RE: APPROVE AMENDMENT #2 TO FACILITY USAGE LICENSE AGREEMENT AND
RENTAL GUIDELINES FOR AREA AGENCY ON AGING (AAA) SENIOR NUTRITION
PROGRAM (FRIENDLY FORK), AND AUTHORIZE CHAIR TO SIGN - ARCHDIOCESE
OF DENVER AND GUARDIAN ANGELS CATHOLIC CHURCH
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Amendment #2 to the Facility Usage
License Agreement and Rental Guidelines for the Area Agency on Aging (AAA) Senior Nutrition
Program (Friendly Fork) between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human Services,
the Archdiocese of Denver, and Guardian Angels Catholic Church, commencing August 31, 2024,
and ending August 30, 2025, with further terms and conditions being as stated in said amendment,
and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Amendment #2 to the Facility Usage License Agreement and Rental
Guidelines for the Area Agency on Aging (AAA) Senior Nutrition Program (Friendly Fork) between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Human Services, the Archdiocese of Denver, and
Guardian Angels Catholic Church, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.

PASS -AROUND TITLE: Facility Usage License Agreement Amendment #2 with the Archdiocese of
Denver/Guardian Angels Catholic Church.

Brief description of the problem/issue: The Department's Area Agency on Aging provides a senior lunch
program called Friendly Fork. Through this program seniors may receive free lunches at various locations
throughout Weld County. The Department currently has a Facility Usage License Agreement with the
Archdiocese of Denver/Guardian Angels Catholic Church known to the Board as Tyler ID# 2022-2505. On
September 11, 2023, the Board approved Amendment #1, known to the Board as Tyler ID# 2023-2614, which
updated the holiday schedule and extended the terms to August 31, 2023 through August 30, 2024.
The Department is requesting to enter into Amendment #2, which updates the holiday schedule and extends the
terms to August 31, 2024 through August 30, 2025.
What options exist for the Board?
Approval of Amendment #2 with the Archdiocese of Denver/Guardian Angels Catholic Church.
Deny approval of Amendment #2 with the Archdiocese of Denver/Guardian Angels Catholic Church.
Consequences: Friendly Fork meal site in Mead, Colorado will not be available as a meal site for seniors.
Impacts: Seniors participating in the Friendly Fork Program in Mead, Colorado will not have a meal site
to receive nutritious meals on Thursdays.
Costs (Current Fiscal Year / Ongoing or Subsequent Fiscal Years):
Total cost = $50.00 per week for 46 weeks ($2,300.00 total)
Funded through: Older Americans Act
Recommendation:
• Approval of the Amendment #2 and authorize the Chair to sign.

ADDENDUM to Rental Agreement from Aug. 31, 2022, known to the Board as Tyler IDM 2022-250S between
Guardian Angels Catholic Church and Weld County Department of Human Services, AAA Friendly Fork Program.
The undersigned parties agree to a weekly rental fee of $50 per meeting/week. All prior arrangements and contract
agreement remain in place.
These are currently the dates where the Senior luncheons don't take place at the Catechetical Center of Guardian
Angels for 2024/25:
