Approve Amendment #3 to Task Order Contract for Syphilis Field Delivery (FTD)
and Jail -Based Sexually Transmitted Infection (STI) Programs, and Authorize Chair
to Sign
Whereas, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
Whereas, the Board has been presented with Amendment #3 to the Task Order Contract
for the Syphilis Field Delivery (FTD) Program and the Jail -Based Sexually Transmitted
Infection (STI) Program between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of
Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing upon full execution of signatures, and ending June 30, 2026,
with further terms and conditions being as stated in said amendment, and
Whereas, after review, the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
Now, therefore, be it resolved by the Board of County Commissioners of Weld County,
Colorado, that the Amendment #3 to the Task Order Contract for the Syphilis Field
Delivery (FTD) Program and the Jail -Based Sexually Transmitted Infection (STI) Program
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment, be, and
hereby is, approved.
Be it further resolved by the Board that the Chair be, and hereby is, authorized to sign
said amendment.

Brief description of the problem/issue:
For the Board's approval is the FY25--26 Field -Delivered Therapy (FDT) and Jail -Based STI Program contract
amendment #3 between the Colorado Department of Public Health and Environment (CDPHE) and the Board
of County Commissioners of Weld County on behalf of the Weld County Department of Public Health and
Environment (WCDPHE).
The WCDPHE has partnered with CDPHE through a contract that funds two key interventions: Field -Delivered
Therapy (FDT)—a nurse -led approach to delivering syphilis treatment directly in the field for 9 counites in NE
Colorado and expanded STI screening and treatment in the Weld County Jail.
What options exist for the Board?
• Approve the contract amendment as presented.
• Decline to approve the contract amendment.
Consequences: Approval will ensure continued funding for the Field -Delivered Therapy and Jail -Based
STI program at current service levels. Declining the contract would result in the loss of funding for these
services and reduced capacity to respond to syphilis in high -risk populations.
Impacts: This funding supports Weld County's local and regional efforts to reduce syphilis transmission
by providing timely testing and treatment in the field and in correctional settings promoting early
identification, access to care, and improved health outcomes.
Costs (Current Fiscal Year / Ongoing or Subsequent Fiscal Years):
This contract provides $90,000 in grant funding for FY 2025-2026. There is no required local match.
Recommendation: I recommend approval to place this contract amendment on a future BOCC agenda for
formal consideration.

1. PARTIES
This Amendment (the "Amendment") to the Original Contract shown on the Cover Page for this
Amendment (the "Contract") is entered into by and between the Contractor, and the State.
2. TERMINOLOGY
Except as specifically modified by the Amendment, all terms used in this Amendment that are
defined in the Contract shall be construed and interpreted in accordance with the Contract.
3. AMENDMENT EFFECTIVE DATE AND TERM
A. Amendment Effective Date
This Amendment shall not be valid or enforceable until the Amendment Effective Date
shown on the Signature and Cover Page for this Amendment. The State shall not be bound
by any provision of this Amendment before that Amendment Effective Date, and shall have
no obligation to pay Contractor for any Work performed or expense incurred under this
Amendment either before or after the Amendment term shown in §3.B of this Amendment.
B. Amendment Term
The Parties' respective performances under this Amendment and the changes to the Contract
contained herein shall commence on the Amendment Effective Date shown on the Signature
and Cover Page for this Amendment or July 1, 2025, whichever is later, and shall terminate
on the termination of the Contract or June 30, 2026, whichever is earlier.
4. PURPOSE
The Parties entered into the agreement to serve to reduce new cases of syphilis by screening,
identifying and treating for syphilis in persons of reproductive age in Colorado.
The Parties now desire to increase funding and change the Statement of Work, renew for an
additional term and change the current Contract Maximum Total for the following reason:
to renew for an additional term and increase the quantity of services for the additional
term.
5. MODIFICATIONS
The Contract and all prior amendments thereto, if any, are modified as follows:
A. The Contract Initial Contract Expiration Date on the Contract's Signature and Cover Page
is hereby deleted and replaced with the Current Contract Expiration Date shown on the
Signature and Cover Page for this Amendment.
B. The Contract Maximum Amount table on the Contract's cover Page is hereby deleted and
replaced with the Current Contract Maximum Amount table shown on the Signature and
Cover Page for this Amendment.

