evidence - based caregiver education, including the Powerful Tools for Caregivers and Stress Busting Program, led by our certified staff and offered both in person and online. We will also continue our successful partnership with North Range Behavioral Health's Peer Counseling Program, which provides emotional support for caregivers across the county. Project Summary (750) The Weld County AAA Caregiver Respite Program has been providing flexible, consumer -directed respite services to family caregivers for 30 years. The program offers vouchers to caregivers age 60 and older, empowering them to choose the type and timing of care that best meets their needs. Due to limited funding, services are prioritized for caregivers who are low-income, live in rural areas, are from minority populations, or are caring for frail or cognitively impaired older adults. While this model has offered valuable support for decades, it is time to expand our approach to meet the evolving and growing needs of local caregivers. Through this grant, we are ready to launch a new model of service delivery that introduces structured respite options to include emergency respite and enhances educational and emotional support services for caregivers such as our peer -counseling through North Range Behavioral Health. Each of these components addresses long- standing service gaps in Weld County and reflects a strategic evolution of our caregiver support. Weld County has been without an adult day program since 2015. The absence of this vital resource has placed significant strain on caregivers, especially those who relied on regular, dependable care for their loved ones. We are excited to report that a new Adult Day Center will soon open in Greeley, the largest city in Weld County that is home to over 30% of the population in the county and is eager to partner with Weld County AAA. Through this collaboration, caregivers will use vouchers for routine or intermittent adult day services in a professionally staffed, safe, and engaging environment. This structured setting not only ensures the well-being of older adults but also gives caregivers time to rest, work, or attend to personal needs. In addition to respite options, the grant will support the development of an emergency respite care option. Caregivers often face unexpected situations, hospitalizations, crisis, or burnout that require immediate relief. Our new model will designate a portion of respite vouchers specifically for emergency use, giving caregivers fast - tracked access to in -home care facility -based services. We will create clear referral and eligibility protocols and partner with health providers and local agencies to streamline access to these short-term services. Beyond respite, this project will transform educational and peer support offerings. We will expand delivery of the Powerful Tools for Caregivers Program, an evidence - based, six -week course that teaches caregivers to manage stress, set goals, communicate effectively, and access community resources. The program is known to improve caregivers' confidence and reduce overwhelming feelings. By offering this course more frequently and in multiple formats (in -person and virtual), we will ensure accessibility for caregivers across rural Weld County and those facing transportation or scheduling challenges. We will also expand our partnership with North Range Behavioral Health to provide Peer -to -Peer Counseling. This service offers emotional support through trained peers who understand the caregiving journey firsthand. Peer support has proven especially effective for caregivers who feel isolated or emotionally depleted, and it complements our educational offerings by addressing the personal, relational, and emotional challenges that come with long-term caregiving. This comprehensive model represents a major innovation in how we support caregivers in Weld County. Historically, our respite program has focused solely on a consumer -directed voucher approach. By layering in structured facility -based care, emergency services, and targeted education and peer support, we are not only expanding the range of services available, but also improving the continuity and responsiveness of care. This initiative allows caregivers to access the right support at the right time, whether that's a weekly adult day session, a last-minute emergency break, or ongoing emotional and educational support. During the project period, Weld County AAA will work closely with all partners to ensure smooth service delivery, monitor utilization, and gather feedback from participants. We will track the number of caregivers served, hours of respite delivered, class attendance, and counseling participation. Data will be used to make real-time adjustments, identify barriers, and demonstrate the value of each component to stakeholders and funders. We will also prioritize outreach to underserved populations to ensure equitable access across rural and minority communities. By funding this project, we will reestablish a key support that has been missing in our community for nearly a decade. Caregivers are often the invisible backbone of long- term care, and this initiative sends a clear message: they are not alone, and their work is valued. Through structured respite, emergency relief, education, and peer connection, this program will strengthen the resilience of caregivers and allow more older adults to safely age in place. 