HomeMy WebLinkAbout20240198.tiffRESOLUTION RE: APPROVE AGENCY PARTNER MEMORANDUM AND AUTHORIZE CHAIR TO SIGN - A PRECIOUS CHILD WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with an Agency Partner Memorandum between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and A Precious Child, commencing upon full execution of signatures, and ending January 31, 2025, with further terms and conditions being as stated in said memorandum, and WHEREAS, after review, the Board deems it advisable to approve said memorandum, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Agency Partner Memorandum between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and A Precious Child, be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said memorandum. A Precious Child assists children and families dealing with abuse, neglect, crises, and poverty through partnerships with agencies throughout several counties, including Weld. The purpose of this MOU is to outline the parameters for WCDHS clients to access services provided through A Precious Child's programs and services. The Board entered into a prior MOU with A Precious Child on March 24, 2021 which is in perpetuity. This request will supersede the current MOU. What options exist for the Board? • Approval of the MOU between WCDHS and A Precious Child. • Deny approval of the MOU between WCDHS and A Precious Child. Consequences: WCDHS will not have an MOU in place with A Precious Child. Impacts: WCDHS will not be able to refer clients to A Precious Child for the services they provide. CO 800201 303.466.4272 I AgencyPartners@APreciousChild.org Agency Partner Memorandum between A Precious Child and Weld County Department of Human Services This Memorandum of Understanding ("MOU") outlines the parameters for accessing programs services provided by A Precious Child. A Precious Child will meet the following criteria: 1) A Precious Child does not discriminate on the basis of race, color, sex, gender identity, veteran status. religion, disability, sexual orientation, marital status, ancestry or national origin_ 2) A Precious Child will treat each and every client and Agency Partner with dignity and respect. 3) A Precious Child will provide vetted Agency Partners with access to the A Precious Child s Agency Partner Portal to submit electronic referrals on behalf of their clients who can then shop at A Precious Child's Empowerment Resource Center for resources and services free of charge, based on availability and funding. Agency Partner Representatives can also shop on behalf of their clients at A Precious Child's Empowerment Resource Center with an appointment. Resources and services can include the following: a Agency Partner onboarding and support b Client resource navigation services c; Clothing, coats, toiletries, home goods and other essentials; including bulk distribution of these items directly to Agency Partners based upon available supplies d Opportunity funds for sports, arts and educational assistance e Seasonal distribution of school supplies, gifts for the holidays and sporting equipment f) Mentorship gi Workforce readiness By signing :his memorandum, the Agency Partner confirms their Agency currently meets and will continue to meet the following criteria: 1) The Ageicy does not discriminate on the basis of race, color, sex gender identity, veteran status, religion, disability, sexual orientation, marital status, ancestry or national origin. 2) The Ageicy treats all their clients and A Precious Child staff, volunteers and stakeholders with digrity and respect. -a precious child a EMPOWERING KIDS TO SUCCEED APreciousChild.org 17051 West 118th Ave. Broomfield, CO 80020 1303.466.4272 j AgencyPartners@APreciousChild.org 3) Agency has a process in place for ensuring they are properly qualifying clients that meet A Precious Child's eligibility criteria. a) Service Eligibility Criteria: (1) Have primary residence in A Precious Child's 8 -county service area: Adams, Arapahoe, Broomfield, Boulder, Denver, Douglas, Jefferson and Weld. (2) Be a parent, primary caregiver or foster parent of a child(ren), 18 years old and younger. Unborn babies may also be added to the referral voucher once the mother is in their second trimester of pregnancy. (a) Any disabled adult children, 19-21 years of age who live at primary caregivers home, can also be added to the referral voucher with proof of disability as defined by the American Disabilities Act. And meet one of the two criteria below: (1) Families whose income equates to 300% and below of the Federal Poverty Level. Examples that can provide proof of this to the Agency Partner are as follows: (a) Enrolled in Medicaid or CHIP (b) Qualify for free or reduced lunch (c) Qualify for government assistance (SNAP, TANF, WIC, CCAP, etc.) (d) Paystub (Please refer to 2024 Federal