SERVICE AREA AND AUTHORIZE DEPARTMENT OF PUBLIC HEALTH AND

WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Grant Application for the 2025
Community Health Assessment Survey Reward Incentive Program for the Platte Valley Hospital
Service Area from the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Public Health and Environment,
to Intermountain Health, commencing April 1, 2025, and ending December 31, 2025, with further
terms and conditions being as stated in said application, and
WHEREAS, after review, the Board deems it advisable to approve said application, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Grant Application for the 2025 Community Health Assessment
Survey Reward Incentive Program for the Platte Valley Hospital Service Area from the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Department of Public Health and Envronment, to Intermountain Health, be, and
hereby is, approved.

SUBJECT: Intermountain Health, Platte Valley Hospital
Grant Application
In prior surveys, Weld County has not used a reward incentive. However, the Health Department has
determined that given the declining response rates of especially young respondents, the sizeable
respondent burden in answering a lengthy survey, and increasing costs, that a reward incentive program
needs to be introduced in 2025. According to health survey and market research leaders, implementing
conditional incentives has gotten easier over the years given the expanded access and use of the
internet, the ability of the general public to use web -based surveys, and the ability to provide electronic
rewards.
Therefore, for the 2025 Community Health Survey a conditional incentive of $5 per household will be
offered once a participant completes their household survey. This reward incentive is being funded by
Weld County based health care organizations. Non -governmental funds will be used to defray the cost
of reward incentives, vendor and contractor fees, and some of the mail and printing costs associated
with the Weld Community Health Survey. Use of these partner, non -governmental funds will also
promote community partnership and trust.
The attached grant application is for the Platte Valley Hospital — Intermountain Health, Community
Health Investment Program. The amount requested is $5,000 to cover the costs of the reward incentive
in the Platte Valley service area as well as to cover some additional hard costs for the Weld Community
Health Survey. In acknowledgment of the provision of funds Intermountain Health's information will be
included on survey mailing materials and websites and Intermountain Health will receive a summary
report of the survey findings from the service area respondents for use by the Platte Valley Hospital's
Community Health Investment Program.

File 2: Executive Summary/Overview
Weld County established the second local health department in Colorado in 1938 and is the longest
continuously operating local health department in the state. The Weld County Health Department
achieved accreditation from the Public Health Accreditation Board in November 2016, making it one of
eight agencies to earn this recognition to date in Colorado. The value of accreditation strengthens the
Health Department to better serve Weld County residents through continuous quality improvement,
partnerships, and accountability. The Weld County Health Department is organized into four service
divisions: Administration and Finance; Environmental Health Services; Health Education,
Communication, and Planning; and Public Health Services. The Health Education, Communication, and
Planning Division organizes and oversees all community health assessment and community health
improvement plan activities for the Health Department.
The goal of Weld County Health Department's health assessment activities is to help the County, its
partners, and funders identify and prioritize the communities' most critical health needs. The Health
Department's community health assessment program also facilitates more effective community
collaboration in planning and decision making by partnering with local organizations and facilitating use
of its high -quality local data.
Weld County Health Department collaborates with Intermountain Health, UCHealth, Banner Health,
Salud, Sunrise Community Health, and North Range Behavior Health as well as others on identified
community priorities through their community health assessment and implementation processes.
The 2025 community health assessment survey aligns with the Health Department's vision and mission
as well as the mission of Intermountain Health's Platte Valley Hospital by providing regular, updated data
on the health of Weld County's population, which is a core function of the Weld County Department of
Public Health and Environment and a component of Platte Valley Hospital's federally required
assessment activities. The Community Health Survey is conducted every 3 years. The aggregated results
provide local -level data on health status, health behaviors, and health concerns and needs of Weld
County residents. This survey is part of the Health Department's continuous effort to understand
residents' health status and encourage ongoing community dialogue about health issues and concerns
through the collection of information and data.
The community health assessment and survey program are also what informs Weld County Department
of Public Health and Environment's community health improvement plan or CHIP. Weld County's CHIP is
a shared responsibility of Thriving Weld, which is a collaboration between Weld County Health
Department and over 100 partner organizations across the county (including Intermountain Health and
Platte Valley Hospital). Weld County Health Department and its partners utilize the CHIP to create
meaningful and lasting changes that can improve the health and wellbeing of all Weld County residents.
Thriving Weld's vision, which also aligns with the mission of Platte Valley Hospital, is healthy, connected,
thriving communities where all people have equitable access to resources that improve quality of life.
The goal of the project is to recruit and receive valid survey responses from up to 275 adult residents in
the Platte Valley Hospital service area by June 30, 2025. Success of the project will be measured by
obtaining the final count of surveys received from the Platte Valley Hospital service area households.

