Attached please find Option Letter #2 for the County Incentive Program Intergovernmental Agreement
(Contract# 2021CMIP063), known to the Board as Tyler# 2020-2389. This Option Letter extends the term
dates to June 30, 2025. We are not receiving any additional funding and no signature is required.

We received the attached Option Letter from the state regarding the County Incentives Program. WCDHS first entered
into this contract in 2020, it is known to the Board as Tyler ID: 2020-2389. In July of 2021 an Amendment #1 was entered
into to extend the term dates, increase the contract maximum amount, modify the scope of work, and add Exhibit D.
This Amendment #1 is known to the Board as Tyler ID: 2021-2198. In February 2023 Amendment #2, known to the Board
as Tyler ID: 2023-0345, was entered into to increase the contract amount and update performance incentives. The State
has now issued an Option Letter #1 to the Amendment #2 to extend the term date to June 30,2024.
This Option Letter does not require a county signature and will be in effect as of July 1, 2023.
