NATIONAL DIABETES MONTH - NOVEMBER 2024
WORLD DIABETES DAY - NOVEMBER 14, 2024
WHEREAS, diabetes is a significant public health challenge affecting Weld County, where
one in nine residents have been diagnosed with type 2 diabetes, and the Centers for
Disease Control and Prevention (CDC) estimates that one in three U.S. adults is pre -
diabetic; and
WHEREAS, lifestyle change interventions are proven to reduce the risk of developing type
2 diabetes, and research shows that individuals who participate in a structured lifestyle
program can reduce their risk of developing type 2 diabetes by 16 percentage points per
year; and
WHEREAS, Weld County is it proud to provide the Heart of Weld, part of the CDC -led
National Diabetes Prevention Program, which empowers individuals with prediabetes to
make lasting, healthy lifestyle changes to delay or prevent the onset of type 2 diabetes;
WHEREAS, the Heart of Weld program offers participants the support of a trained lifestyle
coach and a network of peers working toward similar goals, equipping participants with
skills to at healthy, stay active, manage stress, and address everyday challenges to long-
term health success; and
WHEREAS, November is National Diabetes Month, and November 14th is World Diabetes
Day, recognized in 160 countries, we acknowledge the importance of prevention,
education, and community support in combatting this disease.
NOW, THEREFORE, BE IT PROCLAIMED, by the Board of Weld County Commissioners
that November 2024 be known as National Diabetes Month and November 14, 2024, be
known as World Diabetes Day in Weld County and encourage those at risk to explore
programs like Heart of Weld, which offer valuable tools for a healthier future.