a. Description of Need
b. How needs were determined
History and rational of community health assessment in Weld County
Prior to 2007, consistent, representative, county -wide community health assessment activities had
not been done. In 2007, the Weld Community Survey (Weld CHS) was launched by Weld County
Health Department and has been done every three years since. The survey, which is one of many
assessment activities conducted by the health department, is a mail -based scientific random sample
survey done in English and Spanish and via the internet or paper. The survey results are generalizable
to the county's adult population at two levels — countywide and by several geographic regions
defined by US Census tracts. The objectives of the survey are to assess the health status, concerns,
and needs of Weld County adult residents in a robust way so that inferences can be made about
trends and disparities in health and its determinants. The survey, which contains over 50 questions,
primarily uses previously tested and standardized question items so that comparisons and
longitudinal trends can be analyzed with a known margin of error. Each survey cycle, thousands of
residents voluntarily respond to the survey. Over the course of the survey program, about 15,000
residents have participated including over 1,000 residents from the Platte Valley Hospital service
area. Since its inception, the health department data analytics unit has managed and conducted the
survey primarily with Weld County funds.
The Weld Community Health Survey (Weld CHS) is a rich, unique local source of data about the
health and well-being of county residents used by the health department, municipal governments,
health care organizations like Platte Valley Hospital and Intermountain Health, and community
organizations to better inform local program, funding, and policy decisions. The survey itself covers a
variety of health topics including mental health, health care access and insurance, health conditions
and behaviors, preventive screenings, environmental issues, community concerns and needs, and
health -related social and demographic factors. More information and reports can be found here:

Weld County Health Department's survey data has been used to obtain needed funding (internal and
external) for health -related programs and has been used to help design needed programs. Weld
County Health Department's assessment efforts have been instrumental in helping other county
departments, community organizations, and health and human service leaders to prioritize needs
and collectively work complex health -related common goals such as chronic disease prevention and
improving mental health and well-being of county residents.
Declining survey response rates, representativeness, and the need for survey reward incentives in
Weld County
Survey response rates, while not the only measure of survey quality, have been commonly used to
measure survey quality and representativeness. Across the country, over the past two decades,
survey response rates have been declining significantly. Likewise, in Weld County, response rates
have been declining since 2010 when the response rate was at its highest at 41 percent. In 2022, the
Weld CHS response rate was 11 percent (which is considered very good compared to surveys with
similar methodologies), but the total number of responses was below the health department's

desired minimum sample size of 2,400. Mom importantly, while the number of Hispanic and low-
income respondents was still acceptable, the number of young respondents was less desirable. This
decline in the number of younger respondents and decline in the overall response rate is concerning
and may start to impact the overall representativeness of the Weld CHS survey. Initiating a reward
incentive program in 2025 will help improve the overall response rate and representativeness of
responses.
In prior surveys, Weld County has not used a reward incentive primarily due to the logistic and
financial constraints of offering them. However, the Health Department has determined that given
declining response rates, the sizeable respondent burden in answering a lengthy survey, and
increasing costs, that a reward incentive program needs to be introduced in 2025. According to
health survey and market research leaders, implementing conditional incentives has gotten easier
over the years given the expanded access and use of the internet, the ability of the general public to
use web -based surveys, and the ease and ability to provide rewards for surveys.
Conducting high quality probability -based surveys is all about minimizing bias in the results so that
users can confidently use the information gleaned from the survey. The Health Department Survey
Implementation Team has researched best practices for implementing reward incentives for mail -
based population health surveys by a government entity and determined that using a conditional
reward valued at $5 per household once a participant completes their survey is the best reward
option for Weld County's survey. The reward incentive will be provided electronically (Note:
alternatives will be provided for those residents that respond via the paper option) and won't be tied
to participants' survey responses. A qualified vendor was chosen that will manage the safe, secure
handling of these rewards. Providing a reward of this value is not considered coercive and therefore,
should not impact survey results. Use of the reward incentive will improve questionnaire response
rates and demonstrate respect and appreciation for participant's time and effort in completing the
survey.
c. Activities that will support the project:
The Platte Valley Hospital Service Area Incentive Program activities will include:
Promotion of Intermountain Health as a sponsor of the 2025 Survey Incentive Program. This
entails obtaining the Intermountain Health logo, including it in survey print materials and on
the Health Department survey website as well as in survey reports and presentations.
Obtaining the postal addresses from the mail list vendor for the Platte Valley Hospital service
area to obtain the pre -launch total count of eligible households. This list will be used to
monitor responses from households in the Platte Valley Hospital service area and determine
effectiveness of the program by obtaining a response rate. In 2022, the total number of
households selected was 1,819 and the total number of households that responded was 174
resulting in a 10 percent overall response rate for the Platte Valley Hospital service area. In
2025, the projected number of households to be sampled will be similar, about 2,000, and
the goal is to receive 275 valid survey responses for a response rate of 15%.
The vendor for the incentive program will perform the following services: various customer
service tasks including providing the digital reward of choice in a timely manner, answering
questions, and resolving problems if they arise. They will also track, monitor, and provide
reports on the distribution of the incentives.

