NATIONAL HEALTH EDUCATION WEEK
OCTOBER 20-24, 2025
WHEREAS, the Weld County Department of Public Health and Environment's Community
Health Division plays an important role in the health and wellness of Weld County
residents through education, prevention, and data collection and analysis; and
WHEREAS, the Community Health Division coordinates with partner agencies to empower
local communities to improve health through data -drive education and skills; and
WHEREAS, the division supports these goals through evidence -based programs such as
The Heart of Weld Diabetes Prevention Program for nutrition and chronic disease
prevention, RELATE for healthy relationship and youth development education and, the
Weld County Tobacco Education and Prevention Program for reducing youth nicotine
use; and
WHEREAS, through regular data collection efforts such as the triannual Weld County
Community Health Survey, the division gathers critical insight to guide these programs,
measure progress, and strengthen community health outcomes; and
WHEREAS, the Community Health Division is dedicated to empowering individuals and
communities to make informed health decisions and live healthier lives.
NOW, THEREFORE, BE IT PROCLAIMED by the Weld County Board of Commissioners that
the week of October 20-24, 2025, be known as National Health Education Week. The
Board encourages all residents to learn more about the division's programs and
resources at www.weld.gov/go/HealthEd.
