Weld County, Colorado Dear Surface Owner: Select Water Solutions, LLC ("Select"), is proposing to convert a Class II UIC Disposal well to Class I Non - Hazardous Injection well. The LSWD #1 is an existing Class II UIC well. The well is currently under the Colorado Energy and Carbon Management Commission regulatory oversight. Upon approval of the conversion Class I well, it will fall under the regulatory authority of Federal Environmental Protection Agency. In accordance with EPA 40 CFR 147.305(f), Select is providing notice of its intent to submit a Class I Non - Hazardous permit application. Please find the project details below. Operator name and contact information: Select Water Solutions, LLC 12515 Carriage Way Oklahoma City, OK 73142 Contact Kim Henderson at khenderson@selectwater.com EPA 40 CFR 147.305(f)(2) - The notice of application will briefly describe the injection activities, including well location, name and depth of the injection zone, maximum injection pressure and volume, and fluid to be injected: II/11/25 The intent of the application will be to convert the existing LSWD #1 (API # 05-123-30367) Class II injection vertical well to a Class I Non -Hazardous injection well. The LSWD #1 well location is 510' FNL, 2150' FWL, NENW Sec. 18-T3N-R64W. The LSWD #1 is proposed to continue injection into the Denver Basin Combined Disposal zone which consists of formations but not limited to Lyons, Entrada, and Virgil. The depths of injection zone is between approximately 8,110 feet and 9,592 feet. The requested maximum injection pressure is maintain the current 2,000 psi with a proposed daily volume injection of 20,000 bbls.
