Approve Amendment #7 to Task Order Contract for Prevention of Infertility and
Management of Sexually Transmitted Infections (STI), and authorize Chair to sign
Whereas, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
Whereas, the Board has been presented with Amendment #7 to the Task Order Contract
for the Prevention of Infertility and Management of Sexually Transmitted Infections (STI)
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment,
commencing upon full execution of signatures, and ending February 28, 2026, with further
terms and conditions being as stated in said amendment, and
Whereas, after review, the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
Now, therefore, be it resolved by the Board of County Commissioners of Weld County,
Colorado, that Amendment #7 to the Task Order Contract for the Prevention of Infertility
and Management of Sexually Transmitted Infections (STI) between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Department of Public Health and Environment, and the Colorado
Department of Public Health and Environment, be, and hereby is, approved.
Be it further resolved by the Board that the Chair be, and hereby is, authorized to sign
said amendment.

Brief description of the problem/issue:
For the Board's review and approval for placement on the agenda is contract amendment #6 between the
Colorado Department of Public Health and Environment (CDPHE) and the Board of County Commissioners of
Weld County for the use and benefit of the Weld County Department of Public Health and Environment.
This amendment continues funding and services for the prevention, testing, and treatment of sexually
transmitted infections (STIs) in Weld County. The scope of work includes clinical and non -clinical testing
services, treatment of Chlamydia, Gonorrhea, Syphilis, and HIV, as well as outreach and education to priority
populations. This contract amendment increases funding to $73,572 for the term February 1, 2025, through
February 28, 2026.
What options exist for the Board?
Approve: Approval will allow the health department to continue providing critical STI prevention and treatment
services. This funding supports staff and materials for the program and reduces the risk of disease spread.
Decline: Declining the amendment would result in a reduction in services or an increase in county funding.
Consequences:
If Approved: Weld County residents will benefit from increased access to free STI screening,
treatment, and education services. The program aligns with state and federal public health goals and
ensures compliance with grant deliverables.
If Not Approved: Loss of funding could result in reduced availability of testing and treatment services,
negatively impacting public health outcomes and increased costs to the county for managing STI-
related health complications.
Impacts:
• The amendment allows the department to continue STI testing and treatment in Weld County.
• Maintains critical public health services without increasing county budget burdens.
Costs (Current Fiscal Year/ Ongoing or Subsequent Fiscal Years):
• Contract term: February 1, 2025, to February 28, 2026.
• CDPHE will reimburse the county up to $73,572.
• Activities will be conducted by current staff; no additional FTE is being requested to perform the
contract deliverables.
Recommendation: I recommend approval to place this contract amendment with CDPHE on a future agenda
for formal consideration.

1. PARTIES
This Amendment (the "Amendment") to the Original Contract shown on the Cover Page for this
Amendment (the "Contract") is entered into by and between the Contractor, and the State.
2. TERMINOLOGY
Except as specifically modified by this Amendment, all terms used in this Amendment that are
defined in the Contract shall be construed and interpreted in accordance with the Contract.
3. AMENDMENT EFFECTIVE DATE AND TERM
A. Amendment Effective Date
This Amendment shall not be valid or enforceable until the Amendment Effective Date
shown on the Signature and Cover Page for this Amendment. The State shall not be bound
by any provision of this Amendment before that Amendment Effective Date, and shall have
no obligation to pay Contractor for any Work performed or expense incurred under this
Amendment either before or after of the Amendment term shown in §3.B of this Amendment.
B. Amendment Term
The Parties' respective performances under this Amendment and the changes to the Contract
contained herein shall commence on the Amendment Effective Date shown on the Signature
and Cover Page for this Amendment or February 1, 2025, whichever is later, and shall
terminate on the termination of the Contract or February 28, 2026, whichever is earlier.
4. PURPOSE
The Parties entered into the agreement to serve to reduce the spread of Human Immunodeficiency
Virus (HIV) throughout Colorado, with a focus on serving priority populations at increased risk
for acquiring HIV to prevent new cases of Sexually Transmitted Infections (STI), throughout
Colorado with a focus on serving priority populations at risk for acquiring a sexually transmitted
infection within the Service Area..
The Parties now desire to renew for an additional term, increase funding, change the
Statement of Work and change the current Contract Maximum Total: for the following
reason: to expand the scope of services for the new contract term.
5. MODIFICATIONS
The Contract and all prior amendments thereto, if any, are modified as follows:
A. The Contract Maximum Amount table on the Contract's cover Page is hereby deleted and
replaced with the Current Contract Maximum Amount table shown on the Cover Page for
this Amendment.

