Home My WebLink About20241294.tiffRESOLUTION RE: APPROVE CONSENT TO ASSIGNMENT AND ASSUMPTION OF PURCHASE OF SERVICE AGREEMENT FOR OLDER AMERICANS ACT IN -HOME SERVICES FROM MILE HIGH SENIORS, INC., DBA HOME HELPERS, TO COLORADO DOUBLE H, LLC, DBA HOME HELPERS HOME CARE OF LONGMONT, AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, on the 18th day of March, 2020, the Board of County Commissioners approved staff's recommendations for Bid #62000079 for Older Americans Act In -Home Services, and WHEREAS, on the 13th day of May, 2020, the Board of County Commissioners approved the Purchase of Service Agreement for Older Americans Act In -Home Services between the Department of Human Services and Mile High Seniors, Inc., dba Home Helpers, and WHEREAS, Mile High Seniors, Inc., dba Home Helpers, desires to assign the current Purchase of Service Agreement for Older Americans Act In -Home Services to the new shareholders of Colorado Double H, LLC, dba Home Helpers Home Care of Longmont, and WHEREAS, the Board has been presented with a Consent to Assignment and Assumption of Purchase of Service Agreement for Older Americans Act In -Home Services among the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Mile High Seniors, Inc., dba Home Helpers, and Colorado Double H, LLC, dba Home Helpers Home Care of Longmont, commencing May 1, 2024, with further terms and conditions being as stated in said agreement, and WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy of which is attached hereto and incorporated herein by reference. Home Helpers is a provider of In -Home Services for older adults through the Area Agency on Aging (AAA) division, this Agreement is known as Tyler ID 2020-1456. Mile High Seniors, Inc dba Home Helpers was assumed by Colorado Double H LLC dba Home Helpers Home Care of Longmont. This Assumption Agreement transitions all services and responsibilities of the original agreement to the new owners of the company. What options exist for the Board? Approval of the Assumption of Purchase of Service Agreement with Colorado Double H LLC dba Home Helpers Home Care of Longmont. Deny approval of the Assumption of Purchase of Service Agreement with Colorado Double H LLC dba Home Helpers Home Care of Longmont. Consequences: AAA will not have a current contract in place with this vendor for In -Home Services for our older adult population.
