Approve Acceptance of Donation of $8,000.00 by North Range Behavioral Health to
Weld County Department of Public Health and Environment for 2025 Community
Health Survey Reward Incentive Program
Whereas, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
Whereas, North Range Behavioral Health, 2105 Clubhouse Drive, Greeley,
Colorado 80635, has donated $8,000.00 to the Weld County Department of Public Health
and Environment for the 2025 Community Health Survey Reward Incentive Program, and
Whereas, it is the policy of Weld County to formally accept all donations via Resolution.
Now, therefore, be it resolved by the Board of County Commissioners of Weld County,
Colorado, that the donation of $8,000.00 by North Range Behavioral Health, be, and
hereby is, accepted and designated for use by the Weld County Department of Public
Health and Environment for the 2025 Community Health Survey Reward Incentive
Program.
Be it further resolved by the Board that a copy of this Resolution, along with a letter of
appreciation, be forwarded to North Range Behavioral Health.
The Board of County Commissioners of Weld County, Colorado, approved the above
and foregoing Resolution, on motion duly made and seconded, by the following vote on
the 3rd day of February, A.D., 2025:

SUBJECT: Monetary Donation - North Range Behavioral
Health
The North Range Behavioral Health donation of $8,000 is for the Weld County Department of
Public Health and Environment's new Reward Incentive Program for the 2025 Weld County
Community Health Survey. In prior surveys, Weld County has not used a reward incentive
primarily due to logistic and financial constraints of offering them. However, the Health
Department has determined that given the declining response rates of especially young
respondents, the sizeable respondent burden in answering a lengthy survey, and increasing costs,
a reward incentive program needs to be introduced in 2025. The Health Department's Data Unit
researched best practices for implementing reward incentives for mail -based population health
surveys and determined that using a conditional monetary reward once a participant completes
their survey is the best option for Weld County.
The BOCC approved of the Reward Incentive Program at a Work Session on June 17, 2024 and
were updated on the progress of the 2025 Community Health Survey and Reward Incentive
Program at a Works Session on December 16, 2024.
Local health care partners were asked in 2024 to commit to donating funds by Spring 2025. The
survey Reward Incentive Program will use non -governmental funds to cover the cost of reward
incentives and vendor fees. Additional funds received beyond the amount needed for reward
incentives will help defray some of the mail and printing costs. North Range Behavioral Health
was asked if they were interested in partnering and providing funding through a donation for this
Reward Incentive Program and they agreed.
