Accounts Management Operations Make- Wish COLORADO Make -A -Wish® Colorado 7951 E Maplewood Ave #126 Greenwood Village, CO 80111 303-750-9474 Dear Friends of Make -A -Wish Colorado, Make -A -Wish Colorado creates life -changing wishes for children with critical illnesses and has been doing so since 1983. To date, more than 6,000 children from our community have been able to imagine and experience the transformative power of a wish. We believe wishes have a powerful impact in the lives of wish children and their families during some of their most difficult trials. In the fight against a critical illness, each wish creates an opportunity for hope and the ability for a child to experience life beyond their illness. Research shows that wishes can help improve a child's quality of life and produce better health outcomes with less time spent in the hospital. Right now, our need is greater than ever before. With a record number of children waiting for their wish to be granted, about 500, your support will help ensure a wish is granted for every eligible child as quickly as possible. To achieve this goal, The Wish Ride of Colorado is hosting an event this June 2024 in their community. Their goal is to grant wishes of local children with critical illnesses in their community. Any help and support you can provide in their efforts will be greatly appreciated. Together, we can bring hope, joy and transformation to Colorado families. If you have any questions, please feel free to call me at 720-233-0343 for further information. Thanks in advance for your support! I also wanted to let you know that I personally went and purchased 1800 ft of Caution Tape to put around the property for the alcohol boundaries. I don't want to take any chances. Thank you again for everything you have done to help. We, the entire Wish Ride board, is so appreciative of everything that you have done to help us in our first year of doing this. I am going to figure out how you did this and work with it for next year so you or anyone else will not have to help. This map is complete and accurate to my knowledge. I will make prints of this and give to every board member and to Anderson Farms so everyone is aware of the perimeters and boundaries. I will have signage up and posted to help aid our efforts of keeping this event on the up and up with Weld County, Anderson Farms, and the State of Colorado. This is mandatory, so someone from Anderson Farms will have to provide you with these or someone from the Wish Ride will have to measure themselves and ensure that what is roped off the day of the event is accurate. Ultimately, it's the Wish Ride who is responsible for the map and permit. Thank you for updating the questionnaire and form. Hopefully you'll get the information about the portapotties on Thursday and can add that to the questionnaire too. On Monday, I've available between 10:00 a.m. and 3:30 p.m. What time works best for you? And you don't have to thank me, I'm more than happy to help. Learning the process the first year is always a little difficult, but once you've put together a complete application once, it's much easier to just duplicate for the next year. II Best, Chloe A. I received your email and am working hard to get you the answers that you need. I do not have the perimeters of Anderson Farms and I have been trying to reach them since last week with the initial questionnaire and have not heard anything back. How would you suggest getting the information that I am needing since I am unable to reach them? I have attached the information that was requested in #12 and again, I have no say as to what Anderson Farms says we can and can't use. They have us the information as to the area that we are going to be using and they said that they would put up rope to show where the boundaries are. Other than that, I am just waiting on the Port -a -potties contract and have asked for that by Thursday at the latest. I did take next Monday and Tuesday off of work so hopefully this will be completed and I can come see you and take care of the rest of the business. One thing I will say is next year will be a lot different and I will understand all this better. Thank you again Chloe for everything you are doing to help us. We appreciate it and without you, we wouldn't be where we are now. 1 Thank you, Tabitha From: Chloe White <cwhite@weld.gov> Sent: Tuesday, April 16, 2024 2:00 PM To: Keagle, Tabitha <flingt@eas.enersys.com> Cc: Marketing for The Wish Ride <marketing@thewishride.com> Subject: RE: Special Event Permit CAUTION: This email originated from outside of the Company. Weld County Questionnaire: 1. According to the attached Certificate of Good Standing, the name is just `Wish Ride of Colorado' (no `the'). I guess that the word THE did not make it into the Certificate of Fact of Good Standing. I will look into this and see what our Admin filed with the state. Our Website and emails are all The Wish Ride. I have changed that. Thank you for making the applications match what the State currently has on file. If you end up correcting the name with the State, we can update the liquor forms and information too. c 2. Question #3: Will all proceeds benefit Make -A -Wish Foundation of Colorado or will the Wish Ride of Colorado receive any proceeds? ALL Proceeds benefit Make -a -Wish. No one that volunteers or that is on the board gets paid to do what we are doing. If we took money for that then it would be taking money away from the kids. Thank you for the clarification. Please add this information to the questionnaire. Updated 3. Questions #14 and #15: Who is preparing/making the food? As to questions #14 & #15 — The food is being prepared by members of the Longmont Moose Lodge in their kitchen and will be transported to Anderson Farms to be served by Make -a -Wish volunteers and the Boy Scouts. Thank you — please add this information to the questionnaire for the Department of Public Health and Environment's review. Updated 4. Question #17: How many portapotties and hand washing stations? Who is the company you've hired? Where will the portapotties and hand washing stations be placed/located (see request listed below under `back-up documentation'). We have not hired a Port -a -potty company as they have graciously donated them to us. I do not have the contract with me and I should be able to get that information this weekend. I can see if I can get a copy of that for you. As far as to where they will be located, I have attached a copy of MAP #1 to include the port -a -potties. Thank you for the clarification and for adding the location to the map. The donation is fine, but we will need a copy of the contract. Please add the number of portapotties and hand washing stations, and the name of the company you've hired to the questionnaire. Awaiting for the contract and the name of the company. I have asked that they have this