PROGRAM FUNDS FOR NORTHERN COLORADO REGIONAL ASSESSMENT
COLLABORATIVE PARTNERSHIP AND AUTHORIZE DEPARTMENT OF PUBLIC
HEALTH AND ENVIRONMENT TO SUBMIT ELECTRONICALLY
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Mini -Grant Application for Preventative
Block Grant (PBG) Program Funds for the Northern Colorado Regional Assessment Collaborative
Partnership from the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Public Health and Environment,
to the Colorado Department of Public Health and Environment, Office of Public Health Practice,
Planning, and Local Partnerships, with further terms and conditions being as stated in said
application, and
WHEREAS, after review, the Board deems it advisable to approve said application, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Mini -Grant Application for Preventative Block Grant (PBG)
Program Funds for the Northern Colorado Regional Assessment Collaborative Partnership from
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Public Health and Environment, to the Colorado
Department of Public Health and Environment, Office of Public Health Practice, Planning, and
Local Partnerships, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that Cindy Kronauge, Department of Public
Health and Environment, be, and hereby is, authorized to electronically submit said application.

PASS -AROUND TITLE: Preventive Block Grant to Continue Development of a Regional Data Collaborative
DEPARTMENT: Public Health and Environment DATE: August 5, 2024

Brief description of the problem/issue:
The Weld County Department of Public Health and Environment has developed a regional collaborative
of stakeholders to improve coordination around data collection and data sharing for the purposes of
alignment among partners on key public health priorities, strategies, and indicators of progress. The
department received a 35K preventive block grant from the Office of Public Health Practice, Planning,
and Local Partnerships (OPHP) in the 2023-2024 budget cycle to start this collaborative, and now seeks
to apply for $50,000 in the 2024-2025 budget cycle to continue this work. With the first award we
utilized an external contractor to help convene stakeholders and to facilitate meetings among the
newly formed Northern Colorado regional assessment collaborative that met ten times in 2024. The
purpose of those meetings was to identify overarching goals that have been turned into an initial two-
year work plan. The objectives of the two-year plan are to:
a. improve coordination around data collection,
b. develop a shared regional repository for local assessment data and reports,
c. identify common population indicators of progress,
d. engage in strategic planning for assessment activities and initiatives, and
e. better align our state mandated Community Health Improvement Plan (CHIP) priorities
If successful, the new funding will be used to continue this work. The partnership will now work
together on implementing strategies and activities around shared health priorities driven by the data
that is collected and shared.
What options exist for the Board? •
The Board can choose to approve, or not approve, this request.
Consequences:
Approval of this request will permit WCDPHE to pursue continued funding to further develop and
solidify the work of the regional collaborative.
Impacts:
Declining the request to pursue this grant award may impede the department's progress in developing
the collaborative, which then impedes data collection and data sharing among regional partners.
Costs (Current Fiscal Year / Ongoing or Subsequent Fiscal Years):
There is no associated county cost. Activities will be conducted by temporary staff or contractors paid
through grant funding. No additional County FTE is being requested.
Recommendation: I recommend approval to place this item on a future BOCC agenda for formal
consideration.