the following reason: to add additional activities to the contract.
D. The Parties now agree to modify Exhibit C - Budget of the agreement. Exhibit C - Budget
is deleted and replaced in its entirety with Exhibit C - Budget, attached to this Amendment
for the following reason: tb fund additional activities for the additional term.
6. LIMITS OF EFFECT AND ORDER OF PRECEDENCE
This Amendment is incorporateci by reference into the Contract, and the Contract and all prior
amendments or other modifications to the Contract, if any, remain in full force and effect except
as specifically modified in this Amendment. Except for the Special Provisions contained in the
Contract, in the event of any conflict, inconsistency, variance, or contradiction between the
provisions of this Amendment and any of the provisions of the Contract or any prior modification
to the Contract, the provisions of this Amendment shall in all respects supersede, govern, and
control. The provisions of this Amendment shall only supersede, govern, and control over the
Special Provisions contained in j the Contract to the extent that this Amendment specifically
modifies those Special Provisions.

EXHIBIT B
STATEMENT OF WORK
Amendment Contract Number 2026*0270 Amendment #3
These provisions are to be read and interpreted in conjunction with the provisions of the contract specified above.
I. Entity Name: Weld County Department of Public Health ("WCDPHE")
II. Project Description: This health project serves to prevent new cases of syphilis by screening, identifying
and treating. In partnership with a local county jail, a program will be implemented to offer syphilis
screenings to individuals who are currently incarcerated. In addition to screening, Weld County Department
of Public Health will also provide field -delivered therapy to any individual who is otherwise unable to access
treatment in a clinical setting due to barriers. These barriers include, but are not limited to being unhoused,
lacking primary or prenatal care, lacking adequate insurance coverage, transportation barriers, or substance
use disorders. The project seeks to increase early identification of syphilis, access to treatment and referrals,
linkage to prenatal care services, which will reduce the risk and burden of syphilis transmission. This project
is being funded to address the increasing rates of syphilis in Colorado.
III. Definitions:
1. Advertise - To advertise means a read, written, visual, oral, watched, or heard bid for, or call to
attention. Advertisements can be considered communications or marketing based on the intent and
content of the message.
2. Active Referrals- Active referrals are providing assistance to a client(s) with accessing the service
they are referred to, including but not limited to: assisting clients in making an appointment,
completing a Release of Information, providing resources for client to access services, providing
appointment reminders, following up with the agency providing the service to confirm client's
attendance
3. CDC — Centers for Disease Control and Prevention
4. CDPHE — Colorado Department of Public Health and Environment
5. CLIA — Clinical Laboratory Improvement Amendments
6. DIS - Disease Intervention Specialist
7. Client - An individual receiving contracted services by the Contractor.
8. Comprehensive STI Screening - Chlamydia, gonorrhea, syphilis, and HIV
9. FDT Eligibility Criteria- Any individual with a positive syphilis screen or anyone who is a sex
partner to someone who has tested positive for syphilis.
10. FDA — Food and Drug Administration
11. FDT - Field -Delivered Therapy
12. HCV — Hepatitis C Virus
13. HIV — Human Immunodeficiency Virus
14. Indicated Intervention - Comprehensive STI screening, treatment and referrals
15. LPHA - Local Public Health Departments in Colorado
16. Prenatal care - Medical care, specific to individuals who are pregnant, that is received during
pregnancy; including check-ups and prenatal testing
17. Pregnancy Testing - A test that checks the client or patient's urine or blood for a hormone called
human chorionic gonadotropin (hCG).
18. PrEP — Pre -Exposure Prophylaxis
19. Priority Population: Incarcerated individuals, individuals with a positive syphilis screen, or
anyone who is a sex partner to someone who has tested positive for syphilis.
20. Quality Improvement Plan - A set of focused activities designed to monitor, analyze and
improve the quality of processes in order to improve the outcomes of a program.
21. Rapid Syphilis Testing - point of care tests that can be performed outside a laboratory setting,
using a small amount of whole blood collected by a finger prick.
22. Reactive rapid syphilis result - One of three possible results from a rapid syphilis test. A reactive
result using a rapid syphilis test is also interpreted as `positive'.