B. The Contract Initial Contract Expiration Dale on the Contract's Signature and Cover Page

C. The Parties now agree to modify Exhibit B- SOW of the agreement. Exhibit B- SOW is
deleted and replaced in its entirety with Exhibit B- SOW, attached to this Amendment for
the following reason: to add additional activities to the contract.
D. The Parties now agree to modify Exhibit C- Budget of the agreement. Exhibit C- Budget
is deleted and replaced in its entirety with Exhibit C- Budget, attached to this Amendment
for the following reason: to fund additional activities for the new term.
6. LIMITS OF EFFECT AND ORDER OF PRECEDENCE
This Amendment is incorporated by reference into the Contract, and the Contract and all prior
amendments or other modifications to the Contract, if any, remain in full force and effect except
as specifically modified in this Amendment. Except for the Special Provisions contained in the
Contract, in the event of any conflict, inconsistency, variance, or contradiction between the
provisions of this Amendment and any of the provisions of the Contract or any prior modification
to the Contract, the provisions of this Amendment shall in all respects supersede, govern, and
control. The provisions of this Amendment shall only supersede, govern, and control over the
Special Provisions contained in the Contract to the extent that this Amendment specifically
modifies those Special Provisions.

STATEMENT OF WORK

These provisions are to be read and interpreted in conjunction with the provisions of the contract specified above.
L Entity Name: Weld County Health Department
IL Project Description:
This public health project serves to reduce new cases of Sexually Transmitted Infections (STI) throughout
Colorado with a focus on serving priority populations at risk for acquiring STIs within Weld County through
STI screening and treatment. Colorado has seen substantial increases in sexually transmitted infections (STI)
morbidity in 2021. Appropriate and prompt testing of persons with STIs and exposure is critically important
in preventing health complications and interrupting disease transmissions in communities.
This project benefits individuals who are uninsured or underinsured and need access to free STI screening
and treatment in the federally funded Title X, STI, and community health clinics as well as non -clinical
outreach sites. This project promotes STI testing and treatment, as identified by the Centers of Disease
Control and Prevention's Strengthening Sexually Transmitted Infections' Prevention and Control for Health
Departments (STI PCHD) grant funding, to implement and strengthen STI prevention and control programs.
III. Definitions:
1. 340B Pricing - program allows qualifying hospitals and clinics that treat low-income and uninsured
patients to buy outpatient prescription drugs at a discounted price from the manufacturer.
2. CDC — Centers for Disease Control and Prevention
3. CDPHE- Colorado Department of Public Health and Environment
4. Clinical Setting - hospital, department, outpatient facility or clinic whose primary purpose is
diagnosing.
5. Client - An individual receiving contracted services by the Contractor.
6. CT — Chlamydia Trachomatis - a sexually transmitted disease that when left untreated, may result
in infertility in women
7. EPT — Expedited Partner Therapy
8. HIV- Human Immunodeficiency Virus
9. GC — Neisseria gonorrhea - a sexually transmitted disease that when left untreated, may result in
infertility in women
10. Increased Risk — Those who have a new sex partner, more than one sex partner, a sex partner with
concurrent partners, or a sex partner who has an STI. Additional risk factors include inconsistent
condom use among persons who are not in mutually monogamous relationships; previous or
coexisting sexually transmitted infections; and exchanging sex for money or drugs. Clinicians
should consider the communities they serve and may opt to consult state and local public health
authorities for guidance on identifying groups that are at increased risk.
11. Non -Clinical Setting - community -based, outreach sites, a person's home
12. Priority Populations: adolescents and young adults, men who have sex with men, and pregnant
women
13. Service Area: Weld County
14. STI- Sexually transmitted infections(s), including GC, CT and Syphilis.
15. STI Testing Services - includes screening for chlamydia, gonorrhea and syphilis
16. Syphilis — a sexually transmitted disease that when left untreated, may result in serious
complications or cause congenital syphilis among pregnant women.
17. Title X — Title X of the Public Health Service Act (Public Law 91-572 Population Research and
Voluntary Family Planning Programs). The federal grant program is dedicated solely to providing
individuals with comprehensive family planning and related preventive health services.

IV. WORK PLAN:
Goal #1: Provide high -quality HIV, STI, and VH prevention services to increase the priority population's access
to testing, education, and treatment, and reduce the acquisition of HIV/STINH throughout Colorado in order to
disrupt health inequities.
Objective #1: No later than the expiration date of the contract, reduce STI acquisition by providing STI testing
and treatment services for the priority population in the service area.
Primary Activity #1
The Contractor shall complete STI testing services in clinical settings.
Sub -Activities #1
1. The Contractor shall screen individuals for STI testing services.
2. The Contractor shall provide ST1 testing services to clients.
3. The Contractor shall maintain clinical staff.
4. The Contractor shall notify clients of positive test results.
5. The Contractor shall refer clients with a positive result for STI treatment.
Primary Activity #2
The Contractor shall complete STI testing services in non -clinical settings.
Sub -Activities #2
1. The Contractor shall screen individuals for STI testing services.
2. The Contractor shall provide STI testing services to a minimum of 500 clients.
3. The Contractor shall verify staff providing STI testing in non -clinical settings have
completed CDPHE STI testing course requirements.
4. The Contractor shall notify clients of positive test results.
5. The Contractor shall refer clients with a positive result for STI treatment.
6. The Contractor shall maintain a referral list of partnering clinical agencies.
Primary Activity #3
The Contractor shall provide STI treatment.
Sub -Activities #3
1. The Contractor shall determine the client's eligibility to receive medications through
the 340B pricing program.
2. The Contractor shall provide STI treatment for clients with a positive STI result.
3. The Contractor shall provide partner treatment via EPT, when indicated.
Primary Activity #4
The Contractor shall virtually attend scheduled CDPHE monitoring calls.
Primary Activity #5
The Contractor shall complete monthly narrative reports.
Standards and
Requirements
1. The content of electronic documents located on CDPHE and non-CDPHE
websites and information contained on CDPHE and non-CDPHE websites may
be updated periodically during the contract term. The Contractor shall monitor
documents and website content for updates and comply with the most recent
revisions.
2. The Contractor shall perform STI screening in accordance with current CDC
screening guidelines available on the following website

