COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT AND
AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with two Requests for Proposal for Prevention
of HIV, including Community Identification Process and Group Risk-Reduction Education, from the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Weld County Health Department, to the Colorado Department of Public
Health and Environment, with terms and conditions being as stated in said proposals, and
WHEREAS, after review, the Board deems it advisable to approve said requests, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the two Requests for Proposal for Prevention of HIV, including Community
Identification Process and Group Risk-Reduction Education, from the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Health Department, to the Colorado Department of Public Health and Environment
be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized
to sign said requests.

WELD COUNTY REQUEST FOR PROPOSALS
PREVENTION OF HIV
COMMUNITY IDENTIFICATION PROCESS
A. CONTENT
1. DESCRIPTION OF ORGANIZATION: The Weld County Health Department
(WCHD)is a public health agency providing services to residents of Weld County from Erie to the
Wyoming border. The agency has satellite sites outside of Greeley in 15 locations, allowing the
provision of monthly outreach and home visitation to the most remote areas of the county. The
Department's community health services range from well child check-ups to well oldster clinics,
fielding over 10,000 client visits per year. Specific to the scope of this project, WCHD provides STD
screening and prevention services and began offering HIV services and community education in the
1980's through school policy and a CTS site.
At this time, WCHD offers HIV screening at several satellite sites around the county(including the
University of Northern Colorado, Fort Lupton, and a local outpatient treatment center for addicted
teens) and houses the Northern Colorado Regional Coordinator position providing case managed
services for people with AIDS in the area. Beginning in the fall of 1995, WCHD will become the
third ATS in the state. We anticipate the number of high risk individuals utilizing this service will
increase by a significant amount, thus increasing WCHD's access to this population for outreach and
education.
WCHD plays a lead role in coordinating community awareness projects as the Co-Chair of the Weld
County AIDS Coalition (WCAC). Additionally, WCHD maintains membership in the Northern
Colorado AIDS Project (NCAP). The HIV education program works with area treatment centers,
residential programs for youth at risk and homeless women, family planning service providers and
county correctional facilities to provide HIV risk reduction education and to promote the
development of HIV policy.
The Health Promotion Section of WCHD, which houses the HIV education program, is strongly
committed to community involvement in program planning. The section has received grants from
Colorado Action for Healthy People, March of Dimes, and the Kellogg Foundation to work with
small rural communities in performing assessment and developing community appropriate prevention
programs. The section also houses a Health Data Specialist responsible for the collection and
evaluation of critical health indicator data at the local and state level_
Because of the services WCHD provides, we are in an excellent position to provide outreach to youth
at high risk, high risk women, and individuals with multiple partners. Additionally, our relationship
with the two community health centers in Weld County and the Monfort Family Planning/Prenatal
program have allowed us to develop networks with the Migrant Seasonal Farmworker population.
We have been pleased with the results of these efforts to date. Despite offering HIV services, we
continue to feel that we are less able to identify and reach community members who are using
injection drugs and men having sex with men (including men of color). It is for this reason that the
Community Identification Process is important to the Weld County community.

