RE: APPROVE ACCEPTANCE OF COLLABORATIVE MANAGEMENT PROGRAM (CMP)
FUNDING AWARD ATTESTATION STATEMENT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, on May 1, 2024, the Board of County Commissioners of Weld County,
Colorado, approved Resolution #2024-1057 for Memorandum of Understanding for the
Collaborative Management Program (CMP) between the Department of Human Services and
various agencies, and
WHEREAS, the Board has been presented with a Collaborative Management Program
Funding Award Attestation Statement from the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, to the Colorado Department of Human Services, Office of Children, Youth and Families,
Division of Child Welfare, with further terms and conditions being as stated in said attestation
statement, and
WHEREAS, after review, the Board deems it advisable to accept said funds and approve
said attestation statement, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Collaborative Management Program Funding Award Attestation
Statement from the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, to the Colorado
Department of Human Services, Office of Children, Youth and Families, Division of Child Welfare,
be, and hereby is, approved and accepted.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said attestation statement.

Brief description of the problem/issue: Weld County, along with community partners, entered into a CMP
MOU on May 1, 2024, known as Tyler#2024-1057, which outlines a collaborative approach to the delivery of
services to children and families that will result in a more appropriate and effective service delivery.
The Department has now received notice of the acceptance of the CMP MOU for SFY 2024-2025 and has been
awarded funding.
What options exist for the Board?
Approval of the CMP Funding Award Attestation Statement.
Deny approval of the CMP Funding Award Attestation Statement.
Consequences: Funding will not be received for the CMP.
Impacts: Weld County Department of Human Services, will not be in compliance with
(C.R.5) Section 24-1.9-102(1)(a) for the purpose of promoting a collaborative
system of local level interagency oversight groups.
Costs (Current Fiscal Year / Ongoing or Subsequent Fiscal Years):
Final funding is yet to be determined but will be no less than $200,000.00.
Recommendation:
• Approval of the Collaborative Management Program Funding Award Attestation Statement
and authorize the Chair.

Dear Director Ulrich,
The purpose of this letter is to communicate information about your county's
participation in the Collaborative Management Program (CMP), C.R.S. 24-1.9-101, also
known as HB 04-1451.
The Weld County CMP Memorandum of Understanding (MOU) for SFY 2024-2025 has
been accepted. As specified in Section X of your county's MOU, Weld County needs to
submit the attached attestation statement with signatures prior to receiving the CMP
funds.
The Colorado Department of Human Services appreciates Weld County's continued
commitment to the coordination and integration of services to children, youth, and
families in their community.

In accepting the funding award for Weld County, I, the County Commissioner(s) and/or
County Director, hereby attest that the Collaborative Management Program will:
• Track clients served in all target populations listed in the CMP site MOU;
• Agree to not duplicating clients in the approved databases (Trails and/or Efforts to
Outcome (ETO)); and,
• Comply with Operation Memorandum OM-DCP-2024-0001.