by Thursday. State Form: 5. Please use the attached updated State form (this is their new version). I did use the new form from the state that you had provided. So I am unaware of what is needed there. I saw in your other email that you submitted the new State form — thank you! QQ • On the new form, under Question #2, you indicated the Wish Ride has been issued a Special Event Permit already this calendar year. If you meant to select 'No' instead, can you please update this? Updated 6. On the State form, you indicated that the Wish Ride of Colorado is a national organization? Is that accurate or is it only in Colorado? If it is not a national organization, please make another selection as appropriate, such as philanthropic organization, for example. Our organization is only in the State of Colorado. I took the same information that was sent to me and just copied it. (I had been given copies of what had been previously filled out for this event when it was Franks Ride). I know what we are not Philanthropic Institution, so I am just choosing Social as nothing else fits us. If that is what you felt is the right selection for your organization, I don't have any further questions. Thank you! 7. According to the attached Certificate of Good Standing, the name is just 'Wish Ride of Colorado' (no 'the'). I have fixed the name of The Wish Ride to Wish Ride on the forms. Thank you! 8. Please swap your name and information and Mr. Blackford's. I believe you meant to indicate him as the authorized representative and you as the event manager because that is how you listed the names on the County questionnaire. I see that you corrected this on the new form. Thank you! U 9. Please indicate the time is from 8:00 a.m. to 8:00 p.m., which is what you entered on the County questionnaire. The time has been fixed. I just used military time. Thank you for the clarification and update. Back -Up Documentation: 10. On your diagram, will alcohol be allowed in section #1? If not, please update the description on the side of your diagram to show 'all areas beer/liquor allowed (encompasses all but #3, #2, and #1)'. I have corrected the area as to where alcohol will be allowed. The diagram now shows "#8 all area beer/liquor allowed (encompasses all but areas 1,2,3) Thank you for the update. 11. Please provide a copy of your receipt/contract with the company providing portapotties and hand washing stations. Again, I do not have a copy of that as someone else is handling that and I should be able to get that this weekend, if not next week. Great, thank you. Please send me a copy of the contract when you have it. Waiting on contract and name of company that is donating Port -a -Potties and I will send that over ASAP as I have asked for it by Thursday. 12. How will the different sections on your diagram be marked? I.e. fence, rope barriers, etc. As far as the different sections of the diagram be marked, Anderson Farms takes care of the main perimeter as to where we can and can't go. We will post signs at the entrance and in the kids area regarding alcohol. I just drew a rough estimate as to where Anderson Farms stated that they will be putting up rope to show boundaries. The State requires you to 'submit evidence of intended control, i.e. fencing, ropes, barriers, etc.' If section #8 on the map will be roped off to show the boundaries, please indicate that on the map. Also, the State will require the diagram to be accurate. In the event that Liquor Enforcement comes to the event, the map will need to match what is physically out there, so you'll want to be sure Anderson Farms has a copy of the final map when they set-up the event. I have updated the best to my ability. Wish Ride of Colorado has no say over the boundaries that Anderson Farms sets and I cannot reach anyone out there to ask for exact perimeter. 10 13. On your diagram, please include dimensions of the boundaries. As far as the dimensions of the boundaries, I do not have those calculations as Anderson Farms is putting up the boundaries. This is their property and we do not set those boundaries. The State and County require dimensions on the map. If I cannot get in touch with Anderson Farms, how do I go about getting dimensions? 14. I generated a Certificate of Good Standing for Wish Ride of Colorado and attached it to this email for you. Thank you for generating a Certificate of Good Standing. No problem at all. Happy to help. Once you have the above information updated and the additional items ready, please let me know and we can coordinate a time for submittal. If it is not a national organization, please make another selection as appropriate, such as philanthropic organization, for example. • According to the attached Certificate of Good Standing, the name is just 'Wish Ride of Colorado' (no 'the'). • Please swap your name and information and Mr. Blackford's. I believe you meant to indicate him as the authorized representative and you as the event manager because that is how you listed the names on the County questionnaire. • Please indicate the time is from 8:00 a.m. to 8:00 p.m., which is what you entered on the County questionnaire. Back -Up Documentation: • On your diagram, will alcohol be allowed in section #1? If not, please update the description on the side of your diagram to show 'all areas beer/liquor allowed (encompasses all but #3, #2, and #1)'. • Please provide a copy of your receipt/contract with the company providing portapotties and hand washing stations. • How will the different sections on your diagram be marked? I.e. fence, rope barriers, etc. • On your diagram, please include dimensions of the boundaries. • I generated a Certificate of Good Standing for Wish Ride of Colorado and attached it to this email for you. Please let me know if you have any questions. Thank you, Chloe A. I wanted to send this to you to see if there is anything that I will need to change. I am also including the Liquor License paperwork as well. Please let me know if I will need to change anything as I am out of the office on Wednesday down in CO Springs at a Space Convention for work. I appreciate your help, your time and your efforts in helping us continue granting wishes for children with critical illnesses. I will have all of this filled out sent over to you early next week and I will bring the originals with me on Friday to our appointment at 10:30. Just so I am clear, there will be 2 checks written: 1 in the amount of $100 payable to Weld County and 1 in the amount of $100 payable to Colorado Department of Revenue? And then there is a $25 for Malt. Vinous and Spirituous Liquor (found on DR 8439)? Where and who would we make that check out to? I have to make sure of these things as I do not hold or write checks for The Wish Ride, those come from the Administrator and she will be out of town all next week so I want to make sure before I meet with you.