2. TARGET POPULATION: For the last several years, WCI-ID as been operating on
assumptions about the target population in this community. It is becoming increasingly clear that
these assumptions are misconstrued and are no longer serviceable. WCHD will seek to serve the
highest priority populations, as defined by CWT, in Weld County. These will be: Women at Risk,
IDUs and Their Partners, Runaway/Displaced Youth Who Trade Sex for Survival, and Migrant
Seasonal Farm Workers. The target population for this project will include diverse groups of
individuals and agencies who are committed to improving awareness of the ways to avoid HIV/AIDS
infection in Weld County. This diversity offers both a challenge and opportunity to enhance
collaboration between WCHD and agencies in HIV/AIDS prevention strategies.
Women of child bearing age at risk comprise a risk group for which we have exceptionally high hopes
of developing future interventions. Women have primarily been infected through drug abuse or by
being sex partners of drug abusers. Women of child bearing age have the potential of becoming
infected themselves and of infecting any children they might bear. According to 1993 data, the live
birth rate for Weld County was 16.9 and the fertility rate was 69.4 which higher than the state
average. Many women are unaware that they are at risk for HIV infection and HIV infected women
are often undiagnosed until they become ill or bear an infected ill child. HIV education and
prevention needs to be a part of every woman's self-care. The incidence of AIDS and HIV infection
has been greater among Hispanics and Blacks than any other ethnic minority groups. 38% of Weld
County's population is Hispanic, a number of whom are part of the transient migrant working
population.
IV drug use is the second most common transmission of the HIV virus. Drug users who engage in
unprotected sexual activity may infect their sexual partners, contributing to the spread of the virus
of the non-drug using population.
Use of alcohol and non-injection drugs is associated with sexual risk taking. All alcohol and drug
treatment programs are expected to help prevent the spread of HIV/AIDS.
In Weld County 13.2% of HIV tests are IDUs or sex partner's of IDUs. Of those tested for HIV in
1993 in Weld County, 18% admitted to IDU or having a sex partner that was an IDU. Weld County's
Health Data Specialist emphised that this data is based on self-collected data which means these
numbers are actually much lower than probably true. It is believed that because WCHD has a CTS,
many high risk individuals were testing at sites outside of Weld County.
The geographical community targeted is Weld County. Weld County ranks as Colorado's third
largest geographical county with 4033 square miles and it is eighth largest in terms of population.
WCHD HIV Prevention Program Coordinator will seek out key informants within the high target
populations. These leaders will participate and/or recruit other leaders of these groups, to be
involved in focusgroups and other CIP strategies.
Agencies who provide services to target populations will also be involved in the CIP process. These
agencies will provide additional insight as how to increase awareness to high target population, what
areas of HIV prevention are lacking, and how to improve and more efficiently provide already

existing education. By involving these agencies it is expected that new relationships will evolve and
existing ones will become stronger inspiring more collaborative efforts. Agencies in the county which
will be contacted include by are not limited to: substance abuse programs such as; Island Grove
Outpatient and Residential Treatment Centers and Alpha; Weld County HIV Resource Coordinator
and their clients; WCAC; Interfaith Support Group; Hospice of Northern Colorado; NCAP; UNC
Cultural Centers; UNC Women's Center; Sunrise Community Health Center; Salud; other
healthcare providers; UNC LGBA; correctional facilities including: Restitution Center; new
juvenile detention center and the Weld County Jail; Transitional House; A Woman's Place: and
other key informants who emerge during the CIP process.
Interagency and community collaboration are strong components of service delivery in Weld County.
Collaborative efforts made over time have stemmed in large part from the fact that Weld County is
a rural community with a relatively sparse and diffuse population and a scarcity of providers.
WCHD has provided general HIV prevention to residence of the county for seven years. WCHD has
provided educational materials including pamphlets, brochures, and videos.
As the major provider of HIV services, WCHD's CTS Resource Coordination Mangers both
expressed frustration with not knowing the exact number of people in high target populations or how
to go about accessing the high target populations. CIP will be utilized to identify access points to
derive information about the target population in Weld County
3. OBJECTIVES: The interventions WCHD has taken are all steps toward reaching
objectives as described in Weld County Health Objectives for the Year 2000 Public Planning
Document. By December 31, 1996, WCHD will complete a community identification process among
residents of Weld County. This process will be completed for the purpose of targeting HIV education
and prevention program resources.
To accomplish this objective, the WCHD HIV education program will:
1. Review Weld County indicator data to determine the prevalence and distribution
of HIV in Weld County by November 31, 1995.
2. Facilitation of focus groups and key informant interview (at least two of each) will
have been held by 10-31-95_ The purpose of these activities is to identify major issues
and other key players within high target populations. Initial results will be used to
develop an indept CIP plan.
WCHD will identify agencies and key informants to most appropriately represent high
target populations of Weld County in the CIP process. These agencies and
individuals will include: Weld County HIV Resource Coordinator clients, substance
abuse programs as described in #2 TARGET POPULATIONS, Sunrise Community

Health Center, WCAC, NCAP, UNC Cultural Centers, UNC LGBA, HIV support
roups, Salud, A Women's Place, STD clinics, Monfort, Planned Parenthood etc.
High target population opinion leaders will be trained to lead focus groups and key
informant interview. This training will allow CIP to be lead by community
representatives.
3. Host joint meeting of providers of services to migrant seasonal farm workers to
identify HIV prevention needs and issues by October 31, 1995. Outcomes of this
meeting will be used to develop a coordinated plan for migrant seasonal worker HIV
prevention services in 1996.
4. OPERATIONAL PLAN: The following steps will be done to insure that the objective
for CIP from September to December 1995 is accomplished.