2. Anticipated Outcomes and Evaluation Plan (750 words) The primary anticipated outcome of this project is to increase access to high -quality, flexible respite care for Weld County caregivers, especially those with limited financial resources or caring for frail or cognitively impaired loved ones. Through this expanded model, including structured respite services, emergency respite, education, and peer counseling, we aim to reduce caregiver stress, prevent burnout, and help older adults remain safely at home. The launch of a new Adult Day Center in Greeley presents an opportunity to reintroduce structured respite services to our community for the first time since 2015. Caregivers will gain access to safe, supervised daytime care for their loved ones, allowing them time to rest, work, or manage other responsibilities. The Adult Day Center will also offer transportation, reducing barriers for caregivers who cannot provide transportation themselves. A portion of funding will be reserved for emergency respite, available to caregivers facing sudden challenges such as illness, personal crisis, or caregiver fatigue. This flexible option will serve as a safety net, helping families manage unplanned events that could jeopardize their ability to continue caregiving. This project will also support evidence -based caregiver education through the Powerful Tools for Caregivers and Stress Busting programs. Weld County AAA has three certified staff members trained to lead these six -week classes, which teach caregivers to manage stress, communicate effectively, and care for their own well- being. Courses will be offered in multiple locations and virtually, expanding access for caregivers with transportation or scheduling limitations. In addition, we will continue our partnership with North Range Behavioral Health to provide peer -to -peer caregiver counseling. These sessions offer emotional support from trained peers who understand the challenges caregivers face. Peer counseling helps reduce isolation, enhance mental wellness, and complements our educational programming. We anticipate the following during the grant period: Serve at least 40 caregivers using structured respite, depending on service availability and funding. Provide a minimum of 500 hours of facility -based respite. Support at least 15 caregivers with emergency respite services during times of crisis. Offer at least four educational cohorts of Powerful Tools or Stress -Busting classes, reaching a minimum of 40 caregivers. Provide peer counseling to at least 30 caregivers. Achieve a 90% satisfaction rate across all services, based on the State Unit on Aging (SUA) Performance Measures Survey. Quantitative data will include: • Number of caregivers served by each program component. • Hours of respite care delivered (routine and emergency). • Attendance and completion rates for education sessions. • Peer counseling participation. • Demographic data to assess service equity, particularly among low-income, rural, and minority caregivers. Qualitative data will be collected through: The SUA Performance Measures Survey administered post -service, which captures feedback on satisfaction, stress reduction, and perceived impact. Open-ended caregiver comments that offer insight into emotional well-being and unmet needs. Informal caregiver feedback gathered by AAA staff and partner organizations through follow-up calls and ongoing engagement. Weld County AAA will oversee all program implementation and coordinate closely with the Adult Day Center and North Range Behavioral Health to ensure seamless service delivery, data sharing, and progress monitoring. Internal meetings will be held quarterly to review performance, address barriers, and make any necessary program adjustments. Real-time data will help us remain responsive and effective in meeting caregiver needs. All components of the initiative, adult day services, emergency respite, education, and counseling, will be offered throughout the project period. We will monitor utilization, track outcomes, and continuously refine delivery strategies to maximize impact and accessibility. Ultimately, this project is designed to ease the burden on caregivers by offering meaningful, flexible supports that improve their quality of life and caregiving capacity. By expanding access to structured respite, empowering caregivers with skills and education, and connecting them with emotional support, we will enhance caregiver resilience and improve the likelihood that more older adults can safely age in place. 