ACCEPT CONTRACT AS SUBMITTED BY
ADDISON AND GLENN, INC.;
WELD COUNTY NURSING HOME ADDITION:
WHEREAS, Addison and Glenn, Inc. of Greeley, Colorado did submit a
contract for an addition to the Weld County Nursing Home in the amount of
Four Thousand Three Hundred Twelve and no/100 Dollars ($4,312.00), and
WHEREAS, the Board of County Commissioners believes it to be in the
best interest of the County to accept said contract.
NOW, THEREFORE, BE IT RESOLVED, by the Board of County Commissioners
of Weld County, Colorado that the contract submitted by Addison and Glenn, Inc.
for the construction of an addition to the Weld County Nursing Home in the
amount of $4,312.00, be and the same is hereby accepted and signed.
The above and foregoing resolution was, on motion duly made and
seconded, adopted by the following vote:

Dear Sir:
The undersigned proposes to furnish all materials and perform all labor necessary to complete the following:
All that shown on Plan No. W -C 1i* dated 4-2-66.
This includes a finished job, whether shown or implied on plans.
We further purpose to start the job within 15 days, and finish no later
than 75 days after notified by Owner to start work.
All of the above work to be completed in a substaptial and workmanlike manner for the sum of Four
Thousand Three Hundred Twelve and no/100 0,312.00 ) Dollars

per cent of all work completed. The entire amount of contract to be paid within 15 'lays after completion.
Any alteration or deviation from the above specifications involving extra cost of material or labor will only be executed

must be made in writing.
The Contractor agrees to carry Workmen's Coatpcasa:i ::: and Pabi.,; Liability Insurance, also to pay all Sales Taxes,
Old Age Benefit and Unemployment Compensation Taxes upon the zaaterial and labor furnished under this contract, as
required by the United States Government and the State in which this work is performed.