Health Department staff will monitor the distribution of incentive rewards and track the
response rate for the Platte Valley Hospital service area.
Health Department staff will tabulate and analyze findings from the Platte Valley Hospital
service area responders. A summary report of the survey findings from the service area
respondents for use by Platte Valley Hospital CHIP Program will be created. The tentative
timeline for completion of this activity is December 2025.
Other activities as needed that support the CltS incentive reward program.
d. How funds will be used
The $5,000 requested funds will be used to pay for reward card incentives, program
administration and oversight, and some postage and mailing costs to provide incentives to
residents in the Platte Valley Hospital service area defined by the following census tracts in Weld

The incentive will be given digitally or by mail. The reward card value will be $5 and the
estimated total cost for reward incentives is $1,375. Mailed rewards also incur an additional
$1.50 postage and handling fees of which $25 is included in the requested funds budget to cover
those fees. Other costs included in the funding request are $350 for a portion of the incentive
program management fees (Platte Valley Hospital service area estimate is 10% of total sample
and cost), $600 for a portion of the labor and oversight to monitor, track, and report on surveys,
responses, findings, and reward redemption for this program (10% of total sample and cost).
Finally, $2,650 of the requested funds are to cover a portion of the cost for postage and printing
for the Platte Valley Hospital service area sample. See budget in File 8 for the specific details.
e. How project aligns with the mission of Platte Valley Hospital — Foster Optimal Health for All
Weld County Health Department's vision is that everyone in Weld County has the opportunity
to live their healthiest lives and our overall mission is to serve Weld County by cultivating
partnerships to promote public health and environmental quality through the delivery of
relevant, innovative, and cost-effective services.

This community health assessment project aligns with the Health Department's vision and
mission as well as the mission of Platte Valley Hospital by providing regular, updated data on the
health of Weld County's population, which is a core function of the Weld County Health
Department and a required regular activity of Platte Valley Hospital. The community health
assessment and survey program can then inform community health improvement plans and
implementation strategies to ensure optimal health for all. WCDPHE and Intermountain health
utilize their CHIPs to advance intentional, integrated, and collective approaches to create
meaningful and lasting changes that can improve the health and wellbeing of Weld County
residents.
f. How to assure funds will only be used to benefit individuals in Platte Valley Hospital service area
The funds will only be used for recruiting adults in households within the Platte Valley Hospital
service area. We will obtain a USPS list of addresses and verify they are in the PV service area.
The health department is able to track which households are sent surveys, which households
return surveys, and how many received rewards. The findings (process and outcomes) from the
survey will be provided to the CHIP partners and will be used to assess assets and needs of the
population in the Platte Valley service area.
g.
How project will be evaluated. State overall goal of project. State SMART objectives - include
definition of target population, the measure of success, and the time frame needed.
The goal of the project is to recruit and receive valid survey responses from up to 275 adult
residents (target population) in the Platte Valley Hospital service area by June 30, 2025 (time
frame). Success of the project will be measured by obtaining the final count of surveys received
(measure of success) from Platte Valley Hospital service area households.
Objective 1: By June 30, 2025, invite by mail about 1,820 households in the Platte Valley Hospital
service area to complete the Weld 2025 Community Health Survey either online or by paper
survey.
Measurement: Final count of households in Weld County invited from the Platte Valley
Hospital service area defined by 11 census tracts outlined in item d.
Objective 2. By September 30, 2025, about 275 residents will be eligible and receive a $5 reward
incentive either virtually or by mail after completing the Weld 2025 Community Health Survey.
Measurement: Count of rewards redeemed by residents from the defined service area
by September 30, 2025.
Objective 3. By December 31, 2025, the Platte Valley Hospital Community Investment Program
will receive a CHIP Grant Recipient Final Report for the project that includes results from the
CHIP grant and survey findings for the Platte Valley Hospital Service Area.
Measurement: Documentation of email and reports delivered to Platte Valley Hospital
CHIP program by December 31, 2025.

h. How project will continue to be funded if needed
The Weld Community Health Survey is conducted every three years. Funds for the Platte Valley
Hospital service area incentive program are only needed in Spring 2025. The need for funding for
incentives for the next assessment cycle, which is in 2028, will be re-evaluated in 2027. If
appropriate, funds will be requested again.
