Dear Kari,
It is with honor that we express Weld County's support for Northeast Health Partners (NHP) in their proposal to
continue in the role as the Regional Accountable Entity (RAE) in Phase Ill of the Accountable Care Collaborative
with Colorado Health Care Policy & Financing.
NHP has been a consistent and committed partner with our County Departments in our work with shared members.
They have been responsive when there is an emergent need, particularly around complex cases involving youth. NHP,
our County Departments, and the community mental health provider have worked collaboratively as well to be more
proactive by meeting regularly to discuss complex cases and how to best serve members.
NHP worked diligently with our Human Services Department to maneuver through the process for Qualified
Residential Treatment Program placements. The result of this work is that high level placements are now well defined
and easier to navigate for the families we serve. NHP is receptive to conversations with our County and is quick to
initiate and/or respond to any communication. This is greatly appreciated as frequently the issues needing resolved
are on complex emergency situations.
NHP has been a proven partner, placing people and community as top priorities. NHP has worked diligently to be a
more inclusive and responsive in planning, regardless of the circumstance.
We encourage the Colorado Department of Health Care Policy and Financing and evaluators to consider NHP as a
leading local community partner as they review what is best for our communities in ACC Phase III.