3. Describe your Organizations Capacity to successfully establish, manage and evaluate this grant (750) The Weld County Area Agency on Aging (AAA) has over 30 years of demonstrated success delivering, managing, and evaluating grant funded programs supporting older adults, caregivers, individuals with disabilities, and kinship caregivers. We have extensive experience administering federal and state grants through the Colorado State Unit on Aging (SUA), including Older Americans Act (OAA) Title III programs. In addition to SUA support, we have previously received funding from EasterSeals, which allowed us to serve additional caregivers with much -needed respite assistance. These various funding streams require rigorous fiscal oversight, detailed tracking, and outcome reporting, all of which Weld County AAA has executed with reliability and success. Our internal systems, supported by Weld County's fiscal department, provide a strong foundation for managing budgets, tracking expenditures, and ensuring compliance with grant requirements. We maintain accurate, transparent financial practices and collaborate closely with fiscal staff to monitor fund utilization. To date, we have had no difficulty fully expending allocated funds, especially in high -demand areas like respite care. During the COVID-19 pandemic, we successfully scaled our services using American Rescue Plan Act (ARPA) funds to meet the growing needs of caregivers. However, since the expiration of ARPA funding, sustaining that level of service has become more difficult, even though the demand remains unchanged. Waitlists for respite services have grown due to the funding shortfall, underscoring the urgent need for supplemental support. Weld County AAA has operated a dedicated Caregiver Program for over 30 years, steadily evolving and expanding the services provided. Our team continually adapts programming to meet the changing needs of caregivers and the community. This includes modifications to voucher programs, the integration of new educational tools, and enhanced coordination with internal teams such as the Aging and Disability Resources for Colorado (ADRC), adult protective services and case management for waiver programs. Together, we ensure caregivers are connected to a full spectrum of resources beyond the Caregiver Program itself, including transportation, benefits counseling, behavioral health support, and long-term care options. We have a strong track record in program evaluation and outcome tracking, using both quantitative and qualitative methods to assess impact. Weld County AAA routinely tracks metrics such as units of service, clients served, voucher redemption, and class participation. We also administer the State Unit on Aging's Performance Measures Survey, which collects standardized data on caregiver satisfaction, perceived benefit, and emotional well-being. These results are reviewed to identify trends, highlight successes, and make evidence -based program improvements. Collaboration with community partners is integral to our success. For this project, we are exploring opportunities the new Adult Day Center in Greeley, which is reintroducing structured respite care to our region for the first time since 2015. This relationship will allow us to refer caregivers for services, coordinate voucher usage, and support program accessibility, including transportation. We also continue to partner with North Range Behavioral Health's Peer Counseling Program, which offers emotional and social support for caregivers throughout Weld County. Their distributed locations ensure caregivers have access to support including in -home visits regardless of geography. Additionally, evidence -based classes, such as Powerful Tools for Caregivers and Stress Busting Program, will be offered directly by AAA -certified staff at multiple locations and online to ensure flexibility for participants. Weld County AAA also has extensive experience serving a wide variety of caregivers, including those supporting older adults with dementia or chronic illness, adults with disabilities, and kinship caregivers. Our program is inclusive, adaptive, and deeply rooted in the community. We strive to meet caregivers where they are, offering choice, flexibility, and education to strengthen their ability to continue providing care. Sustainability planning is critical. While this grant would significantly enhance our ability to meet current needs, ongoing funding remains a challenge due to rising demand and the increasing number of older adults nationwide. The expiration of ARPA funding has only made this more apparent. If awarded this grant, we will actively seek additional funding opportunities and partnerships to help sustain the program beyond the grant period. We will also advocate at both the state and federal level for increased investments in caregiver services. Continued collaboration with local partners, data -driven evaluations, and targeted outreach will help ensure that services remain accessible, impactful, and community -responsive even after the conclusion of grant funding. In summary, Weld County AAA has the experience, systems, partnerships, and commitment required to successfully manage and evaluate this project. Vouchers will be used to support: • Structured adult daycare respite with the opportunity of partnering with the