Poverty Level Guidelines: https//www.healthinformatics.dphe_state.co us/NonAuthenticated/FPLCALC/) (2) Are in need of emergency support due to experiencing house fire, flooding, domestic abuse, or Asylee Status/Refugee Status 4) The Agency must complete an onboarding process, which includes: a) An Agency Partner Application (valid for 5 years) b) The completion of an orientation by at least one Agency representative in charge of training their staff on A Precious Child's Referral Process c) Signed Partnership Memorandum of Understanding (valid until January of following year signed) 5) Good Standing The Agency must maintain status of Good Standing to continue to refer clients and access programs and services. To maintain their Good Standing status the Agency commits to the following: a) Participation at A Precious Child's Annual Agency Partner meeting (attendance required) b) Annual signature of Memorandum of Understanding occurring in January of every year c) Annual completion of Agency Partrer Survey and ad hoc surveys d) Completion of data requirements within 15 business days of receiving data request e) Highlighting collaboration with A Precious Child on the Agencies social media, newsletter, annual report and media attention that involves resources, services and goods that A Precious Child helped provide t'a precious child i EMPOWERING KIDS TO SUCCEED APreciousChild.crg J 7051 West 118En Ave. Broomfield, CO 80020 I 303.466 4272 I AgencyPartners@APreciousChild.org 6) Probatioi Period If the Agency does not maintain their commitments they will be placed in a probation period of 30 days to rectify their good standing status and in that time they will not be able to refer clients though the Agency Partner Portal nor receive goods, distributions and services. The deactivat on will remain in effect until requests are completed and a determination is made by A Precious Child to reinstate the partnership. If the Agency reobtains their good standing status, they will have access to programs and services again. If the Agency enters this probatior period more than two times, and does not meet certain requirements or criteria for "good standing" within the given timeframe, the partnership may be terminated completely. 7) The Agency commits to distributing A Precious Child's Referral Vouchers exclusively to its clients ar d only through our designated Agency Partner Portal. The Agency will refrain from using invalid vouchers or outdated versions of vouchers. 8) The Agency will assign a designated individual or team with the responsibility of communicating updates to A Precious Child regarding changes in staff members who require access to our Agency Partner Portal. These updates may include the addition of new memberE or the departure of existing ones. We kindly request that these updates be communicated promptly to ensure our records remain current, thereby facilitating effective collaboration. We will require each new member's first name, last name, phone number, position, and email address. This information is crucial for maintaining security and efficiency in our operations. 9) If applicable, the Agency agrees to accept client referrals from A Precious Child for services provided by their Agency. 10) If applicable, the Agency is responsible for informing A Precious Child if they stop accepting referrals from A Precious Child 11) The Agency agrees not to sell goods received from A Precious Child for monetary or any other kin d of profit. 12) The Agency will recommend A Precious Child as a place to donate in -kind goods. 13) The Agency agrees to communicate feedback or concerns to AgencyF-artners@APreciousChild.org. Terminatior of MOU This MOU becomes effective upon execution date of all parties and will remain in effect until January of the following year unless sooner terminated by either of the following reasons: 1) Witten notice by one party to the others; 2) Upon mutual consent of all parties, 3) Upon good cause of either Agency if the other party fails to comply with the terms of the MOU. A Precious Child reserves the right to suspend or cancel partnerships under conditions including but not limited to: a) Failure to abide by this Partnership Memorandum of Understanding; b) Loss of 501(c)(3) status; c) Abuse of the referral process; d) Mistreatment of clients, staff and volunteers: e) Not recognizing A Precious Child for their support in media communications, if applicable; f) Misuse of A Precious Child's name, etc The Memorandum of Understanding (MOU) is required to be signed by the Agency's CEO. Executive Director, Principal or similar position and it will be signed by the Chief Operating Officer at A Precious Child. The MOU is valid until the following January Each January, the Agency will need to renew the MOU if they wish to continue the partnership.