5. EVALTIATION PLAN: Because of the work to be accomplished during the funding
period, -evaluation will-be primarily of process, measuring whether each activity was completed
correctly-and on time. Additionally, the process evaluation creates a mechanism where by community
involvement in the process will be insured.
Accomplishment of strategy one will be measured by the completion of the indicator review and the
completion of draft recommendations. The summery-report will be submitted to CDPHEI-IIV/STD
Program at the end of the 1-995 funding period.
Successful completion of strategy-two will be measured by completion of the two key informant
interviews and two focus groups. Results of the assessment and recommendations will be submitted
to CDPI-IE HIV/STD Program at the end of the 1995 funding period. Recommendations for these
interviews and participation in them will be used to-further develop community involvement in the
CIP process-

Successful completion of strategy three will be measured by the completion of a strategic planning
effort between agency providers of migrant health services. Ideally the development of a coalition
of providers completing an HIV prevention plan will result. Outcome of this collaborative effort will
be reported to CDPHE HIV/STD Program in the end of the funding period report. Additionally this
• outcome will be used to develop a comprehensive HIV prevention plan for Weld County migrant
seasonal farm workers.
6. BUDGET: WCI D is asking for a total of $5395.35 for the period of this contract,
September to December 31, 1995, to help facilitate the CIP process in Weld County.

GROUP RISK-REDUCTION EDUCATION
A. CONTENT
1. DESCRIPTION OF ORGANIZATION: The Weld County Health Department

women pers year, 25%under the age of 20. The Prenatal_Program serves 300 women per year, 25%
to30%of whom are teens. The Monfort Family Planning/Penatal program serves an estimated 600
women-per year, most of whom-are-Hispanic.
At this time, WCHD offers HIV screening at several satellite sites around the county(including-the
University of Northern Colorado,Fort Lupton, and a local outpatient treatment center for addicted

Women in high risk situations will include (for the purpose of this-project) women in-transitional
housing, women at the battered women' shelter, women in the Victim Assistance Program, women
attending the University of Northern Colorado (UNC), women in the prenatal program Helping
Moms in Weld County-clinics, and women in Weld County correctional facilities.
3. OBJECTIVE(S): By the year 2000, WCHD expects to reduce self reported risk taking
behavior among all Weld County women at risk in the following two areas:
• Increase to al least 30_percent the proportion of sexually active,
unmarried people who use a condomat least sexual intercourse.
• Increase to -at least 80 percent the proportion of HIV infected
people who have been tested for HIV infection.

These objectives will be accomplished by the following strategies:
• Provide educational material which is culturally relevant and of an appropriate
literacy level to at risk maternity clients in Weld County.
• Increase community health nurse sense of efficiency in HIV risk education
through-staff-training.
• Develop-educational program(s) for women incarcerated in the Weld County
jail.

5. EVALUATION PLAN: The project coordinator will track and report the following
information for women in high risk situations:
• Program activities and services provided.
• Demographics of the participants and the specific presentations provided.
• When, how often, and where the presentations were provided.
• Program success, unmet needs, barriers and problems with the program.
• Number of-brochures and condoms distributed.
• Evaluate the effectiveness of the presentations in raising participants
knowledge by-means of pre and post-test.
A summary of all these activities will be provided, as has in the last three quarters of the contract.
A fourth quarter HIV Progress tracking report will be issued to the CDPHE STD/AIDS Program.

6. BUDGET: The Weld County is asking for a total of$4228.57 for the period of this contract,
October to Demember 31, 1995 to continue HIV prevention education to women at high risk of
becoming infected with HIV.
