Re: Agreement to Establish Physician Advisor Services/Platte Valley Fire District
Commissioners:
North Colorado Emergency Physicians, P.C. ("NCEP") and Weld County Medical
Service Providers/Fire Districts ("Fire Districts") are parties to an Agreement to Establish
Physician Advisor Services (the "Agreement"). Under the terms of the Agreement, NCEP
provides professional services regarding emergency medical technician training, oversight and
recertification for the Platte Valley Fire District, among others.
Pursuant to Sections 9(D) and 10 of the Agreement, notice is hereby given that NCEP
is exercising its right to terminate the Agreement as it relates to NCEP's obligation to provide
professional services to the Platte Valley Fire District pursuant to the terms of the Agreement.
Such termination shall be effective sixty (60) days from the date of this notice.
NCEP intends to fully honor and comply with the terms of the Agreement as it relates
to all other participating medical service providers and fire districts. All such entities are
deemed notified of the above-described termination/modification of the Agreement by virtue of
the Weld County Board of Commissioners' status as each entity's agent for service of such
notice, pursuant to Section 10 of the Agreement.
Pursuant to Section 3(A)(4) of the Agreement, this notice shall be transmitted to the
Colorado Board of Medical Examiners as notification that the Platte Valley Fire District is no
longer being supervised by NCEP for purposes of EMT certification required under Colorado
Revised Statute §25-3.5-203.