As we all continue to be reminded, one's health and health status does not recognize
jurisdictional or operational boundaries. Improving population health (a common purpose
of this new collaborative) regionally requires attentive and strong, multi -sector
partnerships. In July 2023, over 50 representatives from local public health agencies,
health and behavioral health care providers, and other health -related stakeholders in
Larimer and Weld counties met and agreed they wanted to advance their impact and
outreach efforts by exploring regional community health assessment, planning, and
partnership work.
In early 2024, an initial group of Northern Colorado health, behavioral health, and public
health partners began meeting regularly to begin to talk about ways to possibly better
align and collaborate on community health assessment and planning work. After lots of
discussion, the group identified five areas of focus that seemed relevant, feasible and
realistic. From those discussions, goals and objectives were developed in an initial two-
year workplan. Over the next year, the collaborative will focus on solidifying and
sustaining the collaborative by developing a governance structure and data sharing
protocols as well as begin work on two overlapping priority areas for all organizations
involved in the collaborative — behavioral health and health equity.
Establishing this regional partnership will mean everyone can have greater reach, as well
as learn from each other and share best -practices. Better aligned activities will enhance
each other's work and make it easier instead of making more work or duplicating similar
work, as well as not interfere with what we already do. Improved collaboration across our
common shared population health assessment activities is the first step in building this
new regional partnership - the NoCo Regional Assessment Collaborative.
Northern Colorado Regional Assessment Collaborative participants are ready to move
from its initial networking strategies to more coordinating and collaboration strategies in
order to move to more collective action around data and community health assessment.
19. Project Design/Methodologies: What procedures and activities will be included in
your project to achieve your goals over the next year? (15 PTS)
The primary purpose of this collaborative is to increase collaboration in our community
health assessment activities and in the utilization of our assessment results via our
community health improvement and implementation plans. This will result in more
alignment of our community health improvement and implementation plans and
increased efficiency and consistency in engaging with our communities. A secondary
goal is to have more shared and accessible data at a regional level.
Activities will include facilitating meetings of the steering committee and its workgroups
and solidifying the structure and governance of the collaborative. This would include
documenting and archiving the workings of the group, and developing a governance
framework, developing and implementing data sharing and updating protocols, and
monitoring and evaluating the activities and governance of the collaborative. Other
activities include creating a Larimer and Weld County assessment data repository,
developing and tracking a set of common health indicators for behavioral health, and
planning a regional qualitative data collection project for at an underserved special
population.
Beneficial outcomes will be the creation of a collaborative with formal membership,
assessment timelines that are better aligned, and potential cost sharing for ongoing
community health assessment data gathering processes. Some initial outcomes (beyond
this grant) will be that partners use data to better prioritize needs and problems, set
policies, refine programs and services, and leverage funds. Developing a strong regional
assessment and data collaborative will help community members and organizations take
action and ultimately lead to improved community health status.
We aim to increase partner engagement through structure, efficient meetings, agreements,
and delivery of high -quality data products (to be determined by the collaborative).
Deliverables for the grant include timely final workplans, budgets, and grant reports.
20. Feasibility: How will this funding be spent to achieve your goals? (Personnel,
project activities timeline, and budget expenditures congruent with project
description and outcomes.) (15 PTS)
This funding will support the further development of a regional assessment collaborative
to align assessment and planning efforts across the Northern Colorado region by
supporting adequate time and effort of Weld and Larimer County staff, and a
subcontractor to carry out the goals and objectives of the collaborative.
The funding of salaries is not due to any loss of state funds for these staff. Other costs
incurred for this project will be paid by Weld and/or Larimer County Government. Any
unspent funds will be returned to OPHP.
21. Assessment/Evaluation: How will assessments/data be collected and used to
demonstrate (the) degree to which outcomes are met? (15 PTS)
Once funds are received, a final workplan with SMART goals and objectives and budget
will be developed. The workplan will identify the activities, timelines, and performance
indicators, and who is responsible for each goal and objective. Once the workplan is
complete with process and outcome measures, health department staff will monitor and
document the collaborative'y work. The tentative goals of the project are: 1) to implement
a governance structure with defined responsibilities of partner organizations including
data sharing agreements by September 2025, 2) to identify and report on a common set of
behavioral health metrics to provide a consistent view of behavioral health outcomes in
the region (timeframe to be determined), and 3) to have a subset of Weld and Lorimer
County organizations collaboratively develop and implement a qualitative approach to
obtain data on a sub -population that is harder to obtain data from and to define strategies
to improve health equity in the region for this population.
Weld and Lorimer staff will communicate regularly to monitor progress on the workplan
and report the degree which outcomes were met to OPHP in reports and other
communication as needed.
