NATIONAL DIABETES MONTH
NOVEMBER 2025
WHEREAS, diabetes continues to impact the health and well-being of Weld County
residents, with nearly one in nine individuals diagnosed with type 2 diabetes and over
1 in 3 residents (38%) living with prediabetes; and
WHEREAS, the Weld County Department of Public Health and Environment is committed
to helping residents prevent or delay the onset of type 2 diabetes through early
detection, data -driven education, and access to evidence -based prevention
programs; and
WHEREAS, Weld County is proud to be among Colorado's leaders in diabetes
prevention through Heart of Weld — the county's own lifestyle change program and
part of the CDC -led National Diabetes Prevention Program — which provides a
culturally competent and inclusive approach, respecting language, traditions, and
unique community needs; and
WHEREAS, participants in Heart of Weld receive encouragement and accountability
from trained lifestyle coaches and a supportive community of peers, helping them
overcome challenges and achieve measurable, life -changing results.
NOW, THEREFORE, BE IT PROCLAIMED by the Board of Weld County Commissioners, that
November 2025, be known as National Diabetes Month and we encourage all residents
to learn their risk for diabetes, support loved ones in making healthy changes and
participate in local programs like Heart of Weld that strengthen and sustain a healthier
Weld County for all.
