Home My WebLink About20241390.tiffRESOLUTION RE: APPROVE AMENDMENT #3 TO INTERGOVERNMENTAL AGREEMENT FOR AGING AND DISABILITY RESOURCES FOR COLORADO (ADRC) TRANSITION -RELATED REFERRALS AND OPTIONS COUNSELING, AND AUTHORIZE CHAIR TO SIGN AND SUBMIT ELECTRONICALLY WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with Amendment #3 to the Intergovernmental Agreement for Aging and Disability Resources for Colorado (ADRC) Transition -Related Referrals and Options Counseling between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and the Colorado Department of Health Care Policy and Financing, commencing July 1, 2019, and ending June 30, 2025, with further terms and conditions being as stated in said amendment, and WHEREAS, after review, the Board deems it advisable to approve said amendment, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that Amendment #3 to the Intergovernmental Agreement for Aging and Disability Resources for Colorado (ADRC) Transition -Related Referrals and Options Counseling between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and the Colorado Department of Health Care Policy and Financing, be, and hereby is, approved. The purpose of this agreement is to expand the capacity of Aging and Disability Resources for Colorado (ADRC) to enable an ADRC Site to respond to Minimum Data Set (M DS) 3.0 Section Q Referrals, and to provide Options Counseling. Options Counseling will be provided to individuals in nursing homes, Intermediate Care Facilities or Regional Centers who have formally indicated, through the process, a desire to leave the facility and relocate to a community setting. HCPF is now amending this IGA for a third time to make the following changes: • Extend the term date through June 30, 2025. • Update the current contract maximum to include State Fiscal Funding of $16,510.23 • Amend Exhibit C-2, Rates. What options exist for the Board? • Approval of the HCPF ADRC Contract Amendment #3 . • Deny approval of the HCPF ADRC Contract Amendment #3. Consequences: Weld County Department of Human Services will not have a current agreement in place with HCPF. Impacts: The Weld County Department of Human Services will not be able to provide Options Counseling to County clients.