incorporated and made part c f this Contract by reference.
3. The Contract shall maintain one (1) of the following for STI testing throughout
the life of the project:
a. Valid, non -expired CLIA waiver certificate
b. Valid, non -expired CLIA certificate
4. The Contractor shall provide open hours for clinical testing at a minimum of 6
hours per week.

5. The Contractor shall use STI treatment drugs from this project to treat STIs for
low-income clients who would be unable to afford the medications otherwise as
outlined in guidelines for 340B Pricing Program found here:
https://www.hrsa.gov/opa/index.html. This information is incorporated and made
part of this contract by reference.
6. The Contractor shall adhere to the current Sexually Transmitted Diseases
Summary of CDC Treatment Guidelines, available on the following website
https://www.cdc.gov/std/treatment-guidelines/toc.htm and are incorporated and
made part of this Contract by reference.
7. The Contractor shall comply with the data collection and reporting requirements
stated in the Guidelines Regarding Data Collection and Reporting. This
document can be accessed at https://www.colorado.gov/pacific/cdphe/sti-hiv-
funded-partner-policies and is incorporated and made part of this Contract by
reference.
8. The Contractor shall comply with the guidelines concerning data security and
confidentiality stated in the Centers for Disease Control's Data Security and
Confidentiality Guidelines for HIV, Viral Hepatitis, Sexually Transmitted
Disease and Tuberculosis Programs. This document can be accessed at

es.pdf is incorporated and made part of this Contract by reference.
9. The Contractor shall comply with the following Standards of Prevention for
HIV/STI/VH and Harm Reduction Services. These Standards are incorporated
and made part of this contract by reference:

10. The Contractor shall comply with the following rules and regulations when
performing data collection and entry, reporting, and the implementation of
activities. These guidelines and standards are located on the state of Colorado
website http://www.sos.state.co.us/CCR/Welcome.do and are incorporated and
made part of this Contract by reference:
a. Colorado Revised Statute § 25-4-14 concerning HIV acquisition.
b. Colorado Revised Statute § 18-4-412 concerning the theft of medical
records.
c. Colorado State Board of Health Rules for the Reporting, Prevention and
Control of AIDS, HIV Related Illness and HIV Infection (6CCR 1009-

11. The Contractor shall complete a monthly progress report in the format provided
by CDPHE.

settings where individuals for whom a STI test is performed:
i. number of individuals screened for STI
ii epidemiologic data
ii. test results (# positive, # negative)
iv. number of individuals treated for positive STI results
14. The Contractor shall submit deliverables to the CDPHE Contract Monitor via
email at cdphe sthv contractmonitoring(a),state.co.us.

V. Monitoring:
CDPHE's monitoring of this contract for compliance with performance requirements will be conducted
throughout the contract period by the CDPHE Contract Monitor. Methods used will include a review of
documentation determined by CDPHE to be reflective of performance to include progress reports and other
fiscal and programmatic documentation as applicable. The Contractor's performance will be evaluated at
set intervals and communicated to the contractor. A Final Contractor Performance Evaluation will be
conducted at the end of the life of the contract.
VI. Resolution of Non -Compliance:
The Contractor will be notified in writing within 15 calendar days of discovery of a compliance issue. Within
30 calendar days of discovery, the Contractor and the State will collaborate, when appropriate, to determine
the action(s) necessary to rectify the compliance issue and determine when the action(s) must be completed.
The action(s) and timeline for completion will be documented in writing and agreed to by both parties. If
extenuating circumstances arise that requires an extension to the timeline, the Contractor must email a request
to the CDPHE Contract Monitor and receive approval for a new due date. The State will oversee the
completion/implementation of the action(s) to ensure timelines are met and the issue(s) is resolved. If the
Contractor demonstrates inaction or disregard for the agreed upon compliance resolution plan, the State may
exercise its rights under the provisions of this contract.