25. Screen -Assessment of clients to identify the possible presence of and/or risk for additional social,
behavioral, and financial risks including, but not limited to, substance use, intimate partner
violence, medication assistance, and linkage to care
26. Service area - Boulder, Lorimer, Morgan, Weld, Washington, Philips, Logan, Yuma, and
Sedgwick Counties
27. STI- Sexually Transmitted Infection
28. Support Services - Support services may consist of STI/HIV case management, linkage to care
services, housing assistance, food services, mental health treatment, substance use treatment,
transportation assistance, intimate partner violence services, crisis services, and medication
assistance.
29. Technical Assistance (TA) - Guidance provided to LPHAs by sharing written protocols,
reviewing processes and best practices, identification of key partnerships, and assistance with
troubleshooting programmatic challenges. TA may be delivered virtually, through email and/or in
person.
30. VH — Viral Hepatitis
IV. Work Plan:
Goal #1: Provide high -quality HIV, STQ and VH prevention services to increase the priority population's access to
testing, education, and treatment, and reduce the acquisition of HIV/STI/VH throughout Colorado in order to disrupt
health inequities.
Objective #1: No later than the expiration date of the contract, reduce the spread of syphilis through syphilis prevention
intervention services for the priority population in the service area.
Primary Activity #1
The Contractor shall complete the indicated intervention services.
Sub -Activities #1
1. The Contractor shall develop protocols for FDT.
2. The Contractor shall develop protocols for delivering the indicated interventions in the
correctional setting.
3. The Contractor shall provide indicated intervention services.
4. The Contractor shall implement the protocols.
5. The Contractor shall provide client syphilis prevention education.
6. The Contractor shall offer confirmatory testing for clients with a reactive rapid syphilis
result. i

Primary Activity #4 The Contractor shall attend scheduled CDPHE Contract Monitoring Calls.
I. The content of electronic documents located on CDPHE and non-CDPHE websites and
information contained on CDPHE and non-CDPHE websites may be updated
periodically during the Contract term. The Contractor shall monitor documents and
website content for updates and comply with the most recent revisions.
2. The Contractor shall comply with the following rules and regulations when performing
data collection and entry, reporting, and the implementation of activities.
a. The CDPHE Communicable Reportable Conditions Guidelines are located on
the following state of Colorado website. This information is incorporated and
made part of this Contract for reference:

b. The CDPHE STI and HIV funded partner guidance and policies are identified
below. These documents are incorporated and made part of this Contract for
reference and are available on the following website:

3. The Contractor shall maintain one (1) of the following for syphilis testing
throughout the life of the project:
a. Valid, non -expired CLIA waiver certificate
b. Valid, non -expired CLIA certificate
4. The Contractor shall use FDA approved testing technology when administering rapid
syphilis testing and adhere to manufacturers' instructions as to use and storage.
5. The Contractor shall request technical assistance via email at
cdphe sthv contractmonitoring(i4state.co.us.

6. CDPHE will provide a response to requests for technical assistance to the Contractor
upon request within five (5) business days.
7. The Contractor shall collect a minimum of 90% of required data elements for Syphilis
testing and other funded intervention services and activities.
8. CDPHE will act as the Contractor's data administrator for REDCap.
9. The Contractor shall provide access to REDCap for a minimum of one (1) staff person
for the purposes of entering client level data..