newly established Adult Day Center in Greeley, marking the return of this vital service to our community for the first time since 2015. • Emergency respite for caregivers facing acute challenges such as illness, crisis, or caregiver fatigue. • Supplemental Support Resources such as evidence -based classes, Powerful Tools for Caregivers and Stressbusting as well as peer counseling through North Range Behavioral Health. This approach promotes flexibility and person -centered support. We expect to serve at least 40 caregivers through adult day services and a minimum of 15 caregivers with emergency respite during the grant period. Caregivers will be prioritized using WCAAA's existing Prioritization Worksheet, which assesses need based on income, functional status, minority status, and other factors. Indirect Costs — $0 Weld County is not requesting reimbursement for indirect costs. Administrative oversight, fiscal monitoring, and reporting will be supported through internal systems and covered under the local match. Matching Funds — Weld County Funds ($16,666.67) The required 25% match will come from Weld County's AAA State funding. These local funds will be applied to voucher distribution and administrative capacity needed to carry out the program. This contribution underscores Weld County's commitment to investing in caregiver support services. Despite a 35% reduction in OAA funding for respite. The county is committed to sustaining high -impact services and leveraging all available resources to serve our caregivers. Program Oversight and Fiscal Capacity Weld County DHS has extensive experience managing a wide range of federal, state, and locally funded programs. Our fiscal department provides consistent oversight to ensure accuracy, transparency, and compliance with all grant requirements. We have a proven record of fully expending allocations in high -demand programs like respite care. During the COVID-19 pandemic, WCAAA successfully scaled services using American Rescue Plan Act (ARPA) funds. Although ARPA funds are no longer available, the demand for caregiver support remains high, and this grant will help prevent service reduction. Anticipated Activities Supported by the Budget All funds will be used to deliver direct respite services through vouchers. This includes: • Adult Day Care Services: In partnership with the new Greeley Adult Day Center, caregivers will have access to structured daytime care and transportation services for their loved ones. • Emergency Respite: Funds will support crisis -based care solutions when caregivers face immediate or unplanned needs. • Supplemental Support: Although not directly funded by the grant, the project will be supported by internal staff delivering evidence -based educational programs (such as Powerful Tools for Caregivers and Stress Busting) and peer -to -peer counseling through our collaboration with North Range Behavioral Health. These wraparound services help address not only the physical needs of caregivers but also their emotional and mental well-being. Sustainability Weld County recognizes the urgency of continuing these services beyond the pilot period. As the aging population continues to grow, caregiver demand will increase. The local match represents our commitment to sustaining this program. We will continue to pursue additional funding opportunities, advocate at the state and federal level, and maintain partnerships to help support ongoing access to care. About This Grant Opportunity 1A. Eligible Applicants This funcing opportunity is open to Aging Network organizations, including: • State Units on Aging • Area Agencies on Aging (AAAs) • Title VI Native American Aging Programs • Community -Based Organizations (CBOs) contracted by AAAs/Title VI Programs to deliver Older Americans Act funded Title III -E or VI -C caregiver services and supports For-profit entities are not eligible for this opportunity. Organizations that serve rural populations or areas are strongly encouraged to apply for this funding opportunity. 1 B. Description U SAging is releasing a call for proposals to provide 18 -month seed grants to be awarded to approximately eight agencies. Grants will support the development of new, or expansion of existing innovative caregiver practices or programs aimed at meeting the unique needs of caregivers. This project is part of the Innovations in Family Caregiver Services and Supports initiative, which aims to expand and enhance the capacity of the Aging N etwork to strengthen and spur innovation in the development and implementation of person and family - centered services and supports that meet the needs of the nation's caregivers. Applicants' proposed program must focus on one or more of the following areas, which align with Goal 3 of the N ationa Strategy to Support Family Caregivers: • Person and family -centered, trauma -informed and culturally competent care • Respite care • Education/counseling/peer supports • Social determinants of health • Volunteer -driven programs • Emergency response, planning and preparedness • Future care/long-term services and supports • Kinship