RE: APPROVE REVISIONS TO FEE SCHEDULE FOR FEES COLLECTED BY WELD
COUNTY DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with revisions to the fee schedule for fees
collected by the Weld County Department of Public Health and Environment, and
WHEREAS, after study and review, the Board deems it advisable to approve the proposed
revisions, effective January 1, 2025, copies of which are attached hereto and incorporated herein
by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the revised fee schedule for fees collected by the Weld County Department
of Public Health and Environment, as attached hereto, be, and hereby are, approved, effective
January 1, 2025.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 18th day of November, A.D., 2024.

SUBJECT: 2024 Fee Schedules for Environmental and
Public Health Divisions
For the Board's review and approval are the Health Department proposed fee changes for 2025.
Program fees are evaluated annually with cost recovery being the primary goal. Most
Environmental Health related fees are based on an hourly rate that represents the cost to the
County. We propose increasing the hourly rate for Environmental Health Services from $90 to
$100 in 2025. All fees based on the hourly rate will be modified accordingly if the increase is
approved by the Board.
The sliding fee scale for Public Health Services will remain in place. This scale aims to recover
cost plus 15% when possible, yet it allows for reduced fees for clients who are unable to pay the
full cost of services. Grant funding generally makes up the difference for clients unable to pay
full rates. We propose that the cost-plus scale remain in place for 2025 with adjustments to the
actual costs of medical supplies and vaccines.
Health Department staff met with the Board in a work session on November 5, 2024 to discuss
proposed fee changes and the Board approved placing the proposed fees on an agenda for formal
consideration.
I recommend approval of the proposed changes to the 2025 fee schedule.

Guiding Principles for Setting Clinic Fees
➢ The majority of our patients come to us because they have Medicaid, are under -insured or have no health
insurance
➢ It is our mission to be a safety net for our community members and reduce barriers to service, including
costs
➢ Aim to recoup the maximum reimbursement from Medicaid
➢ For those that are under -insured and uninsured, we seek grants to eliminate cost barriers
➢ Total Clinic Grants for 2024 - $1,079,322

2024 Highlights
Thru 09/30/24 our overall collections are up
19% compared to 2023
Collections posted vs. Charges posted is at 58%
for 2024 (industry avg. is 30-50%)
2023 Collections vs. Charges was 39.5%
• Code 1 will slide to $0 for Family Planning, STI and Medications
covered by grants. All others will slide to 50% of Code 3 with a
minimum of $3
• Code 2 will be set at 75% of Code 3
• Code 3 will be our starting point. It will be set at Medicaid max
reimbursement or cost +15%, whichever is greater
• Code 4 will be set at Medicaid x 135% (2024 - 125%)
• Code 5 will be set at Medicaid x 225% (2024 — 270%)
• Private Insurance - We are out of network with all private insurances;
however, we are still able to bill through our EMR. Private Insurance
rates will be set at 300% of Medicaid. There is an industry practice of
paying a percentage of what is billed for any out -of -network
providers. Each company has a different percentage, which they don't
divulge. Therefore, standard billing practices is to bill at a higher
amount in hopes of getting our costs covered.
Self -Pay codes are determined by a client's gross family income and family size. Weld County will
use the CDPHE income levels from the Family Planning Title X grant.
