HomeMy WebLinkAbout981116.tiff RESOLUTION RE: APPROVE 1999/2001 APPLICATION FOR PREVENTIVE HEALTH AND HEALTH SERVICE BLOCK GRANT AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a 1999/2001 Application for the Preventive Health and Health Service Block Grant from the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Health Department, to the Colorado Department of Public Health and Environment, with terms and conditions being as stated in said application, and WHEREAS, after review, the Board deems it advisable to approve said application, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the 1999/2001 Application for the Preventive Health and Health Service Block Grant from the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Health Department, to the Colorado Health Data Advisory Committee be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said application. Morrison, Health Data Manager 1)HP Year 2000 Objectives to be addressed: 22 Surveillance and Data Systems - Specifically the following subsections: 22.5 Implement in all states periodic analysis and publication of data needed to measure progress toward objectives for at least ten of the priority areas of the national health objectives. 22.6 Expand in all states systems for the transfer of health information related to the national health objectives among federal,state and local agencies. 22.7 Achieve timely release of national surveillance and survey data needed by health professionals and agencies to measure progress toward the national health objectives 2)Statement of Problem: In the state of Colorado,health related outcome data,on a county,city,or census tract level has not been easily retrievable. What is available is in a variety of formats in several State agencies and divisions within agencies,making access to and utilization of the data cumbersome and time consuming,both to local health agencies and divisions within the Colorado Department of Public Health and Environment(CDPH&E). 3) Strategy for targeting the objective(s): In the last 3 years,these issues have been addressed by the Colorado Health Data Advisory Committee(CoHDAC) via the Colorado Health Information Dataset(CoHID), a project funded by a Preventive Health and Health Services Block Grant in 1996, 1997 and 1998. The electronic infrastructure has been established with the creation of a searchable dataset distributed to the local and state public health agencies on CDRom. This data is currently being ported to a secure web site. In addition to the technical aspects of distributing this data,CoHID has also created alliances with divisions of the CDPHE and local public health agencies to facilitate freer release and distribution of data summaries and record level data. The strategy involves building on the progress made at Weld County over the last 3 years and directing the program in the following manner: • Expansion of datasets through continued procurement and development of new data sources; • Address the issue of continued funding and integration of CoHID within the CDPHE. • Training of local health agency personnel in the use of the electronic infrastructure and the responsibilities that accompany it's use regarding security and confidentiality. • Continue maintenance,development and expansion of the secured web site. 4)Grant Funded FTE Assigned to Project: 2 FTE 1 Programmer/Analyst-Database Administrator-Web Developer 2 Epidemiologist/Biostatistician-Educator-Data Negotiator 5)Other Funds: $14,500.00 Denver,Weld,Jefferson,El Paso, TriCounty &Boulder Health Departments: $2500.00 each(Advisory personnel) Colorado Department of Public Health&Environment: $10,000.00 (Advisory personnel) Various other local health departments and nursing services: $2,000.00($200.00 each for 10 sessions) 6)Target Population: All 63 (64 in 1998?)counties in the state of Colorado with a total population of 3,966,169 (estimated for 1998) as represented by the local public health agencies. 7) Disparate Population: Does not apply. g8 / // (49 8)Impact objectives(3-5 years): A) Continued development and maintenance of web server technology for secure, low level data distribution to county and state public health agencies. Extention of data beyond the current sources. (HP2000-22.6) B)Achieve timely release of surveillance and survey data needed by health professionals and agencies to monitor progress toward meeting national health objectives(HP2000-22.7). C)Increase level of data literacy in local public health agencies. D)Decrease or eliminate the need for PH&HS Block grant funding. E) Provide data to users such that public health agencies or entities can aquire funding for interventions leading to measurable outcomes for analyses and evaluation. 10)Annual objectives: A) Finish web search capabilities missing from the current data collection. B)Begin the addition of Environmental Data into the dataset. C)Discover and create links to non CDPHE sources of data useful to local and state health agencies. D)Develop training materials and strategies to assist local health agencies to enable them to use available electronic infrastructure,technology and resources to their maximum benefit. 1)Create,publish and distribute to CoHID users, a handbook on the CoHID specific data and it's use in public health. E)Include data from the Colorado Hospital Association's hospital discharge data set on the CoHID web site. F)Work towardsfurther cooperation/integration of CoHID and CDPHE. This includes pursuing other funding options. G)Act as a resource for the CDPHE and other agencies in discussions on data integration,web site security,COI programming and other areas related to the expertise housed at CoHID. H)Focus on implementing a process for quick access to and reporting of specific performance measures for local health agencies. Budget: CoHID Personnel: 2.0 FTE* 104,175.00 includes benefits,insurance,etc... Over the course of the last 3 years CoHID has functioned with one person who has had to juggle all the numerous aspects of the project alone. This,while successful,has slowed considerably the progress CoHID has been able to make in achieving its goals. The proposed budget will allow for hiring an additional person whose focus would be on epidemiology,education,technical writing and public speaking. This should allow allow all aspects of the project to proceed at a much faster and more efficient pace. April 1998 Colorado State Payroll rates for positions with comparable skill requirements and position responsibilities are shown here: • Programmer/Analyst III(base level)-$42,684 yearly+ 25%fringe =$53,355 • Epidemiologist III(base level) -$40,656 yearly + 25%fringe =$50,820 Payroll expenses for skilled programmers and other computer professionals has risen beyond the figures shown for the Denver metro area and the rest of the front range. It is vital to the continued success of CoHID that competant personnel be obtained and kept. Adjustments of Weld County personnel classifications may need to be made to make room for these salary ranges in positions at the Health Department. Data purchase $2,000.00 This is to purchase the Colorado Hospital Association discharge data. Training and meeting expenses $5,000.00 Trainings are held onsite throughout Colorado. This requires a consideral amount of travel time,milage and accomodation reembursement. It may also include rental of room space and meal expenses for class attendees. The scope of the project requires a good deal of time spent meeting in various locations around the state for purposes other than training also. This includes CoHDAC meetings,negotiations on data release, security and confidentiality, presentations to interested groups and technical support of local public health CoHID users. Supplies $1,980.00 Miscellaneous supplies include blank CDRoms, Computer hardware like memory and backup tapes,paper,postage, etc. Internet Service costs $1,200.00 Connection to the Internet for the web/database server is required for CoHID to have a web presence. The current arrangement is contracted at$50/month. This is well below the standard rate for this kind of service. The current arrangement may end this year,requiring a new home for the CoHID web equipment.New technologies on the immediate horizon may make$100/month enough to maintain a presence on the web via a high speed connection. Request total w/out Agency Indirect 114.335.00 Agency Indirect 17,041.00 Agency Indirect is added in as 15.17%of the grant request(without agency indirect). This is an rate negotiated with the state for contracts of this type by each county to cover overhead costs incurred by the hosting county. This is the current rate for Weld County's Health Education Division. Pickle, Director Weld County Health Department RE: CoHID Grant Proposal Enclosed for Board review and approval is a proposal for a Public Health and Health Services Block Grant for the continuation of the Colorado Health Information DataSet project, located at the Weld County Health Department. This proposal, if granted, will allow this program to continue at Weld County and, if fully funded, will provide funding for an Epidemiologist for Weld County Health Department. While it is likely that this program will eventually move to the Colorado Department of Public Health and Environment, its presence here at Weld County has benefitted Weld County Health Department by providing immediate access to vital health data and technical expertise to other health department projects. The high profile of this program throughout the state has enhanced Weld County's reputation as an innovator in public health. Should the proposal be accepted without revision, the Weld County Health Department will receive a sum not to exceed $131,396.00.