10. The Contractor shall complete the following REDCap instruments for the following
client -level indicated interventions:
a. Client Intake
b. STI Testing
c. HIV/HCV Testing

1 I. The Contractor shall enter data related to contracted services into REDCap no later
than ten (10) calendar days following each client encounter.
12. The Contractor shall use the required data elements described in the sample document
NHM&E Data Variables and Values. This document is incorporated and made part of
this Contract by reference and is available on the following website:

13. The Contractor shall use the REDCap Monthly Narrative Report form for the monthly
progress reports.
14. CDPHE will provide the Contractor access to the REDCap Monthly Narrative Report
form via REDCap within 30 days of Contract execution.
15. The Contractor shall include the following items in the monthly narrative report for
each respective contracted service:

16. CDPHE will respond to any request for material review within five (5) business days.
17. The Contractor shall provide the following active referrals for clients needing syphilis
treatment.
a. The Contractor shall work to refer patients for clinical treatment.
b. The Contractor shall work to refer clients to field -delivered therapy (FDT)
for those with a positive syphilis test that have barriers to treatment
c. The Contractor shall work with CDPHE DIS to refer clients to treatment.
18. Quality Improvement Plan: The Quality Improvement Plan shall include the following
related to the implementation and delivery of funded indicated intervention services
and activities:
a. Evaluate the number of individuals from the priority population screened for
syphilis.
b. Evaluate the percentage of individuals who refuse screening in the
Icorrectional facility.
c. Evaluate the syphilis positivity rate.
d. Evaluate the efficacy of processes to ensure individuals from the priority
Ipopulations with a positive syphilis screen that receive completed treatment.
19. The Contractor shall maintain the following staff throughout the contract period:
a. Public Health Nurse I at 13 hours per week.
b. I Public Health Nurse II at four (4) hours per week.
c. I Medical Assistant at four(4) hours per week.
d. Program Coordinator at two (2) hours per week.
e. Outreach Program Coordinator at two (2) hours per week.
20. The Contractor shall submit the written draft Quality Improvement Plan to CDPHE
Contract Monitor.
a. CDPHE will provide feedback and make a request for any revisions to the
Contractor within 15 business days of receipt.
b. The Contractor shall complete the request for any suggested changes or
revisions within 30 business days.
c. The Contractor shall submit the final written Quality Improvement Plan.
21. The Contractor shall monitor the following:
a. Evaluate the number of individuals from the priority population screened and
treated in the following settings:
i. Correctional settings
ii. Field delivered therapy.
b. j Evaluate the efficacy of treatment processes to ensure individuals from the
priority populations complete treatment.
c. Monitor refusal rates for individuals who do not want syphilis screening.
22. The Contractor shall develop the following protocols for field -delivered therapy:

V. Monitoring:
CDPHE's monitoring of this Contract for compliance with performance requirements will be conducted

documentation determined by CDPHE to be reflective of performance to include progress reports, REDCap
reports or reports from other databases as designated by CDPHE, site visits, invoices and other fiscal and
programmatic documentation as applicable. The Contractor's performance will be evaluated at set intervals
and communicated to the Contractor. A Final Contractor Performance Evaluation will be conducted at the
end of the life of the contract.

VI. Resolution of Non -Compliance:
The Contractor will be notified in Writing within 15 calendar days of discovery of a compliance issue.
Within 30 calendar days of discovery, the Contractor and the State will collaborate, when appropriate, to
determine the action(s) necessary to rectify the compliance issue and determine when the action(s) must be
completed. The action(s) and time�ine for completion will be documented in writing and agreed to by both
parties. If extenuating circumstan ,es arise that require an extension to the timeline, the Contractor shall
email a request to the CE Contract (Monitor and receive approval for a new due date. The State will oversee
the completion/implementation of the action(s) to ensure timelines are met and the issue(s) is resolved. If
the Contractor demonstrates inaction or disregard for the agreed upon compliance resolution plan, the State
may exercise its rights under the provisions of this Contract.
