and ending on the current contract expiration date shown above, at the rates stated in the Original Contract, as
amended.
B. In accordance with Page 1, Paragraph Option on the Contract's Signature and Cover Page of the Original
Contract referenced above, the State hereby exercises its option to increase the quantity of the Services for
SFY2024 at the rates stated in the Original Contract, as amended.
C. The State Fiscal Year (SFY) 2024 funds are increased in the amount of $36,212.32. The Contract Maximum
Amount table on the Contract's Signature and Cover Page is hereby deleted and replaced with the Current
Contract Maximum Amount table shown above.
3. OPTION EFFECTIVE DATE:
A. The effective date of this Option Letter is upon approval of the State Controller or July 1, 2023, whichever is
later.
4. The attached Revised Exhibit C-3, "Budget", replaces previous Exhibit C-2, "Budget", which is attached and
incorporated by reference, explains the changes made by the additional funding added to this contract period.

Federal Financial Participation (FFP) Intergovernmental Agreement from Colorado Department of Health Care
Policy and Finance (Tyler #2020-3151). This Option Letter is extending the term date to June 30, 2023 and
increasing funding to $36,212.32. This has been entered into CMS as #6021 for tracking purposes only.