care Last Updated: 06/2025 Innovatiors in Family Caregiver Services and Supports is a project of USAging, funded by the Administration for Community Living as part of its National Caregiver Support Collaborative. Direct Service Provision • Applicants must demonstrate the capacity to provide direct services throughout the project period. Involvement in Technical Assistance and Peer Networking Offerings USAging will offer opportunities to support grantees in virtual one-on-one and group settings. Participation in the following technical assistance offerings is a requirement of the grant project. • Affinity Group Participation: Pilot sites will be brought together during their project periods in an affinity group format to share learnings, brainstorm challenges and highlight successes. • Participation in One -on -One Meetings: USAging will schedule regular one-on-one check -in meetings with grantees to discuss project status, brainstorm solutions to challenges and celebrate accomplishments • Participation in the Caregiver Champion Collaborative, a national peer learning and exchange forum for caregiving professionals across the Aging Network. 1 D. Outputs and Outcomes Data Collection: Pilot sites must collect qualitative and quantitative outcome data. An emphasis will be on collecting meaningful outcomes -data that can be used to support and sustain future efforts, and to inform others who will implement similar programs in the future. Applicants should highlight their program evaluation strategies and provide updates during the quarterly reporting process. Applicants should also clearly outline the evaluation methods they intend to use, ensuring that these methods are feasible within the available budget of up to $50,000. A combination of both qualitative and quantitative data collection approaches is recommended. Examples of caregiver outcome measures include the Perceived Self -Efficacy Scale. Additional measures can be found in this document: CARR White Paper Common Data Elements.pdf, Case Studies: Following the 18 -month pilot period, USAging will develop case studies that summarize project innovations, emphasize lessons learned and share recommendations for other agencies looking to implement similar caregiver programs in their communities. Grantees will provide information necessary to develop the case studies and participate in the review process. 2. Reporting Requirements Applicants must commit to reporting on their progress and financials regularly. The following reporting activities will be required: Quarterly Programmatic Reports: Provide narrative programmatic reports every three months and at completion of funding. Reports will include a summary of activities. Program evaluation data (qualitative and quantitative) will be provided to USAging in these reports. Innovative Caregiver Services & Supports Pilot Sites — Request for Proposals USAginq Quarterly Financial Reports: Provide financial reports on spending and project match on a quarterly basis. USAging will provide a financial report template. 3. Application Review 3A. Compliance Review Review Process: All proposals submitted for the pilot site opportunity will undergo a comprehensive programmatic and financial review. USAging will use Submittable, a user-friendly online application and program management tool, to accept and review applications. The initial screening criteria include the following minimum requirements: • The project must address one or more of the Focus Areas outlined in section 1B. • The application must be submitted by an eligible applicant. • The application must include one letter of support. • The applicant must upload a SAM registration certificate and provide a Unique Entity Identifier (UEI) number in the application portal. • The project budget must not exceed $50,000. • The complete application must be submitted in Submittable by August 1, 2025, at 11:59 PM ET. 3B. Review Committee and Application Scoring Review Criteria: Applications that meet the minimum requirements will be reviewed and assessed by a review committee. They will be rated as Highly Recommended (HR), Recommended (R), and Not Recommended (NR) in the following areas: • Project Summary • Anticipated Outcomes and Evaluation Plan • Demonstration of Organizational Capacity • Budget • Workplan The review committee will follow these guidelines: • Highly Recommended (HR): Applications that provide thorough and detailed responses to questions, along with evidence supporting the likelihood of success of the proposed project. HR applications are distinguished by their clarity, feasibility, and potential for meaningful outcomes. • Recommended (R): Applications that exhibit several major strengths and provide evidence of probable success in achieving the goals, objectives, and requirements of the grant. • Not Recommended (NR): Applications that do not respond fully to all questions, omit key information and demonstrate few strengths. An NR rating indicates significant shortcomings or weaknesses, such as lack of clarity, feasibility issues, or insufficient alignment with the grant's priorities. 3C.